INTRODUCTION
Human exposure to 0.2 ppm levels of ozone in ambient air has been shown to cause numerous pulmonary effects such as increased airway inflammation and decreased pulmonary function (1,2). Studies of ozone in animals using up to 3 ppm ozone have been shown to cause
increased airway hyperresponsivenesss and epithelial cell death. It has been hypothesized that the very high chemical reactivity of ozone limits the distribution of this gas in the pulmonary system, preventing direct exposure to the cellular components of the lung. In part ozone may react with the various components of the epithelial cell lining fluid in the lung, also known as pulmonary surfactant, which includes proteins, lipids and single electron antioxidant agents such as ascorbic acid (3-5). Due to the very high reactivity of ozone with lipids containing double bonds, considerable emphasis has been placed on the reaction of ozone with lipid compounds in the lungs and the possibility that the adverse effects of ozone are mediated by lipid ozonized products (6). Evidence in support of this theory has been accumulating with the identification of biologically active phospholipids (7) such as 1-hexadecanoyl-2-(9-oxo-nonanoyl)-glycerophosphocholine, found following ozone exposure to lung surfactant (8). This oxidized phospholipid which eluted as a somewhat polar product on normal phase HPLC was found to initiate apoptotic death in monocytes and macrophages. However, a relatively non-polar component was also found to elute from this normal phase HPLC separation that was also cytotoxic, and preliminary data suggested that several oxidized neutral lipid products were present in this fraction.
Cholesterol is the most abundant neutral lipid present in pulmonary surfactant and this molecule has a double bond that would be susceptible to attack by ozone (9,10). While there has been some controversy about the exact chemical structure of the major ozonolysis product when cholesterol is ozonized in solution at high ozone concentrations (>0.1%) (11-14), electrospray 5 tandem mass spectrometry was recently used to characterize this chemically reactive cholesterol ozonolysis product as 5-hydroperoxy-B-homo-6-oxa-cholestane-3$,7a-diol (Scheme 1) (15). Reduction of 5-hydroperoxy-B-homo-6-oxa-cholestane-3$,7a-diol has been shown to primarily
yield 3$-hydroxy-5-oxo-5,6-secocholestan-6-al (5,6-secosterol) (11-13). In fact, this reduced product has been detected in lung homogenate and broncheoalveolar lavage fluid of rats exposed to ozone, and it has thus been suggested as a biomarker for ozone exposure (16). The recent observation of 5,6-secosterol in arterial plaques has provided evidence that the formation of ozone may also occur as the result of normal biochemical events taking place in vivo during an inflammatory response (17).

It is important to consider that the interaction between ozone and cholesterol has primarily been studied in organic solvents with high levels of ozone, where 5-hydroperoxy-B-homo-6-oxacholestane-3$,7a-diol is the major product (11,14,15). However, environmentally relevant concentrations of ozone acting on lipid cellular membranes or in lipid rich pulmonary surfactant could involve different chemistry, because of the ordered nature of the lipid bilayer, yielding alternate products. Isolated bronchoalveolar lavage fluid was exposed in vitro to precise levels of ozone in a carefully controlled ozone chamber to study the formation of cholesterol derived
ozonolysis products. This revealed the formation of 5$,6$-epoxycholesterol ($-epoxide) (Scheme1) as a more abundant product than 5-hydroperoxy-B-homo-6-oxa-cholestan-3$,7a-diol in this system. The ability of this compound and its cellular metabolites to cause cytotoxicity and to inhibit cholesterol synthesis in cultured human bronchial epithelial cells was subsequently studied.Both 5$,6$-epoxycholesterol and cholestan-6-oxo-3$,5"-diol were shown to be cytotoxic to cultured 16-HBE cells. A possible mechanism for cytotoxicity is the ability of these oxysterols to inhibit isoprenoid based cholesterol biosynthesis in these cells.

