Каждую неделю приходится видеть психиатрических б-х, и клинико-радиологические конференции с психиатрами - норма в любой больнице в связи с: Деменция: PET scans help spot and classify dementia types Потеря памяти:...functional magnetic resonance imaging (fMRI) scanner to study the brain activity of people with amnestic mild cognitive impairment (MCI), a precursor to AD, and found altered functionality in both the frontal and temporal lobes of the brain. Biological Psychiatry is publishing a new study of regional brain activity using functional magnetic resonance imaging, which may help further our understanding of how happiness and suffering are related in depression.

Role of Functional Brain Imaging in the Neurobiology of Depression Basic Principles of Functional Brain Imaging Several modalities of functional brain imaging are available (Table 1). They measure neuronal activity as reflected by cerebral blood flow, oxygen uptake, and glucose utilization.[116] In addition, functional imaging can measure the localization and density of a diverse array of neurotransmitter receptors and transporters.[116] Table 1. Types of Functional Brain Imaging Modality Functions Remarks PET -Measures rCBF and regional metabolic rate -Maps receptor/transporter density, distribution, and occupancy -Better image resolution than SPECT -Expensive SPECT -Measures rCBF -Maps receptor/transporter density, distribution, and occupancy fMRI -Measures rCBF -Relatively fast procedure EEG/ERP -Maps bioelectric activity MEG -Maps biomagnetic activity MRS -Measures quantitative neurochemistry rCBF = regional cerebral blood flow; PET = positron emission tomography; SPECT = single photon emission computed tomography; fMRI = functional magnetic resonance imaging; EEG = electroencephalogram; MEG = magnetoencephalography; MRS = magnetic resonance spectroscopy. These imaging techniques can localize brain regions of altered synaptic activity associated with specific psychiatric disorders and follow these changes after treatment. Also, functional imaging can help to determine the role of specific neurotransmitter receptors and transporters in the pathophysiology and treatment of psychiatric disorders.[117] Functional imaging is helpful in mapping not only the neuroanatomy of the mentally ill brain, but also the pathways of normal brain activities/functions (eg, emotions [amygdala], memory [hippocampus]).[61] Functional imaging is at the center of the burgeoning field of cognitive neuroscience. Neuroanatomy of Depression Researchers have long observed the serious negative effect of mood changes (eg, transient sadness or pervasive depression) on cognitive function (eg, attention span and decisiveness). Thus, they have postulated the importance of the limbic-neocortical connections in emotional behaviors.[118] A recent study[118] demonstrated that both normal sadness and pathological depression produce limbic activation (subgenual cingulate, anterior insula) and cortical deactivation (right prefrontal cortex, inferior parietal) on positron emission tomography (PET) scans. More importantly, following treatment, depressed patients exhibited reciprocal changes in the involved brain areas -- increased activity in the cortex and decreased activity in the limbic region. The authors postulated that treatments that target either affected areas (cognitive-behavioral therapy [cortex], antidepressants [limbic/cortical areas]) should be equally effective given the functional limbic-cortical reciprocity. Other consistent neuroanatomical findings include: Decreased neuronal activity in the left lateral prefrontal cortex,[119-122] which is significantly correlated with the severity of depressive symptoms.[61] Decreased neuronal activity in the basal ganglia, especially the caudate and putamen. [119,123] This finding also correlates with the morphological abnormalities of basal ganglia found on MRI, such as reduced size and increased prevalence of hyperintensities within these structures.[124] Reduction in global cerebral metabolism in the depressed elderly |

Neuro-Psychoanalytic Perspectives on Depression - 19-22 July Depression is conceptualised differently by psychoanalysts and psychopharmacologists. For psychoanalysts it concerns the emotional and moral attitude of the self to its own value, resulting from a lifetime of experiences. For psychopharmacologists it concerns a depletion or imbalance of specific neurotransmitter systems, resulting from epigenetic vulnerabilities. In recent years, neuroscientists have begun to explore the gap between these two apparently contradictory approaches, and an integrated understanding of depression is finally coming into view. This conference will take stock of the current state of these integrative efforts, with leading scientists and clinicians setting out what they have learnt from various disciplinary perspectives. Delegates will discover that our different ways of understanding depression are not so incompatible after all. |
