Öåëü íàñòîÿùåãî èññëåäîâàíèÿ – ñðàâíèòü ýôôåêò îò ìåñòíîãî ïðèìåíåíèÿ 5% ãåëÿ âèòàìèíà Å, ïëàöåáî-ãåëÿ è õëîðãåêñèäèíà ïðè îáðàçîâàíèè áëÿøåê è ðàçâèòèè ïàðîäîíòèòà ó 48 âçðîñëûõ ïàöèåíòîâ, ðàçäåë¸ííûõ íà 3 ãðóïïû.

Ðåçóëüòàò íå ïîêàçûâàåò çíà÷èòåëüíîãî ýôôåêòà íà ðàçâèòèå áëÿøåê è ãèíãèâèòà êàê â ïëàöåáî-, òàê è âèò. Å - ãðóïïàõ. Îäíàêî èñïîëüçîâàíèå 0,12% ðàñòâîðà õëîðãåêñèäèíà çíà÷èòåëüíî âëèÿåò íà óìåíüøåíèå îáðàçîâàíèÿ áëÿøåê. Ýòè ðåçóëüòàòû ãîâîðÿò ïðîòèâ ïðèìåíåíèÿ âèò. Å êàê ñðåäñòâà ìåñòíîé òåðàïèè äëÿ ëå÷åíèÿ ãèíãèâèòà è ïàðîäîíòèòà.

P.S. ß íå ïîìíþ, ÷òîáû âèò. Å ïðèìåíÿëñÿ ìåñòíî ïðè ëå÷åíèè ïàðîäîíòîïàòèé, ìîæåò áûòü ñ 90-õ ãîäîâ ñòàëè èññëåäîâàòü ýòîò âîïðîñ è ïðîáîâàòü àëüòåðíàòèâíûå îáùåïðèíÿòûì ñðåäñòâà. Ãîðàçäî áîëåå âàæíûì, íà ìîé âçãëÿä, ÿâëÿåòñÿ ïîäòâåðæäåíèå ýôôåêòèâíîñòè õëîðãåêñèäèíà.

Àâòîð äîëæåí çàðàíåå ó÷èòûâàòü î÷åíü âûñîêèå òðåáîâàíèÿ Êîêðàíîâñêîãî Ñîòðóäíè÷åñòâà, äðóãèõ îðãàíèçàöèé, çàíèìàþùèõñÿ ñèñòåìàòè÷åñêèìè îáçîðàìè, è çàêëàäûâàòü â ïóáëèêàöèè ñâîèõ èññëåäîâàíèé àëãîðèòìû, ïðèåìëåìûå äëÿ îïèñàííîé îáðàáîòêè, ïî èíîìó îðãàíèçîâûâàòü ñàìè èññëåäîâàíèÿ, ÷òîáû ñòàòüè ïîòîì íå êàçàëèñü ïðîñòî ðåêëàìîé.

ÂÑÒÓÏËÅÍÈÅ: Ïîñëå ââåäåíèÿ ìèêðîõèðóðãè÷åñêèõ ïðèíöèïîâ â ýíäîäîíòèþ, âêëþ÷àÿ íîâûå òåõíîëîãèè ëå÷åíèÿ êîðíåâîãî êàíàëà, áûëî ïðîäîëæåíî èññëåäîâàíèå âîçìîæíîñòè óëó÷øåíèÿ âèçóàëüíîãî âîñïðèÿòèÿ âðà÷îì õèðóðãè÷åñêîãî ïîëÿ. Èíòåðåñíî áûëî áû óçíàòü êàêèå òåõíè÷åñêèå ïðåèìóùåñòâà ïîëó÷àåò îïåðàòîð îò îïòè÷åñêèõ óâåëè÷èòåëüíûõ óñòðîéñòâ, òàêèõ êàê õèðóðãè÷åñêèé ìèêðîñêîï, ýíäîñêîï è óâåëè÷èòåëüíàÿ ëóïà, à òàêæå âûãîäû äëÿ ïàöèåíòà, â ñìûñëå óëó÷øåíèÿ êëèíè÷åñêèõ è ðàäèîãðàôè÷åñêèõ ðåçóëüòàòîâ.

