ÍÆÝ. Ëó÷øå áû áûëî ìåíüøå 76-100 â ñóòêè, ò.ê. åñëè áîëüøå, òî õóæå ïðîãíîç.
ÆÝ. Ëó÷øå áû èõ íå áûëî. Åñëè óæ åñòü, òî ëó÷øå ÷òîáû ìåíüøå 10% îò âñåõ ñîêðàùåíèé èëè 10 000 çà ñóòêè è ÷òîáû íå áûëî ïðîáåæåê íåóñòîé÷èâîé ÆÒ. Õóæå äëÿ ïðîãíîçà, åñëè åñòü áîëåçíü ñåðäöà.

Îáñëåäîâàíèå. Îïðîñ æàëîá, àíàìíåçà, îñìîòð, ÝÊÃ, Õîëòåð, ÝõîÊÃ, ïîèñê ïðè÷èííîãî çàáîëåâàíèÿ ñåðäöà è/èëè ïðîâîöèðóþùåãî ôàêòîðà, ëàáîðàòîðíûå èññëåäîâàíèÿ: ýëåêòðîëèòû (Ê, Mg), êëèíè÷åñêèé àíàëèç êðîâè (àíåìèÿ), ÒÒÃ, BNP (ñåðäå÷íàÿ íåäîñòàòî÷íîñòü), ãàçîâûé ñîñòàâ êðîâè, òåñò íà íàðêîòèêè, îïðåäåëåíèå óðîâíÿ äèãîêñèíà (ïðè åãî ïðèåìå); íàãðóçî÷íûé ÝÊà òåñò îòäåëüíûì ïàöèåíòàì. ÝÔÈ â ðàìêàõ äèàãíîñòèêè ÆÝ ïðîâîäèòü ðóòèííî íå ðåêîìåíäóåòñÿ.

Ëå÷åíèå.

Öåëè ëå÷åíèÿ – 1. Ïðîäëåíèå æèçíè, 2. Óëó÷øåíèå êà÷åñòâà æèçíè. Óëó÷øåíèå êà÷åñòâà æèçíè íå äîëæíî ñîêðàùàòü ïðîäîëæèòåëüíîñòü.

1. Òåðàïåâòè÷åñêîå âîçäåðæàíèå (ò.å. íàáëþäåíèå, à ëå÷åíèå = 0) ïðè áåññèìïòîìíûõ Ý ó îòíîñèòåëüíî çäîðîâûõ ëèö

2. Ëå÷åíèå îñíîâíîãî / ïðè÷èííîãî çàáîëåâàíèÿ (ñåðäå÷íûå è âíåñåðäå÷íûå)

3. Ñíèæåíèå ñåíñèòèâíîñòè (âîñïðèÿòèÿ), ò.å. ïñèõîòåðàïèÿ è/èëè ïñèõîôàðìàêîòåðàïèÿ, íî ïîìíèì ïðî QT – ìíîãèå ïñèõîòðîïíûå ïðåïàðàòû åãî óäëèíÿþò* è òàêèõ ëó÷øå èçáåãàòü ïðè àðèòìèÿõ)

vi. Íàäîëîë 40 ìã ïåðîðàëüíî 1 ðàç â äåíü; ìàêñèìàëüíàÿ äîçà 160 ìã â äåíü; òåðàïèÿ ïåðâîãî âûáîðà ïðè íåêîòîðûõ êàíàëîïàòèÿõ (â ò.÷. ïðè ñèíäðîìå óäëèíåííîãî QT).
vii. Áåòàêñîëîë 5 ìã ïåðîðàëüíî 1 ðàç â äåíü; ìàêñèìàëüíàÿ äîçà 20 ìã 1 ðàç â äåíü
viii. Ïðîïðàíîëîë 10 ìã ïåðîðàëüíî 2 èëè 3 ðàçà â äåíü (àëüòåðíàòèâíî ïðîïðàíîëîë äëèòåëüíîãî äåéñòâèÿ 80 ìã ïåðîðàëüíî 1 ðàç â äåíü); ìàêñèìàëüíàÿ äîçà 320 ìã â äåíü; òåðàïèÿ ïåðâîãî âûáîðà â ïåäèàòðè÷åñêîé ãðóïïå, ïðè íåêîòîðûõ êàíàëîïàòèÿõ è ãèïåðòèðåîçå.

*Íå èìååò êà÷åñòâåííûõ äîêàçàòåëüñòâ åãî ïðîãíîñòè÷åñêîé ýôôåêòèâíîñòè ïðè æåëóäî÷êîâûõ àðèòìèÿõ.


b. Íåäèãèäðîïèðèäèíîâûå êàëüöèåâûå áëîêàòîðû (ìîãóò áûòü ïðåäïî÷òèòåëüíûìè ó ïàöèåíòîâ ñ ÀÃ èëè ó ïàöèåíòîâ ñ ôàñöèêóëÿðíûìè ÆÝ, íî íå äîëæíû ïðèìåíÿòüñÿ ó ïàöèåíòîâ ñ ÑÍ èëè êàðäèîìèîïàòèåé)
i. Äèëòèàçåì 60 ìã ïåðîðàëüíî 2 èëè 3 ðàçà â äåíü; èëè àëüòåðíàòèâíî, äèëòèàçåì ïðîëîíãèðîâàííîãî âûñâîáîæäåíèÿ: îò 120 äî 180 ìã ïåðîðàëüíî 1 ðàç â äåíü; ìàêñèìàëüíàÿ äîçà 480 ìã â äåíü.
ii. Âåðàïàìèë 80 ìã ïåðîðàëüíî 3 ðàçà â äåíü; àëüòåðíàòèâíî, âåðàïàìèë ïðîëîíãèðîâàííîãî âûñâîáîæäåíèÿ: 240 ìã ïåðîðàëüíî 1 ðàç â äåíü; ìàêñèìàëüíàÿ äîçà 480 ìã â äåíü.
5. Ñàìîêîíòðîëü ÝÊà (ðàçëè÷íûìè äåâàéñàìè – óìíûå ÷àñû, ñïåöèàëüíûå ïîðòàòèâíûå êàðäèîãðàôû, òåëåìåäèöèíà) âî âðåìÿ ñèìïòîìîâ óëó÷øàåò êà÷åñòâî æèçíè è ñíèæàåò òðåâîæíîñòü. Íî ÷ðåçìåðíûé ñàìîêîíòðîëü ñïîñîáåí ïðèâåñòè è ê îáðàòíîìó ýôôåêòó (ïîâûøåíèþ òðåâîæíîñòè è äåïðåññèè).

6. Àíòèàðèòìèêè äðóãèõ ãðóïï – ïîìíèòü ïðî íå íàâðåäè! Ñàìûõ ïðîàðèòìîãåííûõ (ñîòàëîë, äðîíåäàðîí) ëó÷øå âñÿ÷åñêè èçáåãàòü! Ïîìíèòü î ãëîáàëüíîé öåëè ëå÷åíèÿ (ïðîäëåíèå êà÷åñòâåííîé æèçíè), ò.å. áåçîïàñíîñòü âî ãëàâå óãëà. Ïðåçóìïöèÿ âèíîâíîñòè: íåèçó÷åííûé àíòèàðèòìèê ñ÷èòàòü ïîòåíöèàëüíî îïàñíûì ïîêà íå äîêàçàíû åãî áåçîïàñíîñòü è ýôôåêòèâíîñòü. Ïðèìåíåíèå àíòèàðèòìè÷åñêèõ ïðåïàðàòîâ (íàïðèìåð, ôëåêàèíèäà è äðóãèõ àíòèàðèòìè÷åñêèõ ïðåïàðàòîâ êëàññà IÑ*) ïðè ÆÝ íå ðåêîìåíäîâàíî, ò.ê. ìíîãèå èç íèõ ñâÿçàíû ñ ïîâûøåííîé ñìåðòíîñòüþ èç-çà ïðîàðèòìèè ïðè íàçíà÷åíèè ïàöèåíòàì ñ ïðåäøåñòâóþùèì ÈÌ.
a. Ïàöèåíòû ñî ñòðóêòóðíûìè çàáîëåâàíèÿìè ñåðäöà:

i. Àìèîäàðîí ÿâëÿåòñÿ íàèìåíåå ïðîàðèòìè÷åñêèì ñðåäñòâîì ó ïàöèåíòîâ ñî ñòðóêòóðíûìè çàáîëåâàíèÿìè ñåðäöà, õîòÿ è ñî çíà÷èòåëüíîé èçâåñòíîé òîêñè÷íîñòüþ äëÿ îðãàíîâ ïðè äëèòåëüíîì ïðèìåíåíèè. Ìîæíî èñïîëüçîâàòü ïåðîðàëüíûå íàãðóçî÷íûå äîçû îò 400 äî 1200 ìã/ñóò â ðàçäåëåííûõ äîçàõ (äî îáùåé íàãðóçî÷íîé äîçû îò 6 äî 10 ãðàìì). Îáû÷íàÿ ïîääåðæèâàþùàÿ äîçà äîëæíà áûòü ñàìîé íèçêîé ýôôåêòèâíîé äîçîé, êîòîðàÿ îáû÷íî ñîñòàâëÿåò 200 ìã â äåíü, íî èíîãäà ìîæåò äîñòèãàòü 100 ìã â äåíü.

ii. Ðàíîëàçèí, ïåðîðàëüíûé àíòèàíãèíàëüíûé ïðåïàðàò, îáëàäàþùèé ñâîéñòâàìè, áëîêèðóþùèìè I Na è I Kr, îäîáðåí òîëüêî äëÿ ëå÷åíèÿ õðîíè÷åñêîé ñòàáèëüíîé ñòåíîêàðäèè; îäíàêî áûëî ïîêàçàíî, ÷òî îí ñíèæàåò áðåìÿ æåëóäî÷êîâûõ àðèòìèé â èññëåäîâàíèè MERLIN-TIMI 36 áåç îïàñåíèÿ êàêîé-ëèáî çíà÷èòåëüíîé ïðîàðèòìèè. Íà÷àëüíàÿ äîçà 500 ìã ïåðîðàëüíî 2 ðàçà â äåíü; ìàêñèìàëüíàÿ ñóòî÷íàÿ äîçà 1000 ìã 2 ðàçà â äåíü.

