Широко нет, но если по каким-то причинам другой венозный доступ невозможен или не хочется теребить ребенка по тяжести его(е) состояния, тогда да.

По поводу того, чего было сказано о пупочных катетрах: не все пупочные катетры приводят к NEC!!! Нельзя отказываться от этого хорошего центрального доступа, потому-что у одного ребенка с пупочным катетром был NEC. Мы используем этот доступ особено у новорожденных с эсктремно ниским весом при рождении (< 1000 g). NEC чаще всего наблюдали у детей с инфекциями, а не у таких с пупочным катетром. Вопрос о катетр-связанной инфекции обсуждают на многих иностранных форумах, но речь обычно идет о эпидемиологической обстановке в отделение новорождиных...

Вливание растворов обходя естественную фильтрацию в обход печени, я считаю не нормальным. Таким образом увеличивается риск развития септицемии

Может быть я оговорился. Имел в виду, что вливание растворов через воротную вену менее физиологичнее, чем через центральный сосуд. Кстати еще вопрос: "А, сколько дней Вы держите линию в сосуде?" Есть ли какие-либо нормы?

Все таки я думаю, что на счет "физилогичности вливаний" я могу поспорить:
1. После формирования плаценты путь поступления питательных субстанций идет через умбиликальные сосуды. Вводя катетр через умбиликальную вену, мы поддерживаем этот естественный путь!!! Катетр вводим не в v.portae, а в v.cava inf. Это контролируем потом на рентгенографии и уровень катетра должен быть Th-7. 2. Манипуляция проводиться в асептических условиях по всем правилам хирургических манипуляций - речь не идет о родильном зале, а о NICU (отделение интенсивной терапии новорожденных). Так что септицемия будет говорить о нарушений правил асептики. 3. Речь не идет тоже о введение пупочных катетров всем новорожденных - приоритет имеют самые маловесные (< 1000 g) и дети в тяжелом состоянии, напр. шок, когда колляпс периферии является сериозным проблемом для трансфузий медикаментов... А когда в родильном зале проводится реанимация новорожденного в тяжелой асфиксии и в терапевтическом плане требуется введение NaHCO3 например, используем венепункцию пупочной вены через пуповину. |

Ну, хорошо, Вы меня убедили!
Спорить больше не буду. Может стоит пересмотреть свою точку зрения, ведь от пупочных катетеров в практике 5-10 лет назад я видел больше вреда, чем пользы. Потерял связь с временем. Хотя от кратковременных введений растворов при реанимации новорожденного в родзале я никогда не отказывался. А, что можете сказать о длительности стояния, о заборе крови, тромбировании. Как я уже говорил, у себя в отделении Мы не пользуемся таким доступом к венозному руслу. Что можете посоветовать? А, о положительных качествах все понятно. |

Здравствуйте! Очень рад, что среди коллег идет такой обмен мнениями по поводу проблем связанных с установкой и использованием ЦВК. В нашей детской реанимации благодаря использование новых технологий; установки ЦВК с предварительной ультразвуковой визуализацией вн. яремной вены, ЭКГ контроля позиции ЦВК и разработанной методики исправления порочного стояния ЦВК существенно снизился процент осложнений как при пункциях и катетеризациях ЦВ так и от проведения интенсивной терапии. Верификация положения ЦВК явл. у нас обязательной процедурой т.к. мы уделяем существенное значение мониторингу ЦВД. Существует единый протокол установки ЦВК используются только катетры Certofix Braun. Уважаемые коллеги осуществляется ли вами мониторинг ЦВД и каковы на ваш взгляд нормальные величины ЦВД у детей в зависимости от возраста? Осуществляются ли в ваших ОРИТ бак. посевы удаляемых катетеров и встречались ли вы с ситуацией когда при бак. исследовании удаленного ЦВК имелся различный бактериологический пейзаж отличающийся также по чувствительности к антибиотикам с наружной и внутренней стороны ЦВК?