ÖÅËÜ ÈÑÑËÅÄÎÂÀÍÈß: Öåëü ýòèõ ñèñòåìàòè÷åñêèõ îáçîðîâ, â ñîîòâåòñòâèè ñ ïðèíöèïàìè äîêàçàòåëüíîé ìåäèöèíû, – óëó÷øèòü è ñðàâíèòü ýôôåêòû ýíäîäîíòè÷åñêîãî ëå÷åíèÿ ïðè ïîìîùè óâåëè÷èòåëüíûõ óñòðîéñòâ è ëå÷åíèÿ áåç òàêîâûõ óñòðîéñòâ. ÑÒÐÀÒÅÃÈß ÈÑÑËÅÄÎÂÀÍÈß: Áàçû äàííûõ of The Cochrane Oral Health Group Trials Register, CENTRAL, MEDLINE and EMBASE áûëè èññëåäîâàíû ñ ïîìîùüþ ïðèåìëåìûõ ñòðàòåãèé èññëåäîâàíèÿ. Ðó÷íîé ïîèñê âêëþ÷àë äåâÿòü ñòîìàòîëîãè÷åñêèõ æóðíàëîâ. Áèáëèîãðàôèè âàæíåéøèõ êëèíè÷åñêèõ èññëåäîâàíèé è âàæíåéøèõ ñòàòåé áûëè èäåíòèôèöèðîâàíû îòäåëüíî îò âðó÷íóþ èññëåäîâàííûõ æóðíàëîâ. Ñåìü ïðîèçâîäèòåëåé èíñòðóìåíòîâ â îáëàñòè ýíäîäîíòèè è/èëè ýíäîäîíòè÷åñêîé õèðóðãèè, òàê æå êàê è àâòîðû èäåíòèôèöèðîâàííûõ ðàíäîìèçèðîâàííûõ êîíòðîëèðóåìûõ èññëåäîâàíèé (RCTs) êîíòàêòèðîâàëè â ïîðÿäêå èäåíòèôèêàöèè íåîïóáëèêîâàííûõ èëè ïðîäîëæàþùèõñÿ RCTs. ßçûêîâûõ îãðàíè÷åíèé íå áûëî. Ïîñëåäíèé ýëåêòðîííûé ïîèñê áûë ïðîâåä¸í 02.04.2009, è ïîñëåäíèé ðó÷íîé ïîèñê áûë ïðåäïðèíÿò 31.01.2009.

ÊÐÈÒÅÐÈÈ ÎÒÁÎÐÀ ÄÀÍÍÛÕ: Ðàíäîìèçèðîâàííûå è êâàçè-ðàíäîìèçèðîâàííûå èññëåäîâàíèÿ ñðàâíèâàëèñü ïðè ýíäîäîíòè÷åñêîé òåðàïèè, ïðîâåä¸ííîé ñ ïîìîùüþ èëè áåç òàêîâîé, îäíîãî èëè áîëåå òèïîâ ïðèáîðîâ, ñëóæàùèõ äîïîëíåíèåì ýíäîäîíòè÷åñêîé òåðàïèè.

ÑÁÎÐ ÄÀÍÍÛÕ È ÀÍÀËÈÇ: Ñêðèíèíã èññëåäîâàíèé è âûáîðêà äàííûõ áûëè ïðîâåäåíû íåçàâèñèìî è â äâóõ ýêçåìïëÿðàõ. Çàòåì äëÿ îáúåäèíåíèÿ äàííûõ ñëåäîâàëè ñòàòèñòè÷åñêèå ãàéäëàéíû Êîêðàíîâñêîãî Ñîòðóäíè÷åñòâà.

ÃËÀÂÍÛÅ ÐÅÇÓËÜÒÀÒÛ: Íè îäíî èññëåäîâàíèå íå áûëî âêëþ÷åíî â íàñòîÿùèé îáçîð. Âñå îæèäàåìûå èññëåäîâàíèÿ, êîòîðûå áûëè èäåíòèôèöèðîâàíû, âñå ðàáîòû ïî ýíäîäîíòè÷åñêîé õèðóðãèè ïðèøëîñü èñêëþ÷èòü ïî ðàçëè÷íûì ïðè÷èíàì. Òîëüêî îäíî RCT áûëî èäåíòèôèöèðîâàíî êàê ñîäåðæàùåå ñðàâíåíèå òð¸õ óâåëè÷èòåëüíûõ óñòðîéñòâ (ëóïà, ìèêðîñêîï, ýíäîñêîï). Íå áûëî íàéäåíî íè îäíîãî èññëåäîâàíèÿ (RCT), êîòîðîå ñðàâíèâàåò ðåçóëüòàò ýíäîäîíòè÷åñêîé òåðàïèè, ñ èñïîëüçîâàíèåì èëè áåç òàêîâîãî, äàííûõ óâåëè÷èòåëüíûõ óñòðîéñòâ.