Ìîãó ëèøü ïîâòîðèòü òî, ÷òî óæå íåñêîëüêî ðàç Âàì îòâå÷àë. Êîñâåííàÿ èíäèôèêàöèÿ îçîíà ïî ïðîäóêòàì ðåàêöèè íå ÿâëÿåòñÿ îäíîçíà÷íûìè äîêàçàòåëüñòâàìè. Õîòÿ áû ïîòîìó, ÷òî îãðîìíîå êîëè÷åñòâî áèîõèìè÷åñêèõ ïðîöåññîâ, êîòîðûå ïðîòåêàþò â îðãàíèçìå, ìíîãîñòóïåí÷àòî, â ïðèñóòñòâèè öåëîãî ðÿäà ôåðìåíòîâ, â ïðîáèðêå âîñïðîèçâåñòè íåâîçìîæíî. È íåëüçÿ èñêëþ÷èòü, ÷òî ýòè ïðîäóêòû îáðàçîâàëèñü íå ïðè ðåàêöèè ñ îçîíîì, à ïðè îêèñëåíèè óæå äàâíî óñòàíîâëåííûìè â îðãàíèçìå äðóãèìè «ðåàêòèâíûìè ôîðìàìè êèñëîðîäà». Äàæå åñëè ïðåäïîëîæåíèÿ èññëåäîâàòåëåé èç TSRI ïîäòâåðäÿòñÿ, ïðè÷åì çäåñü îçîíîòåðàïèÿ? Òðóäíî ïðåäñòàâèòü, ÷òî ðàçîâûå âíóòðèâåííûå ââåäåíèÿ ðàñòâîðåííîãî îçîíà (èëè îçîíèäîâ), íå ïðåâûøàþùèå íåñêîëüêèõ ñîòûõ ìã îçîíà íà êã âåñà ïàöèåíòà (0,0014-0,02 ìã /êã), ìîãóò ÷òî-ëèáî äîáàâèòü ïîñòîÿííûì, â òå÷åíèå âñåé æèçíè, âûáðîñàì îçîíà èììóííûìè êëåòêàìè âî âñåì îáúåìå ñîñóäèñòîãî ðóñëà, åñëè òàêîé ñèíòåç äåéñòâèòåëüíî èìååò ìåñòî.
Òî÷íî òàêæå, â îòíîøåíèè ðàáîò, ãäå èçó÷àëîñü òîêñè÷åñêîå äåéñòâèå îçîíà â âäûõàåìîì âîçäóõå. Åñëè ïîñ÷èòàòü îáùåå êîëè÷åñòâî îçîíà â ýòèõ ðàáîòàõ, ïðîøåäøåãî ÷åðåç äûõàòåëüíûå ïóòè (÷åðåç êîíöåíòðàöèè è îáúåìû ãàçà çà îïðåäåëåííûé ïðîìåæóòîê âðåìåíè), òî ýòî êîëè÷åñòâî â äåñÿòêè ðàç ïðåâûøàåò òåðàïåâòè÷åñêèå äîçû èñïîëüçóåìûå ïðè îçîíîòåðàïèè.

ß óæå ðàç òðèäöàòü äîêëàäûâàë, ÷òî îçîí âî âäûõàåìîì âîçäóõå óæå ïðè äîâîëüíî íèçêèõ êîíöåíòðàöèÿõ – òîêñè÷åí äëÿ ëåãî÷íîãî ýïèòåëèÿ è îáëàäàåò ðàçäðàæàþùèì äåéñòâèåì íà âåðõíèå äûõàòåëüíûå ïóòè.
Ê ñîæàëåíèþ, â ñâîå âðåìÿ, èìåííî, ïóëüìîíîëîãàì ïðèõîäèëîñü â æåñòêîé ôîðìå âäàëáëèâàòü, ÷òî ââåäåíèå îçîíà äëÿ ëå÷åáíûõ öåëåé ÷åðåç äûõàòåëüíûå ïóòè, äàæå â íèçêèõ êîíöåíòðàöèÿõ èñïîëüçîâàòü ÍÅËÜÇß. Ìíå íåèçâåñòíî, ÷òî çíàåò îá îçîíîòåðàïèè àêàäåìèê ×ó÷àëèí è êàêîå ëå÷åáíîå âîçäåéñòâèå îí èìåë âèäó, íî, â íàñòîÿùèé ìîìåíò, òåì âðà÷àì, êîòîðûå çíàþò òåðàïèþ ÍÅ ÏÎÍÀÑËÛØÊÅ, â ãîëîâó íå ïðèäåò äàòü ïîäûøàòü îçîíîì áîëüíûì.
Íó, à â îòíîøåíèè ïîáî÷íûõ äåéñòâèé îçîíîòåðàïèè ïðè äðóãèõ ìåòîäàõ, òî åñëè áû èõ ÷èñëî ñîñòàâëÿëî õîòÿ áû íåáîëüøóþ ÷àñòü îò êîëè÷åñòâà ïîáî÷íûõ ýôôåêòîâ îò ñòàíäàðòíûõ è äîêàçàííûõ ëåêàðñòâ, òî îçîíîòåðàïèþ äàâíî áû çàêðûëè. À âåäü ñ å¸ ïîìîùüþ â ðàçíûõ ñòðàíàõ ïðîëå÷åí íè îäèí äåñÿòîê òûñÿ÷ áîëüíûõ.