*  åâðîïåéñêîì ðóêîâîäñòâå 2022 ïî ëå÷åíèþ æåëóäî÷êîâûõ àðèòìèé è ïðîôèëàêòèêå âíåçàïíîé ñìåðòè ñðåäè ðåêîìåíäàöèé ÍÅÒ ïðåïàðàòîâ 1Ñ êëàññà
7. Ð×À – äàííûõ äëÿ ïîíèìàíèÿ ïðîãíîñòè÷åñêîé ïîëüçû íåäîñòàòî÷íî.
a. Ð×À ïðè ÍÆÝ ïðè äîêàçàííîé ïðîâîöèðóåìîñòè ÔÏ.

b. Ð×À ïðè ÆÝ. Åñëè áåòà-áëîêàòîðû è / èëè íåäèãèäðîïèðèäèíîâûå áëîêàòîðû êàëüöèåâûõ êàíàëîâ íåýôôåêòèâíû èëè íåæåëàòåëüíû, à òàêæå äëÿ ïàöèåíòîâ, ó êîòîðûõ ñîõðàíÿþòñÿ ñèìïòîìû, ñâÿçàííûå ñ ÆÝ, íåñìîòðÿ íà àíòèàðèòìè÷åñêóþ ëåêàðñòâåííóþ òåðàïèþ, èëè êîòîðûå ïðåäïî÷èòàþò íå ïðèíèìàòü àíòèàðèòìè÷åñêóþ ëåêàðñòâåííóþ òåðàïèþ, êàòåòåðíàÿ àáëÿöèÿ ÿâëÿåòñÿ ýôôåêòèâíûì âàðèàíòîì äëÿ óìåíüøåíèÿ èëè óñòðàíåíèÿ ÆÝ. Ïàöèåíòàì ñ ïîäîçðåíèåì íà ÆÝ-èíäóöèðîâàííóþ êàðäèîìèîïàòèþ è ñíèæåííûì ÔÂËÆ (< 50%) ñëåäóåò ïðåäëîæèòü êàòåòåðíóþ àáëÿöèþ, äàæå äëÿ ïàöèåíòîâ ñ èìïëàíòèðîâàííûì êàðäèîâåðòåðîì-äåôèáðèëëÿòîðîì, ñ öåëüþ óëó÷øåíèÿ è, âîçìîæíî, íîðìàëèçàöèè ôóíêöèè ËÆ. Êëèíè÷åñêèå ïåðåìåííûå, êîòîðûå èäåíòèôèöèðóþò ïàöèåíòà, êîòîðûé, ñêîðåå âñåãî, ïîëîæèòåëüíî îòâåòèò íà êàòåòåðíóþ àáëÿöèþ, âêëþ÷àþò:

i. Ìîíîôîêóñíûå ÆÝ.
ii. ×àñòûå ÆÝ (ò.å. > 10 000 çà 24 ÷àñà èëè > 10%-15%; åäèíîãî ìíåíèÿ î êîíêðåòíîì ïîðîãå íåò).
iii. Ìîðôîëîãèÿ ëåâîé íîæêè ïó÷êà Ãèñà.
Ïðè öèòèðîâàíèè ïðîñüáà ññûëàòüñÿ íà àâòîðà.

ÍÆÝ îñîáåííî ÷àñòî âñòðå÷àþòñÿ ó ïàöèåíòîâ ñ ïîðîêîì ìèòðàëüíîãî êëàïàíà è ó ïàöèåíòîâ ñ äèñôóíêöèåé ëåâîãî æåëóäî÷êà íåçàâèñèìî îò ýòèîëîãèè. Îäíàêî âûñîêàÿ ðàñïðîñòðàíåííîñòü ÍÆÝ â íîðìàëüíîé ïîïóëÿöèè äåëàåò òàêèå àññîöèàöèè íåîïðåäåëåííûìè.
 êîãîðòå Êîïåíãàãåíñêîãî õîëòåðîâñêîãî èññëåäîâàíèÿ, â êîòîðîé 638 ÷åëîâåê (â âîçðàñòå îò 55 äî 75 ëåò) ïðîøëè 48-÷àñîâóþ õîëòåðîâñêóþ çàïèñü è íàáëþäàëèñü â ñðåäíåì 14 ëåò, â ãðóïïå ñ ÷àñòûìè ÍÆÝ íàáëþäàëèñü öèðêàäíûå âàðèàöèè, ïðè ýòîì íàèìåíüøåå êîëè÷åñòâî ÍÆÝ â ÷àñ íàáëþäàëîñü íî÷üþ ñ íàäèðîì â 6 ÷àñîâ óòðà è çàòåì äîñòèãàëî ïèêîâîãî çíà÷åíèÿ âî âòîðîé ïîëîâèíå äíÿ â 15 ÷àñîâ. ðèñê ðàçâèòèÿ ôèáðèëëÿöèè ïðåäñåðäèé (ÔÏ) áûë îäèíàêîâûì âî âñåõ âðåìåííûõ èíòåðâàëàõ â òå÷åíèå ñóòîê [Larsen BS, Kumarathurai P, Nielsen OW, Sajadieh A.  èññëåäîâàíèå áûëè âêëþ÷åíû 93 çäîðîâûõ æåíùèíû â ïðåìåíîïàóçå ñî ñðåäíèì âîçðàñòîì 42 ãîäà. Íàëè÷èå ïðîñòûõ ÆÝ ó ïàöèåíòîâ ñ ÿâíî íîðìàëüíûìè ñåðäöàìè ñâÿçàíî ñ ïîâûøåííîé ñìåðòíîñòüþ. Õîòÿ ñóùåñòâóþò ïðîòèâîðå÷èâûå äàííûå î ïðîãíîñòè÷åñêîé çíà÷èìîñòè ÷àñòîé è ñëîæíîé æåëóäî÷êîâîé ýêòîïèè ó ïàöèåíòîâ ñ ÿâíî íîðìàëüíûì ñåðäöåì, â êðóïíåéøåì èññëåäîâàíèè ñîîáùàëîñü îá óâåëè÷åíèè ñìåðòíîñòè ó òàêèõ ïàöèåíòîâ.
Ïðè îòñóòñòâèè ÿâíûõ ñåðäå÷íûõ çàáîëåâàíèé. Íàëè÷èå ïðîñòûõ ÆÝ ó ïàöèåíòîâ ñ ÿâíî íîðìàëüíûìè ñåðäöàìè ñâÿçàíî ñ ïîâûøåííîé ñìåðòíîñòüþ, êàê ïîêàçàíî â ñëåäóþùèõ èññëåäîâàíèÿõ:

Ïîñëå 14-äíåâíîãî ïåðèîäà íåïðåðûâíîãî ìîíèòîðèíãà ÝÊà íåáëàãîïðèÿòíûå ïðåäèêòîðû, âêëþ÷åííûå â îöåíêó ðèñêà ABC-ÆÒ, âêëþ÷àþò: âåðòèêàëüíî íàïðàâëåííóþ îñü ÆÝ (1 áàëë), íàãðóçêó ÆÝ (äîëÿ ÆÝ îò âñåõ ñîêðàùåíèé ñåðäöà) îò 10% äî 20% (2 áàëëà) èëè > 20% (3 áàëëà), èíòåðâàë ñöåïëåíèÿ ÆÝ > 500 ìñ (4 áàëëà) è íåóñòîé÷èâóþ ÆÒ (4 áàëëà). Áîëåå âûñîêèå áàëëû áûëè ñâÿçàíû ñ áîëüøåé âåðîÿòíîñòüþ ðàçâèòèÿ ñíèæåíèÿ Ô ËÆ, ñåðäå÷íîé íåäîñòàòî÷íîñòè, òðåáóþùåé ãîñïèòàëèçàöèè, èëè ñåðäå÷íîé ñìåðòè.

Êàðäèîìèîïàòèþ, ñâÿçàííóþ ñ ÆÝ, ñëåäóåò ïîäîçðåâàòü ó ëèö ñ íåîáúÿñíèìîé êàðäèîìèîïàòèåé è î÷åíü ÷àñòûìè îäíîôîêàëüíûìè ÆÝ (îáû÷íî > 15% îò âñåõ ñîêðàùåíèÿ ñåðäöà), õîòÿ íåêîòîðûå ïàöèåíòû ñî ñòîëü æå âûñîêîé ÷àñòîòîé ÆÝ ìîãóò ïîääåðæèâàòü íîðìàëüíóþ ñåðäå÷íóþ ôóíêöèþ, â òî âðåìÿ êàê êàðäèîìèîïàòèÿ, âûçâàííàÿ ÆÝ, òàêæå ðåãèñòðèðîâàëàñü ó ïàöèåíòîâ ñ ÷àñòîòîé ÆÝ îò 4% äî 5% ïðîöåíòîâ. Óñòðàíåíèå ÆÝ ñ ïîìîùüþ êàòåòåðíîé àáëÿöèè èëè ëåêàðñòâ â òàêèõ ñëó÷àÿõ ÷àñòî ïðèâîäèò ê íîðìàëèçàöèè ñåðäå÷íîé ôóíêöèè. Ýòî áûëî íàèáîëåå âûðàæåíî ó ìîëîäûõ ïàöèåíòîâ áåç ñîïóòñòâóþùèõ çàáîëåâàíèé, ÷òî ïîçâîëÿåò ïðåäïîëîæèòü, ÷òî ÆÝ ìîãóò áûòü âàæíîé ïðè÷èíîé "èäèîïàòè÷åñêîé" ÑÍ.