ÀÂÒÎÐÑÊÎÅ ÇÀÊËÞ×ÅÍÈÅ: Íèêàêîãî îáúåêòèâíîãî çàêëþ÷åíèÿ íåâîçìîæíî äàòü ïî ðåçóëüòàòàì ýòîãî îáçîðà, ïîñêîëüêó íå áûëà íàéäåíà ñòàòüÿ â â ïîñëåäíèõ èçäàíèÿõ, êîòîðàÿ áû óäîâëåòâîðÿëà êðèòåðèÿì äëÿ å¸ âêëþ÷åíèÿ. Íåèçâåñòíî êàê òèï óñòðîéñòâà âëèÿåò íà ðåçóëüòàò ëå÷åíèÿ, ó÷èòûâàÿ áîëüøîå êîëè÷åñòâî ôàêòîðîâ êîòîðûå ìîãóò èìåòü çíà÷èòåëüíîå âëèÿíèå íà óñïåõ õèðóðãè÷åñêèõ ïðîöåäóð. Íåîáõîäèìû äàëüíåéøèå äîëãîâðåìåííûå èññëåäîâàíèÿ ñ áÎëüøèì îáú¸ìîì èññëåäóåìîãî ìàòåðèàëà. Òåõíè÷åñêèå ïðåèìóùåñòâà óñòðîéñòâ øèðîêî ïðåäñòàâëåíû â ïå÷àòè, íî îñíîâûâàþòñÿ íà äîêàçàòåëüñòâàõ íèçêîãî óðîâíÿ èññëåäîâàíèé, îíè äîëæíû áûëè áûòü àäðåñîâàíû ê êîíêðåòíûì êëèíè÷åñêèì ïîêàçàíèÿì äëÿ èñïîëüçîâàíèÿ â ñïåöèôè÷åñêèõ êëèíè÷åñêèõ ñèòóàöèÿõ, íàïðèìåð, äëÿ ëå÷åíèÿ ìîëÿðà èëè ðàâíîçíà÷íî êî âñåì çóáàì. Íóæíî áûëî áû ïðîâîäèòü èññëåäîâàíèÿ îñíîâàííûå íà ëó÷øåì äèçàéíå, ÷òîáû îïðåäåëèòü èñòèííóþ ðàçíèöó ïî îòíîøåíèþ ê óðîâíþ óñïåøíîñòè ëå÷åíèÿ, èñïîëüçóÿ èëè íå èñïîëüçóÿ ýòè óñòðîéñòâà êàê ïðè îáû÷íîì, òàê è õèðóðãè÷åñêîì ýíäîäîíòè÷åñêîì ëå÷åíèè.

 ïîñëåäíèå ãîäû òåõíè÷åñêàÿ ðåâîëþöèÿ èçìåíèëà òàêæå è ëèöî ñòîìàòîëîãèè. Íî ââåäåíèå îïåðàöèîííîãî ìèêðîñêîïà â ñòîìàòîëîãè÷åñêóþ ïðàêòèêó íå ïðîèçîøëî (2004). Àâòîð ñòàòüè æåëàåò ïîêàçàòü - ÷òî ìîæåò ñäåëàòü òàêîé áëåñòÿùèé èíñòðóìåíò êàê õèðóðãè÷åñêèé ìèêðîñêîï, åñëè åãî ïîâåðíóòü äëÿ íóæä ñòîìàòîëîãè÷åñêîé ïðàêòèêè. Ìèêðîñêîï ïðåäëàãàåò ãîìîãåííîå îñâåùåíèå îïåðàöèîííîãî ïîëÿ, áåç òåíåé, íî, òåì íå ìåíåå, ñ âåëèêîëåïíîé êîíòðàñòíîñòüþ è òð¸õìåðíûì ýôôåêòîì. Îí îáåñïå÷èâàåò ÿñíîå âèçóàëüíîå âîñïðèÿòèå âî âðåìÿ ëå÷åíèÿ (÷òî âûãîäíî îòëè÷àåò åãî îò ýíäîñêîïà). Áëàãîäàðÿ îïòèìèçàöèè âèçóàëüíîãî âîñïðèÿòèÿ, áëàãîäàðÿ èíòåíñèâíîìó îñâåùåíèþ è óâåëè÷åíèþ, òåõíè÷åñêîé ïðåöèçèîííîñòè, òî÷íîñòü ìàíèïóëÿöèé âî âðåìÿ ïðîöåäóð çíà÷èòåëüíî óëó÷øàåòñÿ.