Ñïàñèáî áîëüøîå, ÷òî Âû íàêîíåö ïðèçíàëè, ÷òî ïðèâîäèìûå ìíîé ññûëêè èìåþò õîòÿ áû îòäàëåííîå îòíîøåíèå ê îçîíîòåðàïèè. Ëè÷íî ìíå êàæåòñÿ, o÷åíü äàæå ïðÿìîå îòíîøåíèå, íî âñå ðàâíî ñïàñèáî. Âëàäèìèð ßêîâëåâè÷, î çëîâðåäíîì âëèÿíèè îçîíà íà "ëåãî÷íûå ýïèòåëèè" ìû äåéñòâèòåëüíî ñëûøàëè, è ñëûøàëè íå òîëüêî îò Âàñ, è íå òîëüêî ñëûøàëè, íî è ÷èòàëè, è íå òðèäöàòü ðàç, à ãîðàçäî áîëüøå. Òàêîå æå áîëüøåå êîëè÷åñòâî ðàç ìû ÷èòàëè è î òîêñè÷íîì âëèÿíèè îçîíà ïðàêòè÷åñêè íà âñå ñèñòåìû ÷åëîâå÷åñêîãî îðãàíèçìà, è íå ñîâñåì îçîíà, à ïðîäóêòîâ îçîíîëèçà. Äà, òåõ ñàìûõ ïðîäóêòîâ îçîíîëèçà, êîòîðûì îçîíîòåðàïåâòû ïðèïèñûâàþò ÷óäîòâîðíûé ýôôåêò, èçìåíèâ ïóòè ïîïàäàíèÿ â êðîâü. Âû çíàêîìû ñ êàñêàäàíîé òåîðèåé òîêñè÷íîñòè îçîíà? ß ñ íåé çíàêîì, èìåííî ïîýòîìó íå îäîáðÿþ "ïðîôåññèîíàëüíîå âïàðèâàíèå" îçîíîòåðàïèè (ïðîñòèòå çà ïëàãèàò, íî ìíå î÷åíü ïðèãëÿíóëîñü ýòî Âàøå ìåòêîå âûðàæåíèå). Âàøè "ñâåðõìàëûå äîçû îçîíà, ïðîðåàãèðîâàâøèå ñ êðîâüþ âíå îðãàíèçìà" â ðóêàõ õàðüêîâñêèõ îçîíîòåðàïåâòîâ âûçûâàëè ÌÎÐÔÎËÎÃÈ×ÅÑÊÈÅ èçìåíåíèÿ ïëàöåíòû ó áåðåìåííûõ. Îçîíîòåðàïåâòû îöåíèëè èõ êàê ïîçèòèâíûå...
Êñòàòè, Âëàäèìèð ßêîâëåâè÷, ðàññêàæèòå íà ôîðóìå, ïî÷åìó îçîíîòåðàïèÿ ïðîòèâîïîêàçàíà ïðè ãèïåðòèðåîçå? Ñ ÷åãî ýòî âäðóã? Ìåíÿ èíòåðåñóåò íå ëè÷íûé îïûò àêàäåìèêà Âûáåãàéëî, à ïàòîôèçèîëîãè÷åêèå ìåõàíèçìû (Âàø êîíåê), èëè íà õóäîé êîíåö ðåçóëüòàòû êëèíè÷åñêèõ èñïûòàíèé. Ñëîâà àêàäåìèêà ×ó÷àëèíà ìîãó ïîäêðåïèòü äåñÿòêàìè ññûëîê - ýòî àðãóìåíò èç "òîëñòîé êíèæêè". Âàøà "ÏÎÍÀÑËÛØÊÀ" - ýòî, êàê áû òàê ëó÷øå ñêàçàòü, íó êîðî÷å, íå çíàþ...:

Ðàññóæäàòü î ïàòîëîãè÷åñêîì âîçäåéñòâèè êàêèõ-ëèáî ñîåäèíåíèé, âåùåñòâ áåç êîíêðåòíûõ êîëè÷åñòâåííûõ äàííûõ, Âû óæ èçâèíèòå, óäåë äèëåòàíòîâ. Î êàêèõ êîíêðåòíî ïðîäóêòàõ îçîíîëèçà, îçîíîëèçà ×ÅÃÎ, è â êàêèõ-êîëè÷åñòâàõ Âû ÷èòàëè. Î êàêîé «êàñêàäíîé òåîðèåé òîêñè÷íîñòè îçîíà» Âû ãîâîðèòå? Î öåïíûõ ðåàêöèÿõ? Òàê îíè, âîîáùå, ñâîéñòâåííû áèîëîãè÷åñêîìó îêèñëåíèþ, òàê êàê âðåìÿ æèçíè â áèîëîãè÷åñêîé ñðåäå ðàäèêàëîâ, ê ïðèìåðó, ïåðåêèñíûõ, îòíîñèòåëüíî áîëüøîå.

Ìàé 1980.  Ðîññèè, â ýòî âðåìÿ, îçîíîòåðàïèÿ åù¸ íå ïðàêòèêîâàëàñü. Ãåðìàíñêîå ìåäèöèíñêîå Îáùåñòâî îçîíîòåðàïèè ñîáðàëî 1044 àíêåò îò âðà÷åé ïðàêòèêóþùèõ îçîíîòåðàïèþ. Ïîëó÷åííûå äàííûå ñèñòåìàòèçèðîâàíû è îïóáëèêîâàíû. Îçîíîì ïðîëå÷åíî 384 775 ïàöèåíòîâ, êîòîðûå ïîëó÷èëè ìèíèìóì 5579238 ïðîöåäóð. Êàêèå-ëèáî ïîáî÷íûå ýôôåêòû îòìå÷àëèñü ÂÎ ÌÍÎÃÎ ÐÀÇ ðåæå, ÷åì îò ïðèâû÷íûõ, äîêàçàííûõ ëåêàðñòâ. Òàê ìîæåò, Åâãåíèé Åâãåíüåâè÷, íå ñ îçîíîì íàäî áîðîòüñÿ?

Ïîôëåéìèòü, âèäèìî, ïðîèñõîäèò îò àíãëèéñêîãî "flame" - ïûë, ñòðàñòü; to fan a flame - ðàçæå÷ü ñòðàñòè.  àíãëîÿçû÷íîì èíòåðíåòíîì ìèðå ñëîâî ôëeéì èìååò îòðèöàòåëüíîå çíà÷åíèå è îçíà÷àåò àòàêó íà ÷ü¸-ëèáî ìíåíèå (ïî å-ìýéëó èëè â ÷àòå). Ïî ïîâîäó òåðìèíîëîãèè - î÷åíü èíòåðåñíîå çàìå÷àíèå. ß äåéñòâèòåëüíî ïðèâûê âîñïðèíèìàòü ñëîâà HYPERTHYROIDISM è THYROTOXICOSIS êàê ñèíîíèìû, õîòÿ èõ ðàçäåëåíèå â ðóññêîé òåðìèíîëîãèè ïðåäñòàâëÿåòñÿ ëîãè÷íûì.
Íà ðóññêèõ ñàéòàõ îçîíîòåðàïåâòîâ ñðåäè ïðîòèâîïîêàçàíèé (íåìíîãèõ) ê ïðèìåíåíèþ îçîíà óïîìèíàëñÿ èìåííî ãèïåðòèðåîç, õîòÿ, ó÷èòûâàÿ ñïåöèôèêó ðîññèéñêîé òåðìèíîëîãèè, âèäèìî, äîëæåí áûë áûòü òèðåîòîêñèêîç. Íàäåþñü, óâàæàåìûé Âëàäèìèð ßêîâëåâè÷ ïðîÿñíèò ñèòóàöèþ.