Ïðè íàëè÷èè ñòðóêòóðíîãî çàáîëåâàíèÿ ñåðäöà.

ÈÌ.
ÆÝ, îñîáåííî åñëè îíè ÷àñòûå (áîëåå 10 â ÷àñ) èëè ñëîæíûå (ò.å. ïîâòîðÿþùèåñÿ ôîðìû, â ïåðâóþ î÷åðåäü íåóñòîé÷èâûå ÆÒ), ïî-âèäèìîìó, ñâÿçàíû ñ õóäøèì ïðîãíîçîì ó ïàöèåíòîâ ñ ïðåäøåñòâóþùèì èíôàðêòîì ìèîêàðäà. Îäíàêî ïîïûòêè ïîäàâëåíèÿ ýòîé àðèòìèè àíòèàðèòìè÷åñêèìè ïðåïàðàòàìè êëàññà I ó òàêèõ ïàöèåíòîâ áûëè ñâÿçàíû ñ óâåëè÷åíèåì ñìåðòíîñòè, â òî âðåìÿ êàê àìèîäàðîí çíà÷èòåëüíî ñíèæàåò ñìåðòíîñòü îò àðèòìèé, íî íå îáùóþ ñìåðòíîñòü. Òàêèì îáðàçîì, àìèîäàðîí ÿâëÿåòñÿ ïðåäïî÷òèòåëüíûì àíòèàðèòìè÷åñêèì ïðåïàðàòîì ó ïàöèåíòîâ ñ ñèìïòîìàòè÷åñêèì ÆÝ, ïðîäîëæàþùèìèñÿ íåñìîòðÿ íà òåðàïèþ áåòà-áëîêàòîðàìè.  ñëó÷àÿõ ñëîæíîé è / èëè ñèìïòîìàòè÷åñêîé æåëóäî÷êîâîé àðèòìèè ýëåêòðîôèçèîëîãè÷åñêîå èññëåäîâàíèå ñ ïðîãðàììèðîâàííîé æåëóäî÷êîâîé ñòèìóëÿöèåé ìîæåò ëó÷øå íàïðàâëÿòü òåðàïèþ; ó ïàöèåíòîâ ñ èíäóöèðóåìîé óñòîé÷èâîé ÆÒ òðåáóåòñÿ èìïëàíòèðóåìûé êàðäèîâåðòåð-äåôèáðèëëÿòîð [Manolis AS. Âûçâàííàÿ ôèçè÷åñêîé íàãðóçêîé. Ðèñê èøåìè÷åñêîãî èíñóëüòà.  ïðîñïåêòèâíîé îöåíêå 14 783 ñóáúåêòîâ, âîâëå÷åííûõ â èññëåäîâàíèå " Atherosclerosis Risk In Communities" (ARIC), â êîòîðîì ó 6,1% ó÷àñòíèêîâ áûëè îáíàðóæåíû ÆÝ íà èñõîäíîì óðîâíå íà äâóõìèíóòíîé ÝÊÃ, ó 729 (4,9%) áûë èíñóëüò [Agarwal SK, Heiss G, Rautaharju PM, et al. Premature ventricular complexes and the risk of incident stroke: the Atherosclerosis Risk In Communities (ARIC) Study. Stroke 2010; 41:588]. Íåñêîððåêòèðîâàííàÿ ñîâîêóïíàÿ äîëÿ ñëó÷àåâ èíñóëüòà ó ëèö ñ ëþáûìè ÆÝ ñîñòàâèëà 6,6% ïî ñðàâíåíèþ ñ 4,1% ó ëèö áåç ÆÝ (ÎÐ 1,7, 95% ÄÈ 1,3-2,2). Áèãåìèíèÿ (÷åðåäîâàíèå íîðìàëüíûõ ñîêðàùåíèé è ÆÝ), òðèãåìèíèÿ (äâà íîðìàëüíûõ ñîêðàùåíèÿ çà êîòîðûìè ñëåäóåò ÆÝ) è êâàäðèãåìèíèÿ (òðè íîðìàëüíûõ ñîêðàùåíèÿ çà êîòîðûìè ñëåäóåò ÆÝ) ðåäêî âûçûâàþò ñåðüåçíûå ñèìïòîìû è íå èìåþò èçâåñòíîé íåçàâèñèìîé ïðîãíîñòè÷åñêîé çíà÷èìîñòè.

Èíòåðïîëèðîâàííûå ÆÝ, êîòîðûå âñòàâëÿþòñÿ ìåæäó äâóìÿ ñèíóñîâûìè ñîêðàùåíèÿìè áåç êîìïåíñàòîðíîé ïàóçû, ñâÿçàíû ñ ÆÝ-èíäóöèðîâàííîé êàðäèîìèîïàòèåé [Olgun H, Yokokawa M, Baman T, et al. The role of interpolation in PVC-induced cardiomyopathy. Heart Rhythm 2011; 8:1046].

Æåëóäî÷êîâàÿ ïàðàñèñòîëèÿ ïðåäñòàâëÿåò ñîáîé íåçàâèñèìûé ýêòîïè÷åñêèé æåëóäî÷êîâûé ðèòì, êîòîðûé êîíêóðèðóåò ñ ñèíóñîâûì ðèòìîì. Íà ÝÊà îí îòîáðàæàåòñÿ ìîíîìîðôíûìè ÆÝ ñ ïåðåìåííûì öèêëîì (èíòåðâàë ìåæäó ïðåäøåñòâóþùèì ñèíóñîâûì ñîêðàùåíèåì è ÆÝ âàðüèðóåòñÿ). Ò.å. èìååòñÿ î÷àã (èëè îáëàñòü ëîêàëüíîãî ðèýíòðè) êîòîðûé ãåíåðèðóåò èìïóëüñû ñî ñâîåé ñêîðîñòüþ.

Ñèìïòîìàòèêà. Ñåíñèòèâíûå ëþäè.

ÍÆÝ ìîãóò ïðîòåêàòü áåññèìïòîìíî èëè âûçûâàòü òàêèå ñèìïòîìû, êàê îùóùåíèå «ïðîïóñêà» èëè ó÷àùåííîå ñåðäöåáèåíèå, õîòÿ ó ïîäàâëÿþùåãî áîëüøèíñòâà ïàöèåíòîâ îíè âûçûâàþò ìàëî ñèìïòîìîâ èëè íå âûçûâàþò èõ âîâñå (êàê è ó áîëüøèíñòâà ëþäåé ñ ÆÝ), õîòÿ ó íåêîòîðûõ ëþäåé ìîæåò íàáëþäàòüñÿ ñåðäöåáèåíèå èëè ãîëîâîêðóæåíèå. ÍÆÝ ðåäêî âûçûâàþò èñòèííîå ãåìîäèíàìè÷åñêîå íàðóøåíèå, çà èñêëþ÷åíèåì ñëó÷àåâ, êîãäà îíè ñâÿçàíû ñ ëåæàùåé â îñíîâå áðàäèêàðäèåé. ÍÆÝ ìîãóò âûçûâàòü ó÷àùåííîå ñåðäöåáèåíèå (ïðè íàëè÷èè ïàóçû è ïîâûøåííîé èíîòðîïèè ëåâîãî æåëóäî÷êà â ðåçóëüòàòå óâåëè÷åíèÿ óäàðíîãî îáúåìà) èëè îùóùåíèå ïðîïóùåííûõ ñåðäå÷íûõ ñîêðàùåíèé, ÷òî ìîæåò áûòü ñâÿçàíî ñ íåïðîâåäåíèåì ÍÆÝ èëè íåýôôåêòèâíûì ñîêðàùåíèåì â ðåçóëüòàòå ïëîõîãî íàïîëíåíèÿ ëåâîãî æåëóäî÷êà âî âðåìÿ ïðåæäåâðåìåííîãî óäàðà. Ïðåäñåðäíàÿ áèãåìèíèÿ ñ íåïðîâîäÿùèìè ÍÆÝ íà æåëóäî÷êè ìîæåò ïðèâåñòè ê ÷àñòîòå æåëóäî÷êîâ, ïðèáëèæàþùåéñÿ ê 40 óäàðàì â ìèíóòó, ÷òî ìîæåò ïðèâåñòè ê ñèìïòîìàì (ò. å. äóðíîòå, ãîëîâîêðóæåíèþ, ïðåäîáìîðî÷íîìó ñîñòîÿíèþ), ñâÿçàííûì ñ áðàäèêàðäèåé. ×àñòûå ÍÆÝ áûëè ñâÿçàíû ñ îáðàòèìîé êàðäèîìèîïàòèåé íà æèâîòíûõ ìîäåëÿõ, ñ ðåäêèìè ñîîáùåíèÿìè î ñëó÷àÿõ ó ëþäåé, êîãäà ïàöèåíòû ñ íåïðåêðàùàþùèìèñÿ ÍÆÝ ìîãóò èìåòü ñèìïòîìû ÑÍ [Vervueren PL, Delmas C, Berry M, et al. Reversal of Dilated Cardiomyopathy After Successful Radio-Frequency Ablation of Frequent Atrial Premature Beats, a New Cause for Arrhythmia-Induced Cardiomyopathy. J Atr Fibrillation 2012; 5:627].