Öåëü íàñòîÿùåãî èññëåäîâàíèÿ – âîçìîæíîñòè ïðèìåíåíèÿ ìèêðîõèðóðãè÷åñêîé òåõíèêè ïðè ëå÷åíèè îêîëîêîðíåâûõ òêàíåé, êîíêðåòíî – ïåðôîðàöèÿ áîêîâîé ñòåíêè êàíàëà. Îïèñàíèå ñëó÷àÿ îòíîñèòñÿ ê ïàöèåíòêå 52 ëåò, â öåëîì îáëàäàþùåé õîðîøèì çäîðîâüåì (ÀSA 1) ñ îñòðûìè áîëÿìè â îáëàñòè âåðõíåãî ëåâîãî êëûêà (2.3). Ðàäèîãðàôè÷åñêîå èññëåäîâàíèå ïîêàçàëî íàëè÷èå ðåíòãåíîïðîçðà÷íîãî ïîâðåæäåíèÿ íà ñðåäíåé òðåòè êîðíÿ 2.3 è íà äèñòàëüíîé ïîâåðõíîñòè 2.2, îáà ëå÷åííûå ýíäîäîíòè÷åñêè. Èñïîëüçóÿ ýíäîñêîï êàê óâåëè÷èòåëüíûé ïðèáîð, áûëî ïðîèçâåäåíî îáñëåäîâàíèå ñðåäíèõ òðåòåé êîðíåé 2.2 è 2.3. Ñîáùåíèå ìåæäó ïåðèîäîíòàëüíîé ùåëüþ è ñèñòåìîé êîðíåâîãî êàíàëà áûëî îïðåäåëåíî ýíäîäîíòè÷åñêèì ôàéëîì íà ñòåíêå êàíàëà ñðåäíåé òðåòè êîðíÿ. Ïîëîñòü â ýòîì ìåñòå áûëà ïîäãîòîâëåíà ñ ïîìîùüþ ðåòðî-òèïîâ. Öåìåíò ÅÂÀ áûë èñïîëüçîâàí êàê ïëîìáèðîâî÷íûé ìàòåðèàë. ×åðåç 36 ìåñÿöåâ ïîñëå ëå÷åíèÿ, êëèíè÷åñêè è ðàäèîãðàôè÷åñêè ðåçóëüòàòû ëå÷åíèÿ áûëè ïðèçíàíû êàê óñïåøíûå. Ýòîò ñëó÷àé ïîêàçûâàåò ïðèãîäíîñòü èñïîëüçîâàíèÿ ýíäîñêîïà è ìèêðîõèðóðãè÷åñêèõ èíñòðóìåíòîâ äëÿ äèàãíîñòèêè è õèðóðãè÷åñêîãî ëå÷åíèÿ â îáëàñòè ìèêðîàíàòîìè÷åñêèõ êîðíåâûõ ñòðóêòóð.

Ìàãíèòî-ðåçîíàíñíîå èññëåäîâàíèå ïîêàçàëî ñìåùåíèå ïðàâîãî TMJ äèñêà, è íåêðîç ãîëîâêè ñóñòàâíîãî îòðîñòêà.  êîíå÷íîì ñ÷¸òå ïðîèçîøëè äåôîðìàöèÿ ÿìêè, êîñòíàÿ àïïîçèöèÿ, ýðîçèÿ ìûùåëêà, è ïîëíàÿ äåñòðóêöèÿ äèñêà. Çàêëþ÷åíèå: âíóòðèñóñòàâíûå ãëþêîêîðòèêîèäíûå èíúåêöèè, åñëè îíè ñäåëàíû íåïðàâèëüíî, ìîãóò ïðèâåñòè ê ðàçðóøåíèþ ñóñòàâà.

ÌÅÒÎÄ: Ñòâîëîâûå êëåòêè èç âûïàâøåãî ìîëî÷íîãî ÷åëîâå÷åñêîãî çóáà áûëè ïîìåùåíû â ñèíòåòè÷åñêèé îòêðûòûé êîíòåéíåð èç D,D-L,L – ïîëèìîëî÷íîé êèñëîòû ñ äîáàâëåíèåì èëè áåç òàêîâîãî êîñòíîãî ìîðôîãåíè÷åñêîãî ïðîòåèíà-2, è òðàíñôîðìèðóþùèé ôàêòîð ðîñòà áåòà1, ÷òîáû ñîçäàòü êîíñòðóêöèþ èç ïóëüïîâîé òêàíè. ÐÅÇÓËÜÒÀÒÛ: Ïðè óëüòðàñòðóêòóðíîì èññëåäîâàíèè ïðè ïîìîùè SEM ïîëó÷åíû ìèêðîôîòîãðàôèè ñ óâåëè÷åíèåì â 2000 ðàç, êîòîðûå âûÿâèëè òî÷íîå ïîëîæåíèå âñåõ ïóëüïîâûõ êîíñòðóêöèé, ñ íåáîëüøîé ðàçíèöåé â ñëó÷àÿõ ñ èñïîëüçîâàíèåì ïîëèìîëî÷íîé êèñëîòû è ïðè äîáàâëåíèÿõ ôàêòîðà ðîñòà.

ÇÀÊËÞ×ÅÍÈÅ: Ýòè ðåçóëüòàòû ïîääåðæèâàþò êîíöåïöèþ âîçìîæíîñòè èìïëàíòàöèè èñêóññòâåííî âûðàùåííûõ ïóëüïîâûõ êîíñòðóêöèé (ïðåïàðàòîâ) â çóá ïîñëå ëå÷åíèÿ êàíàëà.