Прочитав Ваше обсуждение, решил опубликовать короткие, но вполне достаточные для начала рекомендации по введению некоторых центральных сосудистых катетеров новорожденным. При необходимости инфузия начинается всегда либо с периферической вены, либо - с пупочной ( в зависимости от веса и технических возможностей), потом ( при необходимости длительной инфузии) устанавливаются чрезкожные линии."Подключички" у новорожденных уже практически не используются. ------------------------------------------------ -Percut.centr.ven. cath Indications • Administer intravenous solutions w/ high osmolality, eg, hypertonic glucose, parenteral nutrition solution • Secure access for critical medications (eg, ionotropes, prostaglandin E1) Contraindications Absolute • Unstable vital signs • Infection of skin at site of insertion • Arterial insufficiency of extremity Relative • Ongoing bacteremia Special Considerations • Premature infants: more accessible veins than full term • Prone to prolonged bleeding from site of venous puncture • Prepare hemostatic cotton "ball" prior to procedure • Choice of venous site for use: antecubital>scalp>axillary>saphenous>external jugular>femoral • Do not remove tourniquet until catheter advanced into vein 2-3 cm beyond needle tip • If catheter meets resistance, very gentle massage along vein helps to advance it • Position of catheter tip in superior or inferior vena cava should be confirmed by X ray • Do not to cut catheter to length: leave excess outside of skin • If catheter tip directed retrograde into a vein, it may flip into the central vein by blood flow; recheck X ray in 12-24 h Complications • Infection: sepsis, cellulitis, septic thrombophlebitis, endocarditis » Frequency: directly related to duration catheter remains in place » Prevention: strict aseptic technique on insertion & when interrupting line – Limit interruptions of line – Remove catheter as soon as central venous access no longer req » Management: remove catheter, appropriate antibiotic Rx • Thrombosis » Frequency: more often in premature infants, depends on duration catheter in place » Management: catheter removal • Catheter occlusion, leak » Frequency: depends on quality of catheter care » Prevention: prepare IV solution w/1 unit heparin/mL » Management: catheter removal • Bleeding » Frequency: very rare w/ appropriate hemostatic technique; very rarely significant » Prevention: always prepare small haemostatic "ball" before procedure, apply it under pressure 0.5 cm below point that catheter enters vein ------------------------------------------------------------- -Percut.art. cath Indications • Measurement of blood gases for O2 tension or content (esp if PREDUCTAL monitoring desired) • Continuous measurement of arterial BP • Facilitate frequent blood sampling (usually not sole indication) Contraindications • Skin infection at site • Preexisting circulatory insufficiency in distribution of artery or inadequate collaterals • Uncorrected coagulopathy Special Considerations • Candidate arteries » Radial artery (R for preductal monitoring) » Ulnar artery (R for preductal monitoring) » Posterior tibial artery » Dorsalis pedis artery » Temporal artery: not recommended unless absolutely necessary because of risk of significant CNS sequellae (allows preductal monitoring) • Always ensure adequate collateral circulation (Allen test for radial or ulnar artery) • Technique » Avoid wrist hyperextension » Transillumination extremely useful » Prepare flush of heparinized (1 unit/1 cc) normal saline in 1-cc syringe » Insert 22 or 24G angiocatheter at 30-degree angle very slowly, observing for flashback • Precautions » Leave tips of digits exposed to detect ischemia » Use minimal (0.5-1.5 mL/h) infusion rates » Assure normal arterial waveform, easy blood withdrawal » Avoid large or rapid withdrawal of blood or bolus injections of infusate » Do not infuse hypertonic/irritating solutions or blood products Complications • Ischemia if collateral circulation inadequate » Frequency: rare » Management: remove catheter immediately; insert no other catheters in affected extremity • Vasospasm, thromboembolism, air embolism » Frequency: uncommon » Prevention: minimize infusion rate, avoid large or rapid withdrawals/infusions, avoid hypertonic/irritating solutions & blood products » Management: controversial; usually self-limited; remove if dampening of waveform or difficulty in withdrawing blood is not resolved w/ repositioning or if thromboembolism suspected • Infection: sepsis, cellulitis, abscess » Frequency: rare » Prevention: attention to aseptic technique; prophylactic antibiotic ineffective » Management: catheter removal, antibiotics if necessary • Hemorrhage due to catheter accident or coagulopathy » Frequency: rare » Management: local hemostasis; platelets, coagulation factors if deficient ----------------------------------------------------------- -Umb. art.cath. C тех пор, как была начата эта дискуссия, прошло 3 года.
За это время утекло столько воды, что и вспоминать не хочется. Широчайшее использование глубоких линий на сегодняшний день во всех роддомах Омска, "открытое" ведение пупочных катетеров, практически на "нет" сведенные операции подключичной катетеризации, программа инфекционного контроля, борьба с полипрогмазией! Вот неполный перечень тех изменений произошедших за это время! Поэтому актуальность эта дискуссия на сегодняшний день потеряла, но тогда она помогла в высокой степени! Это хорошо, когда ребёнок лежит в кувезе, а если его необходимо запеленать, тогда будет контакт пеленок с незакрытым пупочным кольцом, я правильно понимаю? Чем Вы предпочитаете обрабатывать пуповинный остаток?
