Äåìîäåêîç (demodicosis)

Î÷åíü âàæíûé è ñïîðíûé âîïðîñ â äåðìàòîëîãèè ñâÿçàí ñ îáíàðóæåíèåì êëåùà äåìîäåêñà íà êîæå ëèöà, âåê, à òàêæå ïîêà åùå ñ íåîïðåäåë¸ííîé åãî ðîëüþ â âîçíèêíîâåíèè êàêîãî-ëèáî ïàòîëîãè÷åñêîãî ïðîöåññà.
Äåìîäåêîç(demodicosis)- îáùåå ñîñòîÿíèå(äàæå ñ òî÷êè çðåíèÿ êëèíè÷åñêîé òåðìèíîëîãèè ëàòèíñêîãî ÿçûêà), îáîçíà÷àþùåå îáíàðóæåíèå íà êîæå ÷åëîâåêà êëåùà Demodex (folliculorum èëè
Demodex brevis). Äàííîå ñîñòîÿíèå(äåìîäåêîç, äåìîäèêîç)- íåëüçÿ íàçâàòü ïàòîëîãèåé, òàê êàê äî ñèõ ïîð íè â îäíîì èññëåäîâàíèè íå îïðåäåëåíà ðîëü ýòîãî êëåùà â ïàòîãåíåçå çàáîëåâàíèÿ.  îñíîâíîì â ñîâðåìåííîé ìåäèöèíå èññëåäîâàíèÿ ñâîäÿòñÿ ê êîëè÷åñòâåííîé îöåíêå îáíàðóæåíèÿ ýòîãî êëåùà íà êîæå çäîðîâûõ ëþäåé è ëþäåé ñ ðàçëè÷íûìè ïîðàæåíèÿìè êîæè, âîëîñÿíûõ ôîëëèêóëîâ è ñëèçèñòûõ (ðîçàöåà, áëåôàðèòû, ôîëëèêóëèòû è äð.). Íàëè÷èå íà êîæå ó çäîðîâûõ ëþäåé áåç êàêèõ ëèáî èçìåíåíèé êîæè êëåùà Demodex íå ïîçâîëÿåò îäíîçíà÷íî óñòàíîâèòü åãî êàê ïåðâîïðè÷èíó êàêîãî-ëèáî çàáîëåâàíèÿ. Îäíàêî èìååò çíà÷åíèå òîò ôàêò, ÷òî âñå-òàêè ó ïàöèåíòîâ ñ òàêèìè çàáîëåâàíèÿìè êàê áëåôàðèò, ðîçàöåà, ïàðàçèòàðíûé ôîëëèêóëèò îí îáíàðóæèâàåòñÿ ñ äîñòîâåðíî áîëüøåé ÷àñòîòîé, ÷åì íà íåèçìåíåííîé êîæå ëèöà, ÷òî ïîçâîëÿåò èñïîëüçîâàòü â ëå÷åíèè(êîìáèíèðîâàííî ñ äðóãèìè ñðåäñòâàìè) ïðåïàðàòû ïðîòèâ êëåùà.
Ìîæíî ñäåëàòü âûâîä, ÷òî ó÷èòûâàòü åãî îáíàðóæåíèå ìîæíî è èñïîëüçîâàòü ñîîòâåòñòâóþùåå ëå÷åíèå(îñîáåííî ðåêîìåíäóåòñÿ ïðè áëåôàðèòàõ-âîñïàëèòåëüíûõ èçìåíåíèÿõ âåê), íî òîëüêî â ñëó÷àå åãî îáíàðóæåíèÿ è èñêëþ÷åíèÿ äðóãèõ ïàòîëîãèé. Íåîáõîäèìî ó÷èòûâàòü è äðóãèå ñîñòàâëÿþùèå ïàòîãåíåçà òàêèõ çàáîëåâàíèé êàê ðîçàöåà, ôîëëèêóëèòû è ò.ä., ïðè êîòîðûõ ýòîò êëåù íåðåäêî îáíàðóæèâàåòñÿ è
íå ñòîèò íåîáîñíîâàííî âûäåëÿòü â ñàìîñòîÿòåëüíîå çàáîëåâàíèå(äåìîäåêîç), ïðè êîòîðîì Demodex ÿâëÿåòñÿ ýòèîëîãè÷åñêèì ôàêòîðîì.