Ïîäàâëÿþùåå áîëüøèíñòâî ïàöèåíòîâ ñ ÆÝ íå èñïûòûâàþò íèêàêèõ ñèìïòîìîâ. Êîãäà ïàöèåíòû èñïûòûâàþò ñèìïòîìû, ó÷àùåííîå ñåðäöåáèåíèå ÿâëÿåòñÿ íàèáîëåå ÷àñòûì ñèìïòîìîì, ïðè ýòîì ãîëîâîêðóæåíèå âîçíèêàåò íå÷àñòî. ÆÝ ðåäêî âûçûâàþò èñòèííûå ãåìîäèíàìè÷åñêèå íàðóøåíèÿ, çà èñêëþ÷åíèåì ñëó÷àåâ, êîãäà îíè ÷àñòî âîçíèêàþò ó ïàöèåíòîâ ñ ñèëüíî ñíèæåííîé ôóíêöèåé ëåâîãî æåëóäî÷êà (ËÆ) èëè êîãäà îíè ñâÿçàíû ñ âûðàæåííîé áðàäèêàðäèåé.
Íàèáîëåå ðàñïðîñòðàíåííûìè ñèìïòîìàìè, âîçíèêàþùèìè â ðåçóëüòàòå ÆÝ, ÿâëÿåòñÿ ó÷àùåííîå ñåðäöåáèåíèå. Ó÷àùåííîå ñåðäöåáèåíèå âîçíèêàåò â ðåçóëüòàòå ãèïåðñîêðàòèìîñòè ïîñëå ýêñòðàñèñòîëû èëè îùóùåíèÿ, ÷òî ñåðäöå îñòàíîâèëîñü, êîòîðîå âòîðè÷íî ê íîðìàëüíîé ïîñòýêñòðàñèñòîëè÷åñêîé êîìïåíñàòîðíîé ïàóçå. Ðåæå ÷àñòûå ÆÝ ìîãóò ïðèâåñòè ê îùóùåíèþ ïóëüñàöèè â øåå, ãîëîâîêðóæåíèþ èëè ïî÷òè îáìîðîêó.
Ñóùåñòâóåò áîëüøàÿ âàðèàáåëüíîñòü â îòíîøåíèè òîãî, êîãäà ñèìïòîìû íàèáîëåå çàìåòíû, õîòÿ ñïîêîéíàÿ îáñòàíîâêà, íàïðèìåð, íî÷üþ, êîãäà âû ëåæèòå â ïîñòåëè, ìîæåò ïîìî÷ü ïàöèåíòàì ëó÷øå îñîçíàòü ñâîè ÆÝ. Îíè ÷àùå îöåíèâàþòñÿ, êîãäà ñóáúåêòû ëåæàò íà ëåâîì áîêó, à ñåðäöå áëèæå ê ãðóäíîé ñòåíêå.
Äëÿ ÆÝ, êàê è äëÿ ÍÆÝ, ñóùåñòâóþò ðàçíîîáðàçíûå òðèããåðû: ñîïóòñòâóþùèå ñåðäå÷íî-ñîñóäèñòûå çàáîëåâàíèÿ, óïîòðåáëåíèå àëêîãîëÿ èëè íàïèòêîâ, ñîäåðæàùèõ êîôåèí, èëè çàïðåùåííûõ íàðêîòèêîâ, ýëåêòðîëèòíûå íàðóøåíèÿ, íàðóøåíèÿ ôóíêöèè ùèòîâèäíîé æåëåçû è ò.ä.

Îùóùåíèÿ ìîãóò âîçíèêàòü è ïðè îòñóòñòâèè èçìåíåíèé íà ÝÊÃ (äî 20%, ò.å. òàêîå íå ðåäêî).

×òîáû îïðåäåëèòü, ïðåäñêàçûâàåò ëè ïîâòîðåíèå ó÷àùåííîãî ñåðäöåáèåíèÿ ïîñëå àáëÿöèè ðåöèäèâ ïóòè, ìû îïðîñèëè 77 ïàöèåíòîâ, ó êîòîðûõ ïî êðàéíåé ìåðå ÷åðåç 4 íåäåëè ïîñëå óñïåøíîé àáëÿöèè âñïîìîãàòåëüíîãî ïóòè (43 ÿâíûõ, 11 ñêðûòûõ) èëè ìåäëåííîãî ïóòè (23) äëÿ AV-óçëîâîé ðåöèäèâèðóþùåé òàõèêàðäèè. Î ó÷àùåííîì ñåðäöåáèåíèè ñîîáùèëè 45 (58%) ïàöèåíòîâ ïîñëå àáëÿöèè, à 28 (36%) ïàöèåíòîâ ñîîáùèëè î ó÷àùåííîì ñåðäöåáèåíèè ïðîäîëæèòåëüíîñòüþ> èëè = 10 ñåêóíä è / èëè ïî÷óâñòâîâàëè, ÷òî èõ ñèìïòîìû ïðåäñòàâëÿþò ñîáîé ðåöèäèâèðóþùóþ òàõèêàðäèþ (ñèëüíîå ñåðäöåáèåíèå). Ïîâòîðíîå ýëåêòðîôèçèîëîãè÷åñêîå òåñòèðîâàíèå áûëî ïðîâåäåíî ÷åðåç 3 ìåñÿöà ïîñëå àáëÿöèè 53 ïàöèåíòàì, â òîì ÷èñëå 34 ïàöèåíòàì ñ ó÷àùåííûì ñåðäöåáèåíèåì (22 ñ îñíîâíûìè ñèìïòîìàìè). Ó âîñüìè (10%) ïàöèåíòîâ ïðè ïîâòîðíîì òåñòèðîâàíèè áûë âûÿâëåí ðåöèäèâ ïðîâîäèìîñòè ïî ïðîâîäÿùåìó ïóòè: ó äâóõ íå áûëî ñèìïòîìîâ äî ïîâòîðíîãî èññëåäîâàíèÿ, à ó øåñòè áûëè ñåðüåçíûå ñèìïòîìû. Ó îäíîãî ïàöèåíòà áûëè îñíîâíûå ñèìïòîìû, íî ïðè ïîâòîðíîì èññëåäîâàíèè áûëî îáíàðóæåíî, ÷òî ó íåãî èíäóöèðóåìàÿ ïðåäñåðäíàÿ òàõèêàðäèÿ, à íå ðåöèäèâ ïóòè. Òàêèì îáðàçîì, ó 15 (68%) èç 22 ïàöèåíòîâ ñ îñíîâíûìè ñèìïòîìàìè, êîòîðûå áûëè ïîâòîðíî îáñëåäîâàíû, íå áûëî ðåöèäèâà èëè èíäóöèðóåìîé àðèòìèè, êîòîðûå ìîãëè áû îáúÿñíèòü èõ ñèìïòîìû. Èç 24 ïàöèåíòîâ, íå îáñëåäîâàííûõ ïîâòîðíî, íè ó îäíîãî íå áûëî çàäîêóìåíòèðîâàííîé ðåöèäèâèðóþùåé òàõèêàðäèè èëè ÿâíîãî íàðóøåíèÿ ïðîâîäèìîñòè ïî ýëåêòðîêàðäèîãðàììå â òå÷åíèå ñðåäíåãî ïåðèîäà íàáëþäåíèÿ 335 +/- 138 (äèàïàçîí 132-616) äíåé. Òàêèì îáðàçîì, ó÷àùåííîå ñåðäöåáèåíèå, â òîì ÷èñëå ó÷àùåííîå ñåðäöåáèåíèå, íàïîìèíàþùåå ñèìïòîìû ïåðåä àáëÿöèåé, ÷àñòî âîçíèêàåò ïîñëå ðàäèî÷àñòîòíîé àáëÿöèè è ÷àñòî íå ïðåäñêàçûâàåò ðåöèäèâ ïóòè. Ïîâòîðíîå ýëåêòðîôèçèîëîãè÷åñêîå òåñòèðîâàíèå ÷àñòî òðåáóåòñÿ äëÿ äîêóìåíòèðîâàíèÿ äîëãîñðî÷íîãî óñïåõà ðàäèî÷àñòîòíîé àáëÿöèè ïðè ñóïðàâåíòðèêóëÿðíîé òàõèêàðäèè ó ïàöèåíòîâ ñ ðåöèäèâîì îñíîâíûõ ñèìïòîìîâ.  èññëåäîâàíèå áûëè âêëþ÷åíû 55 ïàöèåíòîâ ñ ó÷àùåííûì ñåðäöåáèåíèåì ìåíåå îäíîãî ðàçà â íåäåëþ. Ïàöèåíòû ïîëó÷àëè ýëåêòðîêàðäèîãðàô Miniscope MS-3 ñî âñòðîåííûìè ýëåêòðîäàìè (Schiller AG, Øâåéöàðèÿ) äëÿ çàïèñè îäíîêàíàëüíîé ýëåêòðîêàðäèîãðàììû âî âðåìÿ ïîâòîðíîãî ýïèçîäà. Îöåíèâàëè ÷àñòîòó àêòèâàöèè óñòðîéñòâà è ïðåäñåðäíûé ðèòì âî âðåìÿ ýïèçîäà. Ðåçóëüòàòû: Ó ñîðîêà îäíîãî (75%) ïàöèåíòà áûë ïî êðàéíåé ìåðå îäèí ýïèçîä â òå÷åíèå 38 +/- 22 äíåé. Ó äâàäöàòè òðåõ ïàöèåíòîâ áûë ñèíóñîâûé ðèòì, ó 7 ïàðîêñèçìàëüíàÿ ôèáðèëëÿöèÿ ïðåäñåðäèé, ó 7 ïàðîêñèçìàëüíîå òðåïåòàíèå ïðåäñåðäèé è ó 4 ðåöèïðîêíàÿ òàõèêàðäèÿ AV-óçëà. Ó 11 èç 23 ïàöèåíòîâ ñ ñèíóñîâûì ðèòìîì äîïîëíèòåëüíî íàáëþäàëèñü ïðåæäåâðåìåííûå ñîêðàùåíèÿ ïðåäñåðäèé (ÍÆÝ) èëè æåëóäî÷êîâ (ÆÝ). Ò.å. 12 (21,8% îò âñåõ îáñëåäîâàííûõ) èç 23 îùóùàëè ñåðäöåáèåíèÿ íà íîðìàëüíîì ñèíóñîâîì ðèòìå. [Schuchert A, Behrens G, Meinertz T. Êòî-òî îùóùàåò íåïîñðåäñòâåííî ïðîïóñêè áèåíèé ñåðäöà, êòî-òî ÷ðåçìåðíî çàîñòðÿåò âíèìàíèå íà äåÿòåëüíîñòè ñåðäöà (ïðèñëóøèâàíèå, ÷óâñòâèòåëüíîñòü ê òåëåñíûì îùóùåíèÿì), ó êîãî-òî îùóùåíèÿ íåéðîïàòè÷åñêîãî ãåíåçà.
Îùóùåíèå ó÷àùåííîãî ñåðäöåáèåíèÿ áîëåå ñòîéêîå ó ëþäåé, êîòîðûå îäíîâðåìåííî î÷åíü ÷óâñòâèòåëüíû ê òåëåñíûì îùóùåíèÿì è èñïûòûâàþò áîëüøåå êîëè÷åñòâî íåçíà÷èòåëüíûõ åæåäíåâíûõ ðàçäðàæèòåëåé. Îäíîãî ñóùåñòâîâàíèÿ ëþáîãî èç ïðåäèêòîðîâ íåäîñòàòî÷íî äëÿ ïðîÿâëåíèÿ ýòîãî ôóíêöèîíàëüíîãî ñîìàòè÷åñêîãî ñèìïòîìà; äëÿ ýòîãî òðåáóåòñÿ êîìáèíàöèÿ ýòèõ ïðåäèêòîðîâ. Îáùåå êîëè÷åñòâî ïðåæäåâðåìåííûõ ñîêðàùåíèé æåëóäî÷êîâ, ïðîèñõîäÿùèõ âî âðåìÿ àìáóëàòîðíîãî ìîíèòîðèíãà, íå áûëî çíà÷èìûì ïðåäèêòîðîì îùóùåíèÿ ó÷àùåííîãî ñåðäöåáèåíèÿ. ÆÝ – ýòî Ý ñ øèðîêèì êîìïëåêñîì QRS, íî òî÷íî ëè îíè æåëóäî÷êîâûå? Ì.á. ÆÝ ñ ó÷àñòèåì WPW èëè ñ áëîêàäîé íîæêè ïó÷êà Ãèñà, ò.å. ïî ñóòè íå æåëóäî÷êîâûå, à íàäæåëóäî÷êîâûå.  äèàãíîñòèêå âàæíî òî÷íî òèïèðîâàòü Ý, â ò.÷. äëÿ ñíèæåíèÿ òðåâîãè ó ïàöèåíòà.
Ïàöèåíòû áåç ïîäîçðåíèÿ íà ñåðüåçíîå îñíîâíîå çàáîëåâàíèå ñåðäöà, ó êîòîðûõ ìàëî èëè âîîáùå íåò ñèìïòîìîâ è ó êîòîðûõ íèçêîå êîëè÷åñòâî ÆÝ (<500 çà 24 ÷àñà), êàê ïðàâèëî, íå òðåáóþò äîïîëíèòåëüíîãî îáñëåäîâàíèÿ.

