Íåñêîëüêî ëåò íàçàä ÷èòàë ïî÷òè äåòåêòèâíóþ èñòîðèþ èç Ôðàíöèè, êàê â ëàáîðàòîðèþ áîëüíèöû ïîñòóïàëà öèòðàòíàÿ öåëüíàÿ êðîâü íà ãåìîñòàç â ñòàíäàðòíûõ ïðîáèðêàõ ñ ãîëóáîé êðûøêîé, íî êîíòàìèíèðîâàííûå ÝÄÒÀ - êîñâåííî äîêàçàëè ïî óðîâíþ èîíèçèð. êàëüöèÿ: â öèòðàòíîé ïðîáå îí áûë çíà÷èòåëüíî âûøå, à ïðè äîáàâëåíèè äàæå íåáîëüøîãî êîëè÷åñòâà ÝÄÒÀ êðîâè ñòàíîâèëñÿ î÷åíü íèçêèé èëè íåèçìåðÿåìûé. Ïîñëå îòáðàñûâàíèÿ ãèïîòåçû î ñëó÷àéíîé çàâîäñêîé êîíòàìèíàöèè ñåðèè ïðîáèðîê (ïîñòóïàëè òàêèå ïðîáû õàîòè÷íî è íåðåãóëÿðíî) ïðåäïîëîæèëè, ÷òî ñêîðåå âñåãî ïðè êðîâåçàáîðå "îïûòíàÿ ãðàìîòíàÿ" ìåäñåñòðà, óâèäåâ íåäîáîð öèòðàòíîé êðîâè â ïðîáèðêå èç-çà ðàííåãî îòêëþ÷åíèÿ îò ñèñòåìû, îòêðûâàëà ÝÄÒÀ ïðîáèðêó è èç íåå êðîâüþ äîâîäèëà äî íåîáõîäèìîãî óðîâíÿ â öèòðàòíîì îáðàçöå (âî ìíîãèõ ëàáîðàòîðèÿõ â ñëó÷àå "íåäîëèâà" öèòðàòíîé êðîâè íå ïðèíèìàþò îáðàçåö íà àíàëèç è âîçâðàùàþò â îòäåëåíèå äëÿ ïåðåçàáîðà).

Ïàöèåíòû - ðîäíûå áðàò è ñåñòðà (60-66 ãîäà ðîæäåíèÿ), ïðî äèàáåò äàííûõ íåò, òîùàêîâàÿ ãëþêîçà â íîðìå, ðåçóëüòàò HbA1c - íîëü. Ó îáîèõ. Õðîìàòîãðàììó âûëîæó òîëüêî îäíó (îíè ïî÷òè èäåíòè÷íû).

Äà, òóò ãåìîãëîáèíîïàòèÿ è ñâÿçàííûå ñ íåé ãåìîëèòè÷åñêèå ïðîáëåìû.
À òàêæå ïðîáëåìà íåâîçìîæíîñòè ìîíèòîðèòü ÑÄ (åñëè îí åñòü). Íà õðîìàòîãðàììå îòìå÷àåòñÿ S-window 61.2%. Ïî äàííûì ïðîèçâîäèòåëÿ ýòà ôðàêöèÿ ñîîòâåòñòâóåò S-âàðèàíòó ãåìîãëîáèíà. Õîòÿ, êîíå÷íî, âîçìîæíà è ñìåøàííàÿ ãåìîãëîáèíîïàòèÿ. Íà ïèêå, êîòîðûé ïî âðåìåíè âûõîäà äîëæåí ñîîòâåòñâîâàòü ëàáèëüíûì îñíîâàíèÿì (Øèôôîâûì) òîæå âìåøàëîñü íå÷òî â êîëè÷åñòâå 27.7% îò îáùåé ïëîùàäè. Îáû÷íî â ýòîì ìåñòå (èëè î÷åíü ðÿäîì) âûõîäèò ôåòàëüíûé ãåìîãëîáèí. ÎÀÊ äåëàëè íå ìû, ìîðôîëîãèÿ ýðèòðîöèòîâ íå îïèñàíà, à ìîãëà áû áûòü, ò.ê. ñóäÿ ïî âñåìó çäåñü äàëåêî íå íîñèòåëüñòâî. Âîçìîæíî ÷òî è äåôèöèò B12. À ìîæåò ó æåíùèíû ïðåîáëàäàåò ñåðïîâèäíî-êëåòî÷íàÿ ôîðìà, à ó ìóæ÷èíû òàëàññåìèÿ...äîãàäêè Ñîáñòâåííî, äàííûé ìåòîä ïðåäíàçíà÷åí èñêëþ÷èòåëüíî äëÿ îïðåäåëåíèÿ HBA1c, íî åãî íåò. Õðîìàòîãðàììà òîëüêî ïîìîãàåò ñîðèåíòèðîâàòüñÿ, ïîíÿòü, ÷òî ñ àíàëèçàòîðîì âñå îê, à ñ ïàöèåíòîì íå âñå. Ïîïàëàñü íà ãëàçà ëþáîïûòíàÿ áðîøþðêà äëÿ ïàöèåíòîâ ñ ÑÄ: ïðåäóïðåäèòå âðà÷à, ÷òî ó âàñ ìîæåò áûòü ãåìîãëîáèíîïàòèÿ è ðåçóëüòàò íà HbA1c ìîæåò áûòü íå äîñòîâåðåí! (âêëàäûâàþ) |
