Не-не, при гиперальдостеронизме таких вещей не бывает. Там даже не чтобы судороги, а парестезия и мышечная слабость. Причем всё это на фоне тяжелой гипокалиемии.

Я пока на сутках не имею возможности посмотреть видео, но на всякий случай 1) эпилептические и неэпилептические приступы бывают почти неразличимы внешне, 2) могут сочетаться у одного и того же пациента.

Калий у нее всегда низковат, 3,3-3,5. Артериальное давление и ЧСС во время приступа всегда возрастают пропорционально его интенсивности. Даже первые приступы, которые сопровождались только лишь потерей сознания, были с гипертензией и тахикардией (первоначальной версией о причине была даже кардиологическая, но быстро отпала).

Да, компенсируется, пока капаем, пожизненно не будешь же его капать... Верошпирон 200 мг в сутки уже 2 года кажется.

Каким образом доказано наличие гиперальдостеронизма? Альдостерон/ренин? Это первичный ГА или вторичный? Как сформулирован эндокринологической диагноз?

Уважаемая Елена, ну нет данных, связывающих гиперальдостеронизм и судороги. Кроме указаний в энциклопедии, что "может быть". Возникновение судорог, по литературным данным, возможно двумя путями. Гипертоническая энцефалопатия, со всеми сопутствующими проявлениями. В том числе и визуализируемыми очагами повреждения головного мозга. И как следствие расстройства электролитного обмена и кислотно-основного состояния, метаболического гипокалиемического алкалоза. Но речь идет о тяжелых формах нарушений, которым обязательно сопутствует другая симптоматика, кроме неврологической. Говоря о тяжести, во всех случаях гипокалиемии речь идет о цифрах значительно ниже 3 мМоль/л. Нет упоминаний о судорогах в описании гиперальдостеронизма самого по себе например тут [Ссылки доступны только зарегистрированным пользователям ]. UpToDate молчит, как партизан. Медскейп на запрос "Hyperaldosteronism seizures" выдал целых 19 ссылок. Везде вторично, по механизму указанному выше. Алкалоз. Тяжелый. PubMed - 6 ссылок. Мимо. Большой талмуд Felig, Philip; Frohman, Lawrence A. Endocrinology & Metabolism, 4th Edition - тишина. Буду весьма признателен, если Вы предоставите ссылку хотя бы на кейс, где описывались бы судороги как проявление гиперальдостеронизма. Похожих на обсуждаемый случай. Спасибо.

И скорее всего, во втором случае основную роль в судорожном синдроме может играть не гипокалемия, а именно метаболический алкалоз, как показали экспериментальные данные:

Посмотрел видео. Живот видно, конечно, но неврологу интереснее другие части тела. Так что ничего не доказано и сразу останавливаться на пси-версии я бы не стал.

Так можно не сразу. Записывать ЭЭГ при согласии пациентки может каждый желающий. Осталось найти желающего, который бы еще это умел. Будет чем занять пациентку, пока ищется психиатр.

Еще раз, я не отрицаю, что у пациентки есть неэпилептические приступы. Но "наше" пока не исключено, надо доработать.

К вопросу альдостерона: нет у нее алкалоза, ацидозы у нее; первые разы -5 - -6 мы, кажется, поймали просто после приступа; потом эти цифры стойко держались на фоне дачи диакарба (по поводу выявленного повышенного ликворного давления), считаем, что это от него. Причем коррекции не поддается. То есть капаешь 200 4% гидрокарбоната и через 6 часов получаешь ту же цифру. Диакарб уже не даем. Последние дни вроде около -2, но алкалоза не было никогда.

Non-epileptic Seizures Synonyms: the use of the terms hysterical seizures or pseudoseizures is now considered to be inappropriate. Non-epileptic seizures (NES) is a descriptive term for a diverse group of disorders which refers to paroxysmal events that can be mistaken for epilepsy, but are not due to an epileptic disorder. There are two sub-categories of NES:. •Physiological: includes a broad spectrum of disorders, e.g. syncope, paroxysms of acute neurological insults, paroxysmal toxic phenomena, non-toxic organic hallucinosis, non-epileptic myoclonus, sleep disorders, paroxysmal movement disorders, paroxysmal endocrine disturbances and transient ischaemic attacks (TIAs). •Psychogenic seizures include different types:1 ◦Dissociative seizures are involuntary and happen unconsciously. This is the most common type of NES and the person has no control over the seizures. ◦Associated with psychiatric conditions that cause seizures, e.g. panic attacks. ◦Factitious seizures: seizures are consciously or deliberately ‘put on’, e.g. Münchhausen's syndrome. .Epidemiology •The true prevalence is unknown. •Up to one fifth of patients who present to specialist clinics with seizures do not have epilepsy. Treatment regimes for NES include non-psychological, e.g. anti-anxiety and antidepressant medication and psychological therapies (including cognitive behavioural therapy, hypnotherapy and paradoxical injunction therapy). With paradoxical injunction therapy, the therapist imposes a directive that places the client in a therapeutic double bind that promotes change regardless of the client's compliance with the directive. •There is currently no reliable evidence to support the use of any treatment, including hypnosis or paradoxical injunction therapy, in the treatment of NES.4 .Prognosis2 •A recent review found that, after a mean follow-up of three years, about two thirds of patients continued to have dissociative seizures and more than half remained dependent on social security. •Receiving psychiatric treatment has been associated with a positive outcome in some studies, but not in others. •A poor prognosis is predicted by a long delay in diagnosis and the presence of psychiatric comorbidity, including personality disorder. |

Конец истории (или не конец?) Проконсультировала психиатр-эпилептолог, на основании анамнеза и беседы с пациенткой дала заключение, что у пациентки (не буду перевирать современную терминологию, но на языке простых анестезиологов) - истерия. Письменно рекомендовала перевод в пс.учреждение "для уточнения диагноза" и "для безопасной отмены противосудорожных препаратов". Перевод был договорен, но узнав об этом пациентка сказалась здоровой и попросилась домой, куда и была отпущена под расписку в И/Б. Будет продолжение - расскажу...