ÊËÈÍÈ×ÅÑÊÎÅ ÏÐÈÌÅÍÅÍÈÅ:  áóäóùåì âîçìîæíî ðåãåíåðàòèâíîå ýíäîäîíòè÷åñêîå ëå÷åíèå êîòîðîå âîçìîæíî ïîñëå ëå÷åíèÿ êîðíåâîãî êàíàëà, ñ ïîñëåäóþùåé èìïëàíòàöèåé òêàíåâîãî ïðåïàðàòà ïóëüïû, âûðàùåííîãî èç ñòâîëîâûõ êëåòîê è ñîçäàííîãî â ëàáîðàòîðèè.

Abstract

The success of endodontic therapy requires a knowledge of the internal and external dental anatomy and its variations in presentation. The internal anatomy of the maxillary central incisor is well known and usually presents with one root and one radicular canal system. This case report describes an endodontic treatment of a maxillary central incisor with two roots and two canal systems, demonstrated by radiography and computerized tomography examinations.

Óñïåõ ýíäîäîíòè÷åñêîãî ëå÷åíèÿ òðåáóåò çíàíèÿ âíåøíåé è âíóòðåííåé àíàòîìèè è èõ âàðèàöèé. Âíóòðåííÿÿ àíàòîìèÿ âåðõíåãî öåíòðàëüíîãî ðåçöà õîðîøî èçâåñòíà è îáû÷íî ïðåäñòàâëåíà îäíèì êîðíåì è îäíîé ñèñòåìîé êîðíåâîãî êàíàëà. Ýòîò ñëó÷àé äåìîíñòðèðóåò ëå÷åíèå âåðõíåãî öåíòðàëüíîãî ðåçöà ñ äâóìÿ êîðíÿìè è äâóìÿ ñèñòåìàìè êîðíåâîãî êàíàëà; ïðèìåíÿëîñü èññëåäîâàíèå ñ ïîìîùüþ ðàäèîãðàôà è êîìïüþòåðíîãî òîìîãðàôà.

Abstract

Maxillary central incisors have been reported with 1, 2, and occasionally 3 root canals. The complete biomechanical instrumentation and obturation of the root canal system are mandatory to achieve endodontic success. Root canal systems with abnormal variations present a challenge in diagnosis and clinical management to the practitioner. This article presents a detailed case report of the endodontic treatment of a 3-canal maxillary incisor with an associated periodontal defect.

Effect of photo-activated disinfection on endodontic pathogens ex vivo.

AIM: To test the hypothesis that photo-activated disinfection (PAD) has a bactericidal effect on pathogens inoculated in root canals, with emphasis on biofilm formation/destruction. METHODOLOGY: Root canals of extracted teeth (n = 38) were prepared (size 30, 0.10 taper), autoclaved, divided into three groups and two negative controls inoculated (Streptococcus anginosus, Enterococcus faecalis or Fusobacterium nucleatum) and treated (PAD, laser, dye or positive control) according to a cross-sectional design. Resultant colony-forming unit counts were associated with observations of cell structural changes using environmental scanning electron microscopy (ESEM) on inoculated dentinal surfaces (n = 22, two controls) before (1, 2 and 6 days of incubation) and after treatment with PAD. RESULTS: Treatment of root canals with PAD (15 J) caused a significant reduction of the bacterial load, resulting in a 93.8% kill of S. anginosus (P < 0.0001), a 88.4% kill of E. faecalis (P < 0.05) and a 98.5% kill of F. nucleatum (P < 0.0001), but no sterilization. Laser alone had no significant effect on the load nor did the dye without laser. The ESEM experiment showed that individual cells or monolayers were easily eliminated with PAD. But when biofilms were present (2 and 6 days for E. faecalis, 6 days for S. anginosus), bacterial eradication was substantially reduced in deep layers. CONCLUSIONS: Photo-activated disinfection is not an alternative but a possible supplement to the existing protocols for root canal disinfection as the interaction between light (diode laser) and associated dye (TBO) provides a broad-spectrum effect. Some endodontic pathogens that grow as single-species biofilms, however, are difficult to eradicate.