"Demodex folliculorum and Demodex brevis are acari that can be found in hair follicles and sebaceous glands of the skin, especially on face of humans. In this study, Demodex sp was investigated in regard to allergic diseases, age and gender. A total of 197 patients (117 with rosacea, 29 with akne vulgaris, and 51 with allergic diseases) were examined using the standardized skin surface biopsy (SSSB) and 97 out of 197 (49.23%) cases were found to be positive by the Inonu University Medical Faculty Department of Parasitology. There was no significant difference between mite positivity and negativity between the genders, while a higher rate of Demodex sp. was found in patients with rosacea and a lower rate in patients under 20 years old (p0,005). As a result, patients over 20 years old, especially those with rosacea, must be investigated for Demodex sp."

BACKGROUND: Demodex mites are common commensal organisms of the pilosebaceous unit in human beings and have been implicated in pityriasis folliculorum, rosacea-like demodicosis, and demodicosis gravis. OBJECTIVE: We sought to describe the spectrum of clinicopathological findings and therapeutic responses of demodicosis in Taiwanese patients. METHODS: We conducted a retrospective study to review clinicopathologic findings and therapeutic responses of 34 cases of diagnosed demodicosis. RESULTS: Fifteen cases with positive results of potassium hydroxide examination, standardized skin surface biopsy specimen, and/or skin biopsy specimen, and resolution of skin lesions after anti-Demodex treatment were included for final analysis. Nineteen cases were excluded because of insufficient positive data to make a definite diagnosis. There were 4 male and 11 female patients (age 1-64 years, mean age 38.7 years). The disease was recurrent or chronic with a duration ranging from 2 months to 5 years (mean 15.7 months). The skin lesions were acne rosacea-like (n = 8), perioral dermatitis-like (n = 5), granulomatous rosacea-like (n = 1), and pityriasis folliculorum (n = 1). Skin biopsy was performed in 7 patients. Overall, the histopathology was characterized by: (1) dense perivascular and perifollicular lymphohistiocytic infiltrates, often with abundant neutrophils and occasionally with multinucleated histiocytes; (2) excessive Demodex mites in follicular infundibula; and (3) infundibular pustules containing mites or mites in perifollicular inflammatory infiltrate. The skin lesions resolved after treatment including systemic metronidazole, topical metronidazole, crotamiton, or gamma benzene hexachloride. LIMITATIONS: Small sample size and a fraction of patients without long-term follow-up are limitations. CONCLUSION: Demodicosis should be considered in the differential diagnosis of recurrent or recalcitrant rosacea-like, granulomatous rosacea-like, and perioral dermatitis-like eruptions of the face. Potassium hydroxide examination, standardized skin surface biopsy, skin biopsy, or a combination of these are essential to establish the diagnosis.

Retrospective Analysis of the Association Between Demodex Infestation and Rosacea

Conclusions: A significant association exists between Demodex infestation and the development of rosacea. Demodex infestation is a vital risk factor for rosacea according to the time-to-event relationship, and the degree of infestation played a more important role than did the mite infestation rate in the development of rosacea.

Facial dermatosis associated with Demodex: a case-control study [Ññûëêè äîñòóïíû òîëüêî çàðåãèñòðèðîâàííûì ïîëüçîâàòåëÿì ]
Demodex has been considered to be related with multiple skin disorders, but controversy persists. In this case-control study, a survey was conducted with 860 dermatosis patients aged 12 to 84 years in Xi’an, China to identify the association between facial dermatosis and Demodex. Amongst the patients, 539 suffered from facial dermatosis and 321 suffered from non-facial dermatosis. Demodex mites were sampled and examined using the skin pressurization method. Multivariate regression analysis was applied to analyze the association between facial dermatosis and Demodex infestation, and to identify the risk factors of Demodex infestation. The results showed that total detection rate of Demodex was 43.0%. Patients aged above 30 years had higher odds of Demodex infestation than those under 30 years. Compared to patients with neutral skin, patients with mixed, oily, or dry skin were more likely to be infested with Demodex (odds ratios (ORs) were 2.5, 2.4, and 1.6, respectively). Moreover, Demodex infestation was found to be statistically associated with rosacea (OR=8.1), steroid-induced dermatitis (OR=2.7), seborrheic dermatitis (OR=2.2), and primary irritation dermatitis (OR=2.1). In particular, ORs calculated from the severe infestation (?5 mites/cm2) rate were significantly higher than those of the total rate. Therefore, we concluded that Demodex is associated with rosacea, steroid-induced dermatitis, seborrheic dermatitis, and primary irritation dermatitis. The rate of severe infestation is found to be more correlated with various dermatosis than the total infestation rate. The risk factors of Demodex infestation, age, and skin types were identified. Our study also suggested that good hygiene practice might reduce the chances of demodicosis and Demodex infestation.