Íåêîòîðûå õàðàêòåðèñòèêè, êîòîðûå ìîãóò áûòü ïîëåçíû äëÿ âûÿâëåíèÿ çëîêà÷åñòâåííûõ ÆÝ, âêëþ÷àþò: áîëüøóþ äîëþ ÆÝ ñðåäè âñåõ ñîêðàùåíèé ñåðäöà (íàïðèìåð, > 10%), áîëåå øèðîêèå QRS ïðè ÆÝ, õàðàêòåðèñòèêè ÝÊà ýïèêàðäèàëüíîãî ïðîèñõîæäåíèÿ ÆÝ, èíòåðïîëèðîâàííûå ÆÝ, ò.ê. âñå îíè ñâÿçàíû ñ ïîâûøåííûì ðèñêîì êàðäèîìèîïàòèè, âûçâàííîé ÆÝ; â òî âðåìÿ êàê ÆÝ ñ êîðîòêèì èíòåðâàëîì ñöåïëåíèÿ (< 300 ìñ), ÆÝ ñ ÝÊÃ-õàðàêòåðèñòèêàìè ïðîèñõîæäåíèÿ èç ñèñòåìû Ïóðêèíüå, ëèáî ïðàâîé, ëèáî ëåâîé íîæåê ïó÷êà Ãèñà, ìîãóò ñïðîâîöèðîâàòü ïîëèìîðôíûå ÆÒ èëè ÔÆ.

- âåðîÿòíûõ ïðè÷èí
ÍÆÝ – ò.ê. îíè ÷àñòî âîçíèêàþò ó çäîðîâûõ ëþäåé, à òàêæå ó ëèö ñ èçâåñòíûì ñåðäå÷íî-ñîñóäèñòûì çàáîëåâàíèåì, òðóäíî óñòàíîâèòü îïðåäåëåííóþ ñâÿçü ñ äðóãèìè ðàññòðîéñòâàìè èëè îïðåäåëèòü ôàêòîðû, êîòîðûå ïðåäðàñïîëàãàþò ê íèì. Êðîìå òîãî, ÷àñòîòà ÍÆÝ øèðîêî âàðüèðóåò ïðè ðàçëè÷íûõ çàáîëåâàíèÿõ ñåðäöà.
Èäèîïàòè÷åñêèå ÍÆÝ. Ó ïàöèåíòîâ áåç ñòðóêòóðíûõ çàáîëåâàíèé ñåðäöà ÍÆÝ ÷àñòî ïðîèñõîäÿò èç ëåãî÷íûõ âåí. Äîêàçàíî, ÷òî ÍÆÝ ïðåäøåñòâóþò äåãåíåðàöèè ñèíóñîâîãî ðèòìà â ôèáðèëëÿöèþ ïðåäñåðäèé è, òàêèì îáðàçîì, ñ÷èòàþòñÿ îñíîâíûìè òðèããåðàìè ýòîé ðàñïðîñòðàíåííîé àðèòìèè.

- ñåðäå÷íûå
Îñòðûé èíôàðêò ìèîêàðäà. Ó ïàöèåíòîâ ñ îñòðûì èíôàðêòîì ìèîêàðäà (ÈÌ) îòìå÷àåòñÿ ðàííåå óâåëè÷åíèå ÷àñòîòû ÍÆÝ ñ ÷àñòîòîé îò 25 äî 81%.  îäíîì èññëåäîâàíèè áûëî îòìå÷åíî ñðåäíåå çíà÷åíèå îò 9 äî 14 ÍÆÝ â ÷àñ â 1-é äåíü ïîñëå ÈÌ, êîòîðîå ñíèçèëîñü äî 1-2 ÍÆÝ â ÷àñ íà 10-é äåíü ïîñëå ÈÌ [Rechavia E, Strasberg B, Mager A, et al. Èäèîïàòè÷åñêàÿ æåëóäî÷êîâàÿ òàõèêàðäèÿ

Èøåìè÷åñêàÿ áîëåçíü ñåðäöà. Ó ïàöèåíòîâ ñ èçâåñòíîé èëè ïîäîçðåâàåìîé èøåìè÷åñêîé áîëåçíüþ ñåðäöà âî âðåìÿ íàãðóçî÷íîãî òåñòà ìîãóò áûòü èíäóöèðîâàíû ÍÆÝ, íî ïðîãíîñòè÷åñêîå çíà÷åíèå ÍÆÝ âî âðåìÿ íàãðóçî÷íîãî òåñòà îñòàåòñÿ íåèçâåñòíûì [Bunch TJ, Chandrasekaran K, Gersh BJ, et al. The prognostic significance of exercise-induced atrial arrhythmias. J Am Coll Cardiol 2004; 43:1236].
Äðóãèå áîëåçíè ñåðäöà. ×àñòîòà ÍÆÝ óâåëè÷èâàåòñÿ ïðè ìèòðàëüíîì ñòåíîçå, ãèïåðòðîôè÷åñêîé êàðäèîìèîïàòèè è ëþáûõ ñîñòîÿíèÿõ, êîòîðûå ïðèâîäÿò ê ïîâûøåíèþ äàâëåíèÿ èëè äèëàòàöèè ïðàâîãî èëè ëåâîãî ïðåäñåðäèÿ, âêëþ÷àÿ êàðäèîìèîïàòèþ èëè ïîðîêè êëàïàíîâ ñåðäöà.