Ëå÷åíèå ex vivo êîðíåâûõ êàíàëîâ ñ ïðèìåíåíèåì ôîòî-àêòèâèðîâàííîé äåçèíôåêöèè (PAD) ïîêàçàëî çíà÷èòåëüíîå óìåíüøåíèå ìèêðîáíîé ñîñòàâëÿþùåé, (ïîãèáëî: 93.8% of S. ànginosus, 88.4% of E. faecalis, 98.5% of F. nucleatum), íî íå ñòåðèëèçàöèþ. Çàêëþ÷åíèå: ôîòî-àêòèâèðîâàííàÿ äåçèíôåêöèÿ íå ÿâëÿåòñÿ àëüòåðíàòèâîé, à âîçìîæíûì äîïîëíåíèåì ê ïðîòîêîëàì äåçèíôåêöèè êîðíåâûõ êàíàëîâ. Íåêîòîðûå ýíäîäîíòè÷åñêèå ìèêðîîðãàíèçìû, êîòîðûå ðàñòóò êàê ìîíîêóëüòóðíûå áèîïë¸íêè, î÷åíü ñëîæíî óäàëèòü ðàäèêàëüíî.

ABSTRACT

Öåëü íàñòîÿùåé ñòàòüè – îïèñàòü óñïåøíîå óäàëåíèå èíîðîäíîãî òåëà, ðàñïîëîæåííîãî â àïèêàëüíîé ÷àñòè íåñôîðìèðîâàâøåãîñÿ êîðíåâîãî êàíàëà ïðè ïîìîùè ïðîñòûõ èíñòðóìåíòîâ, âûòÿãèâàíèåì âäîëü îñè çóáà, ÷òî ïîçâîëèëî íå ïðèáåãàòü ê îïåðàòèâíîìó âìåøàòåëüñòâó â îáëàñòè âåðõóøêè êîðíÿ èëè óäàëåíèþ çóáà ñ ïîñëåäóþùåé ðåèìïëàíòàöèåé.

Ðåíòãåíîêîíòðàñòíîå èíîðîäíîå òåëî, ðàñïîëîæåííîå â àïèêàëüíîé ÷àñòè íåñôîðìèðîâàâøåãîñÿ êîðíåâîãî êàíàëà, áûëî îáíàðóæåíî ïðè ïîìîùè ðàäèîãðàôè÷åñêîãî èññëåäîâàíèÿ ïàöèåíòà ñ îñëîæí¸ííûì ïåðåëîìîì êîðîíêè. Ïîïûòêè âûòàùèòü èíîðîäíûé ïðåäìåò ïðèâåëè ê ïðîòàëêèâàíèþ åãî äàëüøå â ïåðèàïèêàëüíóþ îáëàñòü.  êîíöå êîíöîâ, ïðåäìåò áûë óäàë¸í ïðîñòûì ñïîñîáîì ïîñëå óäà÷íîãî çàõâàòà çà îêîíå÷íîñòü è òÿãè âäîëü îñè çóáà, ñ ïîñëåäóþùèì ïëîìáèðîâàíèåì êîðíåâîãî êàíàëà è ïîêðûòèåì æàêåòíîé êîðîíêîé.

Âûâîäû: èíîðîäíûå òåëà â êîðíåâûõ êàíàëàõ äîëæíû áûòü òùàòåëüíî èññëåäîâàíû â îòíîøåíèè èõ ïðèðîäû, ðàñïîëîæåíèÿ, ðàçìåðà, ñòåïåíè ñëîæíîñòè è äîïîëíèòåëüíûõ òðóäíîñòåé, êîòîðûå ìîãóò âîçíèêíóòü â ïðîöåññå óäàëåíèÿ.

Òåðïåíèå, àêêóðàòíîñòü è íàëè÷èå ïîäõîäÿùèõ èíñòðóìåíòîâ ìîãóò áûòü ïîëåçíûìè ïðè óäàëåíèè èíîðîäíîãî òåëà, âî èçáåæàíèå îïåðàòèâíîãî âìåøàòåëüñòâà â îáëàñòè âåðõóøêè êîðíÿ.

Ê ëå÷åíèþ îñëîæí¸ííûõ ïåðåëîìîâ êîðîíîê ñëåäóåò ïðèáåãàòü íåçàìåäëèòåëüíî, èçáåãàòü äîëãîãî îòêðûòîãî äðåíàæà ó äåòåé, åñëè âèäíî, ÷òî ðèñêè ïîñëåäñòâèé âîçäåéñòâèÿ èíîðîäíîãî òåëà ìèíèìèçèðîâàíû.

Êîíòàêòíûé àëëåðãè÷åñêèé äåðìàòèò îò âîçäåéñòâèÿ íèêåëÿ ìîæåò ðàçâèòüñÿ â ëþáîì âîçðàñòå. Åñëè õîòÿ áû ðàç ðàçâèëàñü àëëåðãèÿ íà íèêåëü, òî çàáîëåâàíèå çàòåì óïîðñòâóåò ìíîãèå ãîäû, è ÷àùå âñåãî îñòà¸òñÿ íà âñþ ïîñëåäóþùóþ æèçíü.

