Do Early Skin Care Practices Alter the Risk of Atopic Dermatitis? A Case-Control Study

Our study highlights the widespread use of skin care practices in neonates and infants, which may be potentially detrimental to the skin barrier. Because of the unanticipated high rate of moisturizer use in all groups, we were unable to determine whether the regular use of a moisturizer altered the risk for AD development. Despite current skin care guidelines that do not recommend regular moisturizer use in neonates (4), the majority of all caregivers in this population used them on a regular basis, with the vast majority of parents using watery lotions. Parents also bathed and cleansed their infants with soap or cleansers on a frequent basis. Water exposure (e.g., via bathing or possibly lotion use) and cleansing are known to have detrimental effects on the skin barrier (5). Skin care practices that harm the skin barrier may potentially promote AD development. Our data reveal the importance of further research on the effect cleansing and moisturizing may have on neonatal skin physiology, given its routine use in some populations at risk for AD.

TFFD is one of the dirtlike dermatoses (4), along with dermatitis neglecta, confluent and reticulated papillomatosis, and acanthosis nigricans. The differential diagnosis of TFFD should also include the “dirty neck syndrome” of atopic dermatitis (5), which is more common in adults, is macular, and cannot be removed with alcohol swabbing. Because TFFD is readily identified and treated, biopsies have rarely been taken, limiting the availability of histopathological analyses that might shed light on why this condition occurs. In addition to the sparse histopathologic information, clinical data regarding TFFD is limited in the literature, which previously contained no large series.
It is important for pediatricians and dermatologists to recognize TFFD. In addition to the many patients who experienced painful attempts at cleansing (by parents or patients themselves), more than one-fifth of the cases here (and many in the literature) had unnecessary prescriptions or blood tests. Recognizing and treating this condition can be gratifying for physicians, who can provide almost instant relief to their sometimes stunned patients.

Conclusions Allele G of rs9468925 on HLA-C-HLA-B may be associated with a higher risk of vitiligo. Our study showed a significant genotypic variation between patients with age of onset ?20*years and age of onset >20*years. To conclude, depigmentation therapies are the last resort for extensive vitiligo cases. These therapies are evolving and new therapies have come in vogue apart from MBEH and 4 MP. In future, some experimental products may become mainstay of depigmentation therapies.

Например, при гнездной алопеции рекомендуют

Бетаметазон, мазь, наружно 2 р/сут, 2 месяца.

ИМХО. Нельзя столь длительно непрерывно наносить на кожу мазь Бетаметазон, так как это может привести к атрофии кожи.

In summary, the current study confirms the importance of S.*aureus as a trigger for AD flares. Evaluation of wash fluid samples by quantitative bacterial culture from clinically impetiginized AD lesions before and after defined treatment revealed improvement in staphylococcal CFU and EASI scores and lower levels of staphylococcal products and inflammatory cytokines. A subset of patients whose AD lesions were impetiginized with MRSA improved with a treatment regimen that included oral cephalexin as the only antimicrobial. The presence of high levels of defensins in wash fluid specimens of AD lesions after treatment provides a mechanism by which the S.*aureus bacterial superinfection is resolved.

However, little is known about their combined effect on acne. A multicentre, double-blind, 12-week randomized vehicle and parallel-active control study was conducted by clinical and biophysical non-invasive measurements to evaluate the efficacy, tolerability and safety of a 4% nicotinamide-phospholipidic (N-PHCL) emulsion vs. 1% topical clindamycin phosphate applied once daily. Four percentage N-PHCL cosmetic treatment resulted slightly superior to topical clindamycin with all the parameters studied for its better compliance and the global clinical improvement.

Demodex has been considered to be related with multiple skin disorders, but controversy persists. In this case-control study, a survey was conducted with 860 dermatosis patients aged 12 to 84 years in Xi’an, China to identify the association between facial dermatosis and Demodex. Amongst the patients, 539 suffered from facial dermatosis and 321 suffered from non-facial dermatosis. Demodex mites were sampled and examined using the skin pressurization method. Multivariate regression analysis was applied to analyze the association between facial dermatosis and Demodex infestation, and to identify the risk factors of Demodex infestation. The results showed that total detection rate of Demodex was 43.0%. Patients aged above 30 years had higher odds of Demodex infestation than those under 30 years. Compared to patients with neutral skin, patients with mixed, oily, or dry skin were more likely to be infested with Demodex (odds ratios (ORs) were 2.5, 2.4, and 1.6, respectively). Moreover, Demodex infestation was found to be statistically associated with rosacea (OR=8.1), steroid-induced dermatitis (OR=2.7), seborrheic dermatitis (OR=2.2), and primary irritation dermatitis (OR=2.1). In particular, ORs calculated from the severe infestation (?5 mites/cm2) rate were significantly higher than those of the total rate. Therefore, we concluded that Demodex is associated with rosacea, steroid-induced dermatitis, seborrheic dermatitis, and primary irritation dermatitis. The rate of severe infestation is found to be more correlated with various dermatosis than the total infestation rate. The risk factors of Demodex infestation, age, and skin types were identified. Our study also suggested that good hygiene practice might reduce the chances of demodicosis and Demodex infestation.