Íå ñîáèðàëñÿ ÿ ðàçìûøëÿòü çäåñü î öåïíûõ ðåàêöèÿõ èëè òàì «öèêëå Êðåáñà».
À ëèøü îòìåòèë, ÷òî òàêèå ðåàêöèè, ÷åðåç òå æå ñàìûå ðàäèêàëû, îòíþäü íå ïðåðîãàòèâà îçîíà, à è äðóãèõ ðåàêòèâíûõ ôîðì êèñëîðîäà, êîòîðûå íåîáõîäèìû ïðè æèçíåäåÿòåëüíîñòè îðãàíèçìà. À âîò êîãäà îíè íóæíû, à êîãäà òîêñè÷íû, àáñòðàêòíî îáñóæäàòü áåç êîíêðåòíûõ äàííûõ ïî èõ íàïðàâëåíèþ è êîëè÷åñòâó íå ïðåäñòàâëÿåòñÿ âîçìîæíûì.
Êðîìå òîãî, ïîëàãàþ, ÷òî êðîìå Âàñ «êàñêàäíàÿ òåîðèÿ òîêñè÷íîñòè îçîíà» íåèçâåñòíà íèêîìó. Ïîòîìó, ÷òî öåïü âîäîïàäîâ, ïðèíÿòî íàçûâàòü êàñêàäîì, à öåïíûå ðåàêöèè íåò.
«Ïàòîôèçèîëîãè÷åñêèå ìåõàíèçìû» îçîíîòåðàïèè ïðè èçáûòî÷íîì ñèíòåçå ãîðìîíîâ ÙÆ ìîæíî òîëüêî ïðåäïîëàãàòü, íî òàê êàê òàêèå ïðåäïîëîæåíèÿ, îòíþäü íå îáÿçàòåëüíî ñîîòâåòñòâóþò äåéñòâèòåëüíîñòè, ðàçðåøèòå èõ íå îçâó÷èâàòü.

The cascade mechanism to explain ozone toxicity: the role of lipid ozonation products.

Ozone is so reactive that it can be predicted to be entirely consumed as it passes through the first layer of tissue it contacts at the lung/air interface. This layer includes the lung lining fluid (tracheobronchial surface fluid and alveolar and small airway lining fluid) and, where the lung lining fluid is thin or absent, the membranes of the epithelial cells that line the airways. Therefore, the biochemical changes that follow the inhalation of ozone must be relayed into deeper tissue strat by a cascade of ozonation products. Lipid ozonation products (LOP) are suggested to be the most likely species to act as signal transduction molecules. This is because unsaturated fatty acids are present in the lipids in both the lung lining fluid and in pulmonary cell bilayers, and ozone reacts with unsaturated fatty acids to produce ozone-specific products. Further, lipid ozonation products are finite in number, have structures that are predictable from the Criegee ozonation mechanism, and are small, diffusible, stable (or metastable) molecules. Preliminary data show that individual LOP cause the activation of specific lipases, which trigger the release of endogenous mediators of inflammation.

Ðàç ïàëêà è äðåâêî îäíî è òîæå, òî, òåïåðü, ê ïðèìåðó, öåïíóþ ðåàêöèþ äåëåíèÿ òÿæåëûõ ÿäåð, áóäó íàçûâàòü êàñêàäíîé ðåàêöèåé - çâó÷èò êðàñèâåå.