Àëëåðãèÿ íà íèêåëü êàê ïðàâèëî âîçíèêàåò ó æåíùèí, âîçìîæíî ïîòîìó, ÷òî îíè áîëüøå óâëåêàþòñÿ ïèðñèíãîì, ÷åì ìóæ÷èíû, õîòÿ ìóæ÷èíû èõ óæå â ýòîì äîãîíÿþò.

Àëëåðãèÿ íà íèêåëü îñîáåííî ñèëüíî ðàñïðîñòðàíÿëàñü â ïîñëåäíèå ãîäû â ñâÿçè ñ ïîÿâëåíèåì íà ðûíêå îãðîìíîãî êîëè÷åñòâà äåø¸âûõ óêðàøåíèé, ïîêðûòèå êîòîðûõ, êàê ïðàâèëî, ñîäåðæèò íèêåëü. Ïðåäïîëàãàåòñÿ, ÷òî îò 10 äî 12% æåíñêîé ïîïóëÿöèè è 6% ìóæñêîé ïîïóëÿöèè ÿâëÿþòñÿ àëëåðãèêàìè íà íèêåëü. Àëëåðãèÿ îáóñëîâëåíà íå ñàìèì íèêåëåì, à åãî ñîëÿìè, êîòîðûå îáðàçóþòñÿ ïðè äîòðàãèâàíèè äî þâåëèðíûõ èçäåëèé, ÷àñîâ. Ýòîò ôåíîìåí òàêæå ñîïðîâîæäàåòñÿ êîððîçèåé ýòèõ ïðåäìåòîâ.

Ñòåïåíü ïðîÿâëåíèÿ àëëåðãèè ðàçëè÷íà. Ó íåêîòîðûõ ðàçâèâàåòñÿ äåðìàòèò (òàêæå íàçûâàåìûé ýêçåìîé) äàæå îò êðàòêîâðåìåííîãî êîíòàêòà ñ íèêåëü-ñîäåðæàùèìè ïðåäìåòàìè, à ó äðóãèõ - ïîñëå ìíîãîëåòíåãî êîíòàêòà íèêåëÿ ñ êîæåé. Ó íåêîòîðûõ ëþäåé ðàçâèâàåòñÿ ýïèçîäè÷åñêè èëè óñòîé÷èâî ïðîòåêàþùàÿ ýêçåìà íà ðóêàõ è ñòîïàõ.

Îáû÷íî ýòî ðàçâèâàåòñÿ â ðåçóëüòàòå êîíòàêòà ñ ïðåäìåòàìè, ñîäåðæàùèìè íèêåëü, íî ÷àñòî íåò î÷åâèäíîé ïðè÷èíû. Îáû÷íî ýòî òàê íàçûâàåìûé ãîðÿ÷èé òèï ýêçåìû, èçâåñòíûé êàê äèñãèäðîç, âîäÿíèöà (çàêóïîðêà ïðîòîêîâ ïîòîâûõ æåëåç). Ñóùåñòâîâàëî ïðåäïîëîæåíèå, ÷òî â íåêîòîðûõ ñëó÷àÿõ ðàçâèâàåòñÿ äèñãèäðîòè÷åñêèé äåðìàòèò ðóê, áëàãîäàðÿ ñîäåðæàíèþ íèêåëÿ â ïèùå. Ê ñîæàëåíèþ, íà ñåãîäíÿ íåâîçìîæíî èçáåæàòü âíåäðåíèÿ íèêåëÿ â îðãàíèçì ñ ïðîäóêòàìè ïèòàíèÿ. Äèåòà ñ íèçêèì ñîäåðæàíèåì íèêåëÿ, òîæå ðåäêî áûâàåò ïîëåçíîé è íå ÿâëÿåòñÿ ðàçðåøåíèåì ïðîáëåìû.

Åñëè Âû ñòðàäàåòå îò ýòîãî òèïà àëëåðãèè Âàì ñëåäóåò èçáåãàòü êîíòàêòà ñ íèêåëü-ñîäåðæàùèìè ìàòåðèàëàìè. Ïðîòåñòèðóéòå Âàøè âåùè íà ïðåäìåò ñîäåðæàíèÿ â íèõ íèêåëÿ. Ïðèîáðåòèòå íàáîð äëÿ òåñòèðîâàíèÿ â äîìàøíèõ óñëîâèÿõ. Îí ñîäåðæèò äèìåòèëãëèîêñèì dimethylglyoxime è àììîíèÿ ãèäðîêñèä ammonium hydroxide. Èõ ñìåøèâàþò íà ïîâåðõíîñòè ïðåäìåòà, îêðàøèâàíèå â ðîçîâûé öâåò ãîâîðèò î íàëè÷èè íèêåëÿ. Íàíåñèòå ïî êàïëå èç êàæäîé áóòûëî÷êè íà ìåòàëëè÷åñêèé ïðåäìåò, ñíà÷àëà ïîïðîáóéòå ýòî íà 10 öåíòîâîé ìîíåòå. Èñïîëüçóéòå õëîï÷àòîáóìàæíûé òàìïîí, àêêóðàòíî ïðîòðèòå è ïîñìîòðèòå – êàê îêðàñèëñÿ òàìïîí. Åñëè îí îñòàëñÿ ÷èñòûì, ïðåäìåò íå ñîäåðæèò ñâîáîäíîãî íèêåëÿ è íå âûçîâåò äåðìàòèòà. Åñëè íà òàìïîíå áóäóò ðîçîâûå ïÿòíà, - çíà÷èò îí ñîäåðæèò íèêåëü. Õèìèêàòû ýòîãî íàáîðà íå ïîâðåäÿò Âàøèì þâåëèðíûì èçäåëèÿì.

