Ух,авно не была на форуме и сразу какая интересная тема!Оля,так что же с больной-чем все закончилось?Спрашиваю ради интереса .Буквально на днях имела такую интересную ситуацию,ау,господин американец,ситуация для вашего разгрома.
В июне женщину родоразрешили путем кесаревого сечения с ужасным исходом-ребенок умер на 2 сутки.Ко мне она поступила на 2 сутки после операции с очень высокой темтературой,получала аб и тк и была выписана на 5 сутки в удовлетворительном состоянии.Через 12 дней появилась на повторный осмотр и мной был поставлен диагноз субинвалюция матки и назначено соответствующее лечение.На 2 месяца женщина выпала из моего поля зрения и появилась,чтобы поставить спираль,предъявляя жалобы на постоянное вздутие живота и Все! На узи-RIF представлено образование 12-20 см неоднородной эхоструктуры мобильное б\болезненное.Назначили СT-skan-заключение абсцесс.Родственники настаивают на радикальной операции.Во время операции-огромный конгламерат,состоящий из правого яичника,трубы,вторично измененного аппендикса и все это было запаяно и окутано сальником.Что по вашему мне нужно было делать? |

Прооперированная мадам ушла здоровая, довольная, без единого осложнения домой. Короче, исход: выздоровление. ПиСи. Не далее как сегодня, видела ее, пациентку то есть, на центральном рынке. Очень резво рыбку покупала. |

Ув.доктор,с географией слегка у вас того,ну главно не в Америке и то хорошо.Извините,давно на Родине не была\я из Украины\,что как такового диагноза без эндометрита уже не существует?Вы в этом уверены?В 2000 году,работая в родильном стационаре,частенько выставлялся диагноз субинволюция матки даже без эндометрита.А вообще как приятно что ни форум,то и подколки.

Ув.господин Медведев,вы во всем безусловно правы,просто маленькая проблемка-называется этикой и деонтологией,ее оперировали мои советские-не знаю теперь из какого государства-коллеги,поэтому открыто сообщать,что было что-то не стала и по иннерции и здесь все это написала.А вообще все намного сложнее-во время патогистологического исследовыания обнаружена большая салфетка,которая за 2 месяца после операции окуталась сальником и к себе "подтянула'' и яичник и трубу и аппендикс.Постоперационно я вскрыла только гнойную часть образования,чтобы взять гной для посева!
Перед операцией были сомнения в пользу забытой салфетки,но к моему огромному облегчению и CT-skan дал заключение-абсцесс.Вы вправе обвинить меня в алогизме,но такова правда в этой истории. А Йемен находится на Aравийском полуострове,за Саудией.Да, нас уже визу можно получить прямо в аэропорту,приезжайте. |

Pregnancy is a predisposing factor for TTP, and as many as 10% of all TTP cases occur in parturients. Clinical manifestations include: thrombocytopenia central neurologic abnormalities (headache, seizures) hemolytic anemia fever renal impairment Effective treatment includes plasma exchange (which can lead to remission in 75% of patients), intravenous IgG, prednisone, and prostacyclin infusion. Delivery of the fetus does not necessarily result in clinical improvement. Preeclampsia-induced thrombocytopenia is not associated with fever. Idiopathic or autoimmune thrombocytopenic purpura (ITP) is the most common cause of thrombocytopenia during the first trimester of pregnancy and accounts for approximately 5% of all cases of pregnancy-associated thrombocytopenia. ITP is an autoimmune disorder characterized by the production of IgG antiplatelet antibodies. Transplacental transfer of these antibodies to the fetus does occur and can lead to fetal thrombocytopenia. About 10% to 20% of neonates of mothers with ITP have platelet counts below 50,000/mm3 at delivery. The major risk to the fetus is intracranial hemorrhage, though this is rare. It may be difficult to distinguish ITP from gestational thrombocytopenia. However, most patients with ITP have a history of easy bruisability, petechiae, epistaxis, and gingival bleeding that precedes pregnancy. In addition, ITP is more likely if the patient has a platelet count less than 50,000/mm3 or if the platelet count is progressively decreasing throughout pregnancy. Fever is not associated with this disorder. Treatment usually consists of oral prednisone with a goal of stabilizing the platelet count at 75-100,000/mm3. High doses of intravenous immunoglobulin may also be administered near the time of delivery. In patients refractory to steroid or immunoglobulin therapy, splenectomy may be required. The major risk for parturients with ITP is hemorrhage caused by a surgical incision, including episiotomy, or a perineal laceration. They are not at increased risk for postpartum hemorrhage caused by uterine atony. A major issue confronting the anesthesiologist caring for a parturient with thrombocytopenia is the safety of neuraxial anesthesia. Traditionally, a platelet count of 100,000/mm3 has been considered safe for performing neuraxial blockade. Controversy remains concerning the safety of performing neuraxial anesthesia in patients with platelet counts between 50,000 and 100,000/mm3. Retrospective studies have reported no complications in small numbers of patients (24-30) with platelet counts in that range who received epidural anesthesia. A survey of anesthesiologists in the United States found that the majority would perform epidural anesthesia in pregnant patients with platelet counts between 80,000 and 100,000/mm3. However, a prospective study with more than 200,000 patients would be required to definitively prove the safety of neuraxial anesthesia in patients with platelet counts below 100,000/mm3. Therefore, the risks and benefits of epidural anesthesia should be considered for the individual patient when making a decision about proceeding with neuraxial anesthesia. Может быть сочетание инфекции и HELLP, иногда может быть лихорадка при HELLP. |

По прочтении темы возник следущий вопрос. "О перспективе".
Уже упоминалось, что сохранённая труба имеет неплохие шансы стать нефункциональной. А значит повышается риск трубной беременности. Плюс, хотелось бы узнать, какова вероятность перехода ТОА в хронический процесс. |