Ïðåäñåðäíàÿ ìèîïàòèÿ. Íåêîòîðûå èññëåäîâàíèÿ ïðåäïîëàãàþò íàëè÷èå ïðåäñåðäíîé ìèîïàòèè â êà÷åñòâå îñíîâíîãî çàáîëåâàíèÿ ó ïàöèåíòîâ ñ ÔÏ è áåç íåå. Êîíöåïöèÿ çàêëþ÷àåòñÿ â òîì, ÷òî ñòàðåíèå, ðàñòÿæåíèå ïðåäñåðäèé è/èëè âîñïàëåíèå ìîãóò âûçûâàòü ðåìîäåëèðîâàíèå ïðåäñåðäèé, ÷òî ìîæåò ïðèâåñòè ê òðîìáîîáðàçîâàíèþ ëåâîãî ïðåäñåðäèÿ ó íåêîòîðûõ ïàöèåíòîâ äàæå ïðè îòñóòñòâèè ÔÏ [Shen MJ, Arora R, Jalife J. Òîêñèíû èëè õèìè÷åñêèå âåùåñòâà.
Êóðåíèå, àëêîãîëü è êîôå øèðîêî ðàññìàòðèâàþòñÿ êàê ïîòåíöèàëüíûå ïðîâîêàòîðû ÍÆÝ è ÆÝ. Èçâåñòíî, ÷òî êóðåíèå è àëêîãîëü ïîâûøàþò ñèìïàòè÷åñêèé òîíóñ, ÷òî ìîæåò âëèÿòü íà ÷àñòîòó ÍÆÝ è ÆÝ. Õîòÿ øèðîêî ðàñïðîñòðàíåíî ìíåíèå, ÷òî êîôåèí, îñîáåííî â âûñîêèõ äîçàõ, âûçûâàåò ó÷àùåííîå ñåðäöåáèåíèå è ðÿä àðèòìèé, íåò íèêàêèõ äîêàçàòåëüñòâ åãî ïðîàðèòìè÷åñêîãî äåéñòâèÿ. Êîôåèí îêàçûâàåò ÿâíîå ýëåêòðîôèçèîëîãè÷åñêîå âîçäåéñòâèå íà ïðåäñåðäèÿ, õîòÿ ñâÿçü ñ ÍÆÝ íåÿñíà.  èññëåäîâàíèè 1388 ó÷àñòíèêîâ èññëåäîâàíèÿ ñåðäå÷íî-ñîñóäèñòîãî çäîðîâüÿ, â õîäå êîòîðîãî ïàöèåíòû ñîîáùàëè î ïîòðåáëåíèè êîôåèíà, à ïàöèåíòû ïîäâåðãàëèñü 24-÷àñîâîìó àìáóëàòîðíîìó ìîíèòîðèíãó, íå áûëî âûÿâëåíî ñóùåñòâåííûõ ðàçëè÷èé â ÷àñòîòå ÍÆÝ è ÆÝ ìåæäó ïîòðåáèòåëÿìè è òåìè, êòî íå óïîòðåáëÿë êîôåèí [Dixit S, Stein PK, Dewland TA, et al. Consumption of Caffeinated Products and Cardiac Ectopy. J Am Heart Assoc 2016; 5.]. Òåì íå ìåíåå, åñòü ïàöèåíòû, êîòîðûå ìîãóò áûòü áîëåå ÷óâñòâèòåëüíû ê êîôåèíó è îòìå÷àòü ñâÿçü ñåðäöåáèåíèÿ ñ ïðèåìîì êîôåèíà. Òåîôèëëèí, äðóãîå ñîåäèíåíèå ìåòèëêñàíòèíà, òàêæå ìîæåò óâåëè÷èâàòü ÷àñòîòó ÍÆÝ.

Èíòîêñèêàöèÿ íàïåðñòÿíêîé (äèãîêñèíîì)

Óïîòðåáëåíèå ìåäèêàìåíòîçíûõ ñòèìóëÿòîðîâ, âêëþ÷àÿ ñèìïàòîìèìåòè÷åñêèå ïðåïàðàòû (áåòààäðåíîìèìåòèêè [íàïðèìåð, ñàëüáóòàìîë äëÿ ëå÷åíèÿ áðîíõîîáñòðóêöèè], ïðîòèâîîòå÷íûå è àíòèãèñòàìèííûå).

Óïîòðåáëåíèå àìôåòàìèíà, êîêàèíà è/èëè äðóãèõ íàðêîòèêîâ

Âîñïàëåíèå â áëèçêî ðàñïîëîæåííûõ ê ñåðäöó îðãàíàõ (ïèùåâîä, æåëóäîê, æåë÷íûé ïóçûðü, ïëåâðà, ëåãêèå), âîñïàëåíèå ìîæåò áûòü èíôåêöèîííûì (ìèêðîáû, âèðóñû, ãðèáû), àñåïòè÷åñêèì (èììóííûì, àëëåðãè÷åñêèì, ðåâìàòè÷åñêèì) èëè âíóòðè èëè âîêðóã îíêîëîãè÷åñêîãî ïðîöåññà.

Ñèñòåìíûå è/èëè òÿæåëûå èíôåêöèè: ÂÈ×, ãåïàòèòû  è Ñ, à òàêæå èõ ëå÷åíèå, ñèôèëèñ, áðþøíîé òèô, Ëàéì-áîððåëèîç, Êîâèä-19 è åãî ëå÷åíèå.

Ëå÷åíèå

Öåëè ëå÷åíèÿ – 1. Ïðîäëåíèå æèçíè, 2. Óëó÷øåíèå êà÷åñòâà æèçíè. Îöåíêà êà÷åñòâà æèçíè íå ïðîñòà è ïðîòèâîðå÷èâà. Óëó÷øåíèå êà÷åñòâà æèçíè íå äîëæíî ñîêðàùàòü ïðîäîëæèòåëüíîñòü.


ÍÆÝ
Ó ëèö ñ ÷àñòûìè ÍÆÝ äàëüíåéøàÿ îöåíêà è ëå÷åíèå îñíîâàíû íà íàëè÷èè èëè îòñóòñòâèè îñíîâíîãî ñòðóêòóðíîãî çàáîëåâàíèÿ ñåðäöà è/èëè ñèìïòîìîâ.

Ëå÷åíèå ÍÆÝ ó áåññèìïòîìíûõ ïàöèåíòîâ íå òðåáóåòñÿ. Äëÿ ïàöèåíòîâ ñ ñèìïòîìàòè÷åñêèìè ÍÆÝ ÷àñòî áûâàåò äîñòàòî÷íî ïðîñòîé óâåðåííîñòè â äîáðîêà÷åñòâåííîé ïðèðîäå ÍÆÝ äëÿ îáëåã÷åíèÿ ñèìïòîìîâ. Êðîìå òîãî, ïàöèåíòàì ñëåäóåò ðåêîìåíäîâàòü èçáåãàòü èëè ñâîäèòü ê ìèíèìóìó ïîòåíöèàëüíûå ôàêòîðû, ïðîâîöèðóþùèå ÍÆÝ (íàïðèìåð, êóðåíèå, óïîòðåáëåíèå àëêîãîëÿ, ñòðåññ, ïîòðåáëåíèå êîôåèíà ó ïàöèåíòîâ, ñèìïòîìû êîòîðûõ ñ ýòèì ñâÿçàíû).
Äëÿ ïàöèåíòîâ ñ ïðîäîëæàþùèìñÿ ñèìïòîìàòè÷åñêèì ÍÆÝ ïîñëå óñèëèé ïî ìèíèìèçàöèè ïîòåíöèàëüíûõ ïðîâîêàòîðîâ ÍÆÝ ðåêîìåíäóåòñÿ ïðîáíàÿ ìåäèêàìåíòîçíàÿ òåðàïèÿ áåòà-áëîêàòîðîì (íàïðèìåð, ïåðîðàëüíûé ìåòîïðîëîë 25 ìã äâà ðàçà â äåíü [èëè åãî ôîðìû âûïóñêà äëÿ îäíîêðàòíîãî ïðèìåíåíèÿ] ñ ïîâûøåíèåì äîçû ïî ìåðå íåîáõîäèìîñòè äëÿ äîñòèæåíèÿ ýôôåêòà) (2C).

Êîãäà ÍÆÝ ÿâëÿþòñÿ ñèìïòîìàòè÷åñêèìè è çàäîêóìåíòèðîâàíî, ÷òî îíè âûçûâàþò ÔÏ, îíè ìîãóò áûòü ìèøåíüþ äëÿ êàòåòåðíîé àáëàöèè (Ð×À), îñîáåííî ó ïàöèåíòîâ ñ ïîäîçðåíèåì íà êàðäèîìèîïàòèþ èç-çà ÷àñòûõ ÍÆÝ èëè ó ïàöèåíòîâ ñ ïåðñèñòèðóþùèìè ÍÆÝ è ñèìïòîìàìè, íåñìîòðÿ íà ìåäèêàìåíòîçíóþ òåðàïèþ.

Íà ïðàêòèêå ïðàâèëîì ÿâëÿåòñÿ òåðàïåâòè÷åñêîå âîçäåðæàíèå, îñîáåííî ïðè ëàòåíòíîé ýêñòðàñèñòîëèè.  ñëó÷àÿõ ñ ïëîõîé ôóíêöèîíàëüíîé ïåðåíîñèìîñòüþ ìîãóò áûòü ïîëåçíû íåéðîñåäàòèâíûå áåòà-áëîêàòîðû. Ëå÷åíèå ÆÝ çàâèñèò îò òÿæåñòè ÆÝ, íàëè÷èÿ îñíîâíîãî î÷åâèäíîãî ñòðóêòóðíîãî çàáîëåâàíèÿ ñåðäöà èëè/è íàëè÷èÿ ïðîâîöèðóþùåãî ñîñòîÿíèÿ/çàáîëåâàíèÿ è íàëè÷èÿ ñîïóòñòâóþùèõ ñèìïòîìîâ.

Áåññèìïòîìíûå ïàöèåíòû ñ íèçêîé íàãðóçêîé ÆÝ è áåç ÿâíûõ ñòðóêòóðíûõ çàáîëåâàíèé ñåðäöà:
- Äëÿ ïàöèåíòîâ ñ íèçêîé íàãðóçêîé ÆÝ (< 10% îò âñåõ ñîêðàùåíèÿ ñåðäöà), áåç âèäèìûõ ñòðóêòóðíûõ çàáîëåâàíèé ñåðäöà è áåç ñèìïòîìîâ ðåêîìåíäóåòñÿ òîëüêî íàáëþäåíèå è óñïîêîåíèå, à íå ìåäèêàìåíòîçíàÿ òåðàïèÿ èëè êàòåòåðíàÿ àáëÿöèÿ (2B).

