Интересно – у меня нет полного текста данной статьи. В той ссылке на абстракт статьи, которую я привел я вообще не вижу цифры 62%. Там написано - The incidence of bilateral clinical CTS in our population was 87% - и это общий а не промежуточный вывод, по моему достаточно однозначно трактуемый.
Кроме того, обратите внимание что те исследования которые Вы и я приводим были направлены на решение принципиально разных задач (и кстати проводились одной и той же группой.- Italian CTS Study Group), то иссл. которое я привел ставило задачу выявить % билатеральности СЗК в т.ч. и асимптомного – результат 87%. Ваша ссылка The Italian CTS Study Group studied 740 patients (a total of 1,123 hands with CTS, 383 affected bilaterally)[50] in 20 centers distributed throughout Italy Данное мультицентровое исследование решало совсем другую задачу, а именно сравнение нейрофиз. и клинических данных у мужчин и женщин с идиопатическим СЗК а не % билатеральных СЗК в популяции, поэтому сравнивать его с тем, что я привел не совсем корректно. Что касается . Heidi Michelsen, Martin A. Posner.Medical history of carpal tunnel syndrome //Hand Clin 18 (2002) 257–268 – не нашел ни количества пациентов исследованных, ничего (может у Вас есть информация) – я так понимаю, что это что-то вроде обзора а не оригинальное исследование? |

и вопрос если можно. какому примерно % пациентов из обратившихся к Вам, которым Вы поставили диагноз СЗК, Вы проводите хир. лечение?

Не знаю, не считал. По показаниям больному назначаю либо консервативное лечение ( правда сам его не провожу, но советую), либо операцию. А больной уже сам решает, что делать. Насильно же не заставишь лечиться. Кто придет, того и лечим.

Привожу давно написанный ответ рецензенту одного из журналов, который тоже возмущался моим отношением к электродиагностике. Если не устроят эти ссылки, то поищу более новые, где-то они у меня "гуляют". Итак.
Отношение к электродиагностическим исследованиям при СКЗ – очень неоднозначное из-за фальш-позитивных и фальш-негативных результатов. К примеру, в статье Braun RM, Jackson NJ. Electrical studies as a prognostic factor in the surgical treatment of carpal tunnel syndrome. J Hand Surg 1994; 19A: 895 приводятся данные о результатах операций при СКЗ, где у 50 прооперированных пациентов (33% от всех прооперированных больных) не было патологических электродиагностических изменений. То есть по данным электродиагностики они были здоровы, хотя имели выраженные клинические проявления СКЗ ( почему и были прооперированы). Еще ссылки. 1. Baltimore:Williams&Wilkins, 1990: 522 –“Electrodiagnostic studies are particularly helpful in diagnosing questionable or borderline cases but are not required for every patient. … False-negative results are reported in up 20% of patients”. 2. It may still be useful in cases where the clinical diagnosis is not clear». 3. Finsen V. Russwurm H,. Neurophysiology not required before surgery for typical carpal tunnel syndrome. J Hand Surg. 2001; 26B: 61-64.-“We conclude that neurophysiological studies contribute little to the diagnosis in typical cases of carpal tunnel syndrome, and are more often confounding than of assistance”. По поводу "сдохших мышц" - это насчет другой патологии и другой темы, с которой мне предложила ознакомиться моя оппонентка. |

В статье об этом сообщается следующее, в таблице 2: “Group 3 ( 151 patients) - …. Symptoms moderate to severe and impair productivity”. Это основная группа, которая и исследоваась.
Больные с атрофией тенара в эту группу из 151 человека включены не были. Цитата:

По-моему, слишком много уже сказано про СЗК. Может попробовать обсудить другие варианты, например, синдром Зудека или еще что-нибудь?