Conclusion* The OSI is a new, simple, objective, reproducible numeric system to grade the severity of onychomycosis.
The definition of onychomycosis has been well established; however, there is no
system to evaluate and grade the clinical severity of infection. Most published
reports list poor prognostic factors but do not allow the evaluator to categorize the
nail as mild, moderate, or severe. Using approximately 100 photos of onychomycotic
nails of varying severity, a consensus panel developed a new classification
system based on a several clinical factors to define mild, moderate, and severe
onychomycosis. The scoring model created is similar to the severity index score
used for psoriasis and nail psoriasis and named the Onychomycosis Area Severity
Index Score (OASIS). By defining severity of infection, it may be possible to better
predict the response to therapy and tailor therapy to severity of the disease.

In this review, we focus on the role of the skin pH in dermatophytosis and the possible mechanisms involved in the adaptive response of dermatophytes to ambient pH changes. Model fungi, in which the pH regulatory system has been extensively characterised, were used as comparative examples.Numerous studies, starting at the end of the nineteenth century, have confirmed the acidic nature of the skin and nail plates.4 The average skin surface and nail plate pH is 4.7, with the skin presenting a pH gradient in which the upper layers are acidic and the pH increases in the deeper layers,5 whereas the pH of the nail plate surface is apparently higher than its interior.6 The skin pH gradient is important for the proper regulation of the activity of enzymes involved in the keratinisation process that is essential for the removal of dead cells and renewal of the stratum corneum, which prevents the colonisation of the skin by harmful microorganisms.4

Leukonychia totalis (LT) is a rare condition with autosomal dominant inheritance.1 Hereditary LT has often been reported in conjunction with other dermatoses, including nail features.2–6 To our knowledge, few cases of sporadic LT with koilonychia have been reported.7,8
A 22-year-oldman was referred to the Department of Dermatology at St. Mary’s Hospital for white concave nails since birth (Figs*1 and 2). He had never complained of any pain or decreased sensation in his nails or digits. He was a student without any notable medical history or trauma to his nails or digits, and was otherwise healthy. This condition was not changed by temperature, activity, or stress. The nail color was uniformly white on the fingernails and on the toenails except for some linear streaks on the right great toenail. All the nails were concave with everted edges (so-called “spoon nails”) and smooth surfaces. There was no family history of leukonychia or koilonychia. A complete chemistry profile including iron profile, complete blood count with differential, and urinalysis were unremarkable.
Leukonychia refers to whitening of the nail plate and can be classified according to the distribution of white coloring or the pattern of nail involvement. This condition can be temporary or permanent depending on its etiology.9
True leukonychia occurs in response to a structural abnormality of the nail plate and the nail matrix. Normally, the nail plate consists of cornified cells, but in leukonychia a defect occurs in the keratinization of cells so that immature, large nucleated cells are present. The keratohyaline-containing cells reflect light and prevent the visualization of the pink vascular bed. The cells produce an appearance that is milky, chalky, bluish, ivory or porcelain white in color, and the opacity of the whiteness varies. When the nails are faintly opaque, transverse streaks are seen.3,10
True leukonychia can be separated into total and subtotal or partial forms, and the latter can occur in a punctate, striate or distal manner. Koilonychia is commonly found in the nails of normal children, but usually disappears spontaneously. All the fingernails may be involved and, less frequently, the toenails. Proliferation of connective tissue and sometimes increased vascular flow causes the fingernails to lift up. The depressed distal portion of the affected toenails may reflect distal connective tissue anoxia and atrophy.

Conclusions* Current smokers with lupus erythematosus had worse disease, had worse quality of life, and were more often treated with a combination of hydroxychloroquine and quinacrine than were nonsmokers. Never and past smokers showed greater improvement when treated with antimalarial agents plus at least 1 additional immunomodulator. Complete clinical examination, including careful inspection of genital mucosa, should therefore be mandatory in patients with morphea because genital LS bears a risk of evolution into squamous cell carcinoma and thus needs treatment with topical corticosteroids.