Áåññèìïòîìíûå ïàöèåíòû ñî ñòðóêòóðíûì çàáîëåâàíèåì ñåðäöà:
- Äëÿ ïàöèåíòîâ áåç ÿâíûõ ñèìïòîìîâ, ñâÿçàííûõ ñ ÆÝ, íî ó êîòîðûõ âûÿâëåíî ñòðóêòóðíîå çàáîëåâàíèå ñåðäöà, îñíîâíîå âíèìàíèå óäåëÿåòñÿ íàçíà÷åíèþ ñîîòâåòñòâóþùåé òåðàïèè, àäàïòèðîâàííîé ê êîíêðåòíîìó îñíîâíîìó çàáîëåâàíèþ. ×àñòî, îäíàêî, êîìïîíåíòû ýòîé òåðàïèè (íàïðèìåð, áåòà-áëîêàòîðû) ìîãóò ñïîñîáñòâîâàòü ñíèæåíèþ ÆÝ.

Ïàöèåíòû ñ ñèìïòîìàìè:
- Óìåíüøèòü èëè óñòðàíèòü ëåæàùèå â îñíîâå òðèããåðû. Ïåðåä íà÷àëîì òåðàïèè ñëåäóåò âûÿâèòü è óñòðàíèòü óñòðàíÿåìûå ïðè÷èíû èëè òðèããåðû (íàïðèìåð, àëêîãîëü èëè íàïèòêè, ñîäåðæàùèå êîôåèí, íàðêîòèêè, çàáîëåâàíèÿ ùèòîâèäíîé æåëåçû, íàðóøåíèÿ ýëåêòðîëèòíîãî áàëàíñà).

- Ìåäèêàìåíòîçíàÿ òåðàïèÿ. Äëÿ áîëüøèíñòâà ïàöèåíòîâ ñ ñèìïòîìàòè÷åñêèìè ÆÝ ðåêîìåäóåòñÿ òåðàïèÿ ïåðâîé ëèíèè áåòà-áëîêàòîðàìè èëè, ðåæå, íåäèãèäðîïèðèäèíîâûìè áëîêàòîðàìè êàëüöèåâûõ êàíàëîâ, à íå àíòèàðèòìè÷åñêèìè ïðåïàðàòàìè èëè êàòåòåðíîé àáëÿöèåé (2Ñ). Ýòî îñíîâàíî â îñíîâíîì íà ïðîôèëå áåçîïàñíîñòè áåòà-áëîêàòîðîâ è áëîêàòîðîâ êàëüöèåâûõ êàíàëîâ, êîòîðûå, êàê ïðàâèëî, áîëåå áëàãîïðèÿòíû, ÷åì ó äðóãèõ àíòèàðèòìè÷åñêèõ ïðåïàðàòîâ.

- Êàòåòåðíàÿ àáëÿöèÿ (Ð×À) äëÿ ðåôðàêòåðíûõ ïàöèåíòîâ è äëÿ òåõ, êòî íå ïðåäïî÷èòàåò äëèòåëüíóþ àíòèàðèòìè÷åñêóþ òåðàïèþ. Äëÿ ïàöèåíòîâ, ó êîòîðûõ ëèáî ñîõðàíÿþòñÿ ñèìïòîìû, ñâÿçàííûå ñ ÆÝ, íåñìîòðÿ íà íà÷àëüíóþ ìåäèêàìåíòîçíóþ òåðàïèþ, ëèáî îíè íå ïåðåíîñÿò ìåäèêàìåíòîçíóþ òåðàïèþ èç–çà ïîáî÷íûõ ýôôåêòîâ, ëèáî ïðåäïî÷èòàþò íå ïîëó÷àòü àíòèàðèòìè÷åñêóþ òåðàïèþ, äîïóñêàåòñÿ âûïîëíåíèå êàòåòåðíîé àáëÿöèè, âìåñòî àíòèàðèòìè÷åñêîé ìåäèêàìåíòîçíîé òåðàïèè, â êà÷åñòâå ñëåäóþùåãî ýòàïà ëå÷åíèÿ (2Ñ). Êàòåòåðíàÿ àáëÿöèÿ, âåðîÿòíî, èìååò áîëüøóþ äîëãîñðî÷íóþ ýôôåêòèâíîñòü è ïîçâîëÿåò èçáåæàòü òîêñè÷íîñòè äëèòåëüíîé àíòèàðèòìè÷åñêîé ëåêàðñòâåííîé òåðàïèè. Ëå÷åíèå àíòèàðèòìè÷åñêèìè ïðåïàðàòàìè êëàññà IC ÿâëÿåòñÿ ðàçóìíîé àëüòåðíàòèâîé äëÿ ïàöèåíòîâ áåç ÿâíûõ ñòðóêòóðíûõ çàáîëåâàíèé ñåðäöà, êîòîðûå íå èìåþò äîñòóïà ê êàòåòåðíîé àáëÿöèè èëè ïðåäïî÷èòàþò èçáåãàòü èíâàçèâíîé ïðîöåäóðû. Îäíàêî ñóùåñòâóåò î÷åíü îãðàíè÷åííîå êîëè÷åñòâî áåçîïàñíûõ è ýôôåêòèâíûõ àíòèàðèòìè÷åñêèõ ïðåïàðàòîâ äëÿ ïàöèåíòîâ ñ ñîïóòñòâóþùèìè ñòðóêòóðíûìè çàáîëåâàíèÿìè ñåðäöà.

Ïàöèåíòû ñ âûñîêîé äîëåé ÆÝ è êàðäèîìèîïàòèåé, âûçâàííîé ÆÝ, ñî ñíèæåííîé ôðàêöèåé âûáðîñà ëåâîãî æåëóäî÷êà (ÔÂËÆ < 50%). Âåäåíèå ïàöèåíòîâ ñî ñíèæåííîé ÔÂËÆ, ïðåäïîëîæèòåëüíî ñâÿçàííîé ñ ÷àñòûìè ÆÝ, ñëåäóþùåå:
Áåññèìïòîìíûå ïàöèåíòû. Äëÿ áåññèìïòîìíûõ ïàöèåíòîâ ñ êàðäèîìèîïàòèåé ñî ñíèæåííîé ÔÂËÆ (<50%), ïðåäïîëîæèòåëüíî âûçâàííîé âûñîêîé äîëåé ÆÝ, ðåêîìåíäóåòñÿ íà÷àëüíàÿ ìåäèêàìåíòîçíàÿ òåðàïèÿ áåòà-áëîêàòîðàìè èëè íåäèãèäðîïèðèäèíîâûìè áëîêàòîðàìè êàëüöèåâûõ êàíàëîâ (2Ñ). Ñóùåñòâóåò íèçêèé ïîðîã äëÿ âûïîëíåíèÿ êàòåòåðíîé àáëÿöèè, åñëè äîëÿ ÆÝ îñòàåòñÿ âûñîêîé èëè ÔÂËÆ íå óëó÷øàåòñÿ.

Ïàöèåíòû ñ ñåðäå÷íîé íåäîñòàòî÷íîñòüþ (ÑÍ). Äëÿ ïàöèåíòîâ ñ ñèìïòîìàìè ÑÍ èç-çà ïîäîçðåíèÿ íà ÆÝ-èíäóöèðîâàííóþ êàðäèîìèîïàòèþ ñî ñíèæåííûì ÔÂËÆ (<50%) öåëåñîîáðàçíî íà÷àëüíîå ëå÷åíèå ñ ïîìîùüþ ðàäèî÷àñòîòíîé êàòåòåðíîé àáëÿöèè â äîïîëíåíèå ê ìåäèêàìåíòîçíîé òåðàïèè áåòà-áëîêàòîðàìè (2Ñ).

vii. Áåòàêñîëîë 5 ìã ïåðîðàëüíî 1 ðàç â äåíü; ìàêñèìàëüíàÿ äîçà 20 ìã 1 ðàç â äåíü
viii. Ïðîïðàíîëîë 10 ìã ïåðîðàëüíî 2 èëè 3 ðàçà â äåíü (àëüòåðíàòèâíî ïðîïðàíîëîë äëèòåëüíîãî äåéñòâèÿ 80 ìã ïåðîðàëüíî 1 ðàç â äåíü); ìàêñèìàëüíàÿ äîçà 320 ìã â äåíü; òåðàïèÿ ïåðâîãî âûáîðà â ïåäèàòðè÷åñêîé ãðóïïå, ïðè íåêîòîðûõ êàíàëîïàòèÿõ è ãèïåðòèðåîçå.
*Íå èìååò êà÷åñòâåííûõ äîêàçàòåëüñòâ åãî ïðîãíîñòè÷åñêîé ýôôåêòèâíîñòè ïðè æåëóäî÷êîâûõ àðèòìèÿõ.

