Would be good to assess her volume status to see if the high sodium is due to water loss (GI from diarrhea or GU via diabetes insipidus) or sodium gain (antibiotic esp the penicillins) or endocrinopathy (hyperaldosteronism). I'd try to give her free water to slowly correct the hypernatremia. This should help with the mental status. Corrections of hypomagnesemia and hypokalemia can help indirectly. (Hypokalemia can cause nephrogenic DI.