b. Íåäèãèäðîïèðèäèíîâûå êàëüöèåâûå áëîêàòîðû (ìîãóò áûòü ïðåäïî÷òèòåëüíûìè ó ïàöèåíòîâ ñ ÀÃ èëè ó ïàöèåíòîâ ñ ôàñöèêóëÿðíûìè ÆÝ, íî íå äîëæíû ïðèìåíÿòüñÿ ó ïàöèåíòîâ ñ ÑÍ èëè êàðäèîìèîïàòèåé)

i. Äèëòèàçåì 60 ìã ïåðîðàëüíî 2 èëè 3 ðàçà â äåíü; èëè àëüòåðíàòèâíî, äèëòèàçåì ïðîëîíãèðîâàííîãî âûñâîáîæäåíèÿ: îò 120 äî 180 ìã ïåðîðàëüíî 1 ðàç â äåíü; ìàêñèìàëüíàÿ äîçà 480 ìã â äåíü.
ii. Âåðàïàìèë 80 ìã ïåðîðàëüíî 3 ðàçà â äåíü; àëüòåðíàòèâíî, âåðàïàìèë ïðîëîíãèðîâàííîãî âûñâîáîæäåíèÿ: 240 ìã ïåðîðàëüíî 1 ðàç â äåíü; ìàêñèìàëüíàÿ äîçà 480 ìã â äåíü.
5. Ñàìîêîíòðîëü ÝÊà (ðàçëè÷íûìè äåâàéñàìè – óìíûå ÷àñû, ñïåöèàëüíûå ïîðòàòèâíûå êàðäèîãðàôû, òåëåìåäèöèíà) âî âðåìÿ ñèìïòîìîâ óëó÷øàåò êà÷åñòâî æèçíè è ñíèæàåò òðåâîæíîñòü.
 èññëåäîâàíèå áûëà âêëþ÷åíà 821 æåíùèíà â âîçðàñòå 21-88 ëåò (â ñðåäíåì 57 ± 11 ëåò), ó êîòîðûõ áûëè ñèìïòîìû ó÷àùåííîãî ñåðäöåáèåíèÿ, íàáðàííûå ñ ïîìîùüþ ñîöèàëüíûõ ñåòåé.  òå÷åíèå 60 äíåé ó÷àñòíèêè èñïîëüçîâàëè ïîðòàòèâíîå óñòðîéñòâî äëÿ çàïèñè ýëåêòðîêàðäèîãðàììû (ÝÊÃ) (Coala Heart Monitor), ïîäêëþ÷åííîå ê èõ ñìàðòôîíàì. ÝÊà çàïèñûâàëàñü äâà ðàçà â äåíü è ïðè íàëè÷èè ñèìïòîìîâ è àâòîìàòè÷åñêè èíòåðïðåòèðîâàëàñü àëãîðèòìîì ñ íåìåäëåííûì îòâåòîì ïîëüçîâàòåëþ. Äîáðîêà÷åñòâåííûå àðèòìèè òàêæå àíàëèçèðîâàëèñü âðó÷íóþ. Âîïðîñíèêè, êàñàþùèåñÿ òðåâîãè è äåïðåññèè - Áîëüíè÷íàÿ øêàëà òðåâîãè è äåïðåññèè (HADS), ãåíåðàëèçîâàííîå òðåâîæíîå ðàññòðîéñòâî (GAD-7), HRQOL (RAND-36) è Êîíòðîëüíûé ñïèñîê ñèìïòîìîâ: ÷àñòîòà è òÿæåñòü (SCL) - áûëè ïðîàíàëèçèðîâàíû äî è ïîñëå ñáîðà äàííûõ.  îáùåé ñëîæíîñòè áûëî àâòîìàòè÷åñêè ïðîàíàëèçèðîâàíî 101 804 çàïèñè ÝÊÃ.  94% ñëó÷àåâ áûë çàðåãèñòðèðîâàí ñèíóñîâûé ðèòì èëè ïðåæäåâðåìåííîå ñîêðàùåíèå ïðåäñåðäèé / æåëóäî÷êîâ; â 6% ñëó÷àåâ áûëà çàðåãèñòðèðîâàíà ôèáðèëëÿöèÿ ïðåäñåðäèé èëè ñóïðàâåíòðèêóëÿðíàÿ òàõèêàðäèÿ. Ïîìèìî ïðåæäåâðåìåííûõ ñîêðàùåíèé æåëóäî÷êîâ, íèêàêèõ æåëóäî÷êîâûõ àðèòìèé çàðåãèñòðèðîâàíî íå áûëî. Òðåâîãà è äåïðåññèÿ (HADS è GAD-7) óìåíüøèëèñü (Ð < 0,001), êàê è ÷àñòîòà è òÿæåñòü ñèìïòîìîâ, à HRQOL óâåëè÷èëñÿ âî âñåõ îáëàñòÿõ (Ð < 0,001) ïðè 2-ìåñÿ÷íîì íàáëþäåíèè. Ðåãèñòðàòîðû ñîáûòèé áîëåå ïîëåçíû ïðè ñåðäöåáèåíèÿõ è ìåíåå ïðè ñèíêîïàõ è ïðåñèíêîïàõ, âåðîÿòíî ëþäè íå óñïåâàþò èõ èñïîëüçîâàòü [Fogel RI, Evans JJ, Prystowsky EN. Íî ÷ðåçìåðíûé ñàìîêîíòðîëü ñïîñîáåí ïðèâåñòè è ê îáðàòíîìó ýôôåêòó (ïîâûøåíèþ òðåâîãè è/èëè óãëóáëåíèþ äåïðåññèè).

6. Àíòèàðèòìèêè äðóãèõ ãðóïï – ïîìíèòü ïðî íå íàâðåäè! Ñàìûõ ïðîàðèòìîãåííûõ (ñîòàëîë, äðîíåäàðîí) ëó÷øå âñÿ÷åñêè èçáåãàòü! Ïîìíèòü î ãëîáàëüíîé öåëè ëå÷åíèÿ (ïðîäëåíèå êà÷åñòâåííîé æèçíè), ò.å. áåçîïàñíîñòü âî ãëàâå óãëà. Ïðåçóìïöèÿ âèíîâíîñòè: íåèçó÷åííûé àíòèàðèòìèê ñ÷èòàòü ïîòåíöèàëüíî îïàñíûì ïîêà íå äîêàçàíû åãî áåçîïàñíîñòü è ýôôåêòèâíîñòü. a. Ïàöèåíòû áåç ÿâíûõ ñòðóêòóðíûõ çàáîëåâàíèé ñåðäöà (òðåáóåòñÿ îäíîçíà÷íîå èñêëþ÷åíèå ÈÁÑ ìåòîäîì ñòðåññ-ÝõîÊÃ)*:
i. Ôëåêàèíèä* íà÷àëüíàÿ äîçà îò 50 äî 100 ìã ïåðîðàëüíî 2 ðàçà â äåíü; ìàêñèìàëüíàÿ ñóòî÷íàÿ äîçà 150 ìã 2 ðàçà â äåíü (õîòÿ äîçû âûøå 100 ìã 2 ðàçà â äåíü èñïîëüçóþòñÿ ðåäêî)
ii. Ïðîïàôåíîí* 150 ìã 3 ðàçà â äåíü; ìàêñèìàëüíàÿ ñóòî÷íàÿ äîçà 300 ìã 3 ðàçà â äåíü

b. Ïàöèåíòû ñî ñòðóêòóðíûìè çàáîëåâàíèÿìè ñåðäöà:
i. Àìèîäàðîí ÿâëÿåòñÿ íàèìåíåå ïðîàðèòìè÷åñêèì ñðåäñòâîì ó ïàöèåíòîâ ñî ñòðóêòóðíûìè çàáîëåâàíèÿìè ñåðäöà, õîòÿ è ñî çíà÷èòåëüíîé èçâåñòíîé òîêñè÷íîñòüþ äëÿ îðãàíîâ ïðè äëèòåëüíîì ïðèìåíåíèè. Ìîæíî èñïîëüçîâàòü ïåðîðàëüíûå íàãðóçî÷íûå äîçû îò 400 äî 1200 ìã/ñóò â ðàçäåëåííûõ äîçàõ (äî îáùåé íàãðóçî÷íîé äîçû îò 6 äî 10 ãðàìì). Îáû÷íàÿ ïîääåðæèâàþùàÿ äîçà äîëæíà áûòü ñàìîé íèçêîé ýôôåêòèâíîé äîçîé, êîòîðàÿ îáû÷íî ñîñòàâëÿåò 200 ìã â äåíü, íî èíîãäà ìîæåò äîñòèãàòü 100 ìã â äåíü.
ii. Ðàíîëàçèí, ïåðîðàëüíûé àíòèàíãèíàëüíûé ïðåïàðàò, îáëàäàþùèé ñâîéñòâàìè, áëîêèðóþùèìè I Na è I Kr, îäîáðåí òîëüêî äëÿ ëå÷åíèÿ õðîíè÷åñêîé ñòàáèëüíîé ñòåíîêàðäèè; îäíàêî áûëî ïîêàçàíî, ÷òî îí ñíèæàåò áðåìÿ æåëóäî÷êîâûõ àðèòìèé â èññëåäîâàíèè MERLIN-TIMI 36 [Scirica BM, Morrow DA, Hod H, et al. Effect of ranolazine, an antianginal agent with novel electrophysiological properties, on the incidence of arrhythmias in patients with non ST-segment elevation acute coronary syndrome: results from the Metabolic Efficiency With Ranolazine for Less Ischemia in Non ST-Elevation Acute Coronary Syndrome Thrombolysis in Myocardial Infarction 36 (MERLIN-TIMI 36) randomized controlled trial.  ìåòà-àíàëèçå íåáîëüøèõ íåðàíäîìèçèðîâàííûõ èññëåäîâàíèé 2014 ãîäà ðàäèî÷àñòîòíîé àáëÿöèè äëÿ ëå÷åíèÿ èäèîïàòè÷åñêèõ ÆÝ, ïðîèñõîäÿùèõ èç âûâîäíîãî òðàêòà ïðàâîãî æåëóäî÷êà, êàòåòåðíàÿ àáëÿöèÿ áûëà ñâÿçàíà ñî çíà÷èòåëüíûì ñíèæåíèåì äîëè ÆÝ (ñðåäíåå ñíèæåíèå ïîñëå àáëÿöèè íà 97%) íàðÿäó ñî çíà÷èòåëüíûì óëó÷øåíèåì ÔÂËÆ (ñðåäíåå óâåëè÷åíèå íà 10%), õîòÿ ìåòà-àíàëèç áûë îãðàíè÷åí çíà÷èòåëüíîé íåîäíîðîäíîñòüþ ñðåäè èññëåäîâàíèé [Lamba J, Redfearn DP, Michael KA, et al.