Èîíû ìåòàëëà, êîòîðûå âûñâîáîæäàþòñÿ èç ñòîìàòîëîãè÷åñêèõ ðåñòàâðàöèé ìîãóò ïðîâîöèðîâàòü ñèñòåìíûå è ëîêàëüíûå àëëåðãè÷åñêèå ðåàêöèè. Îäèí èç ïàöèåíòîâ ïîëó÷èë íåôðîïàòèþ, 139 ñëó÷àåâ ïîêàçàëè ìåñòíûå ïðîÿâëåíèÿ, ïðåèìóùåñòâåííî ãèíãèâèò è ñòîìàòèò.

Òîëüêî 33 èç 139 ïàöèåíòîâ ïîêàçàëè îáùóþ ðåàêöèþ. Àíàëèç ýòèõ äàííûõ ïîêàçûâàåò, ÷òî ìåñòíûå àëëåðãè÷åñêèå ðåàêöèè ÷àñòî ïðèíèìàþòñÿ çà âîñïàëèòåëüíûå ðåàêöèè. Èíòåðåñíî, ÷òî ìíîãèå ïàöèåíòû èñïûòûâàþò íåïåðåíîñèìîñòü çîëîòûõ óêðàøåíèé.

Äâîéíîé ñëåïîé ìåòîä èññëåäîâàíèÿ, âêëþ÷àÿ patch-test screening 708 ïàöèåíòîâ ïîêàçàë 12 ïîëîæèòåëüíûõ ðåàêöèé(1.7%): Ni (ïÿòü), Cr (øåñòü), and Co (îäèí) (Morris, 1987). Ñëåäóåò çàìåòèòü, ÷òî ïîëîæèòåëüíûå êîæíûå ðåàêöèè íå îáÿçàòåëüíî àññîöèèðóþòñÿ ñ âíóòðèðîòîâûìè àëëåðãè÷åñêèìè ðåàêöèÿìè. Íàïðèìåð, îáíàðóæåíî, ÷òî ïàöèåíòû èìåþùèå â àíàìíåçå ïîëîæèòåëüíûå êîæíûå ðåàêöèè íà íèêåëü íå èìåëè ðàçâèòèÿ ìåñòíûõ èëè ñèñòåìíûõ ïðîÿâëåíèé â îòâåò íà íèêåëü-ñîäåðæàùèé ñïëàâ âî âðåìÿ ïåðèîäà îáñëåäîâàíèÿ â 15 ëåò.
Ðàçëè÷íûå ñïëàâû, ïðèìåíÿåìûå äëÿ ìåòàëëîêåðàìèêè ñîäåðæàò Èðèäèé, Èíäèé; ïîëîæèòåëüíûå ðåàêöèè íà ýòè ìåòàëëû íàáëþäàëèñü ó 5 ïàöèåíòîâ èç 205 èñïûòóåìûõ.

Çàäîêóìåíòèðîâàíû ðåçóëüòàòû òåñòèðîâàíèÿ 60 âîëîíò¸ðîâ: ðåçóëüòàòû – íèêåëü ÿâëÿåòñÿ ñàìûì ñèëüíûì àëëåðãåíîì, çàòåì èäóò K, Co, Ag, Cu, Pd, Pt, and Au.  èñïûòóåìîé ãðóïïå ñ ðåñòàâðàöèÿìè 23% èìåëè ãèíãèâèò, ïîâðåæäåíèÿ ÿçûêà ó 16%, è 5 èç 86 – ëèõåíîèäíûå (ëèøàé) ïîðàæåíèÿ.

Abstract


Ñðåäñòâà ãèãèåíû ïîëîñòè ðòà ñ ñîäåðæàíèåì íàíî-êîìïëåêñà - êàçåèí ôîñôîïåïòèä è àìîðôíûé êàëüöèé ôîñôàò (CPP-ACP) – èñïîëüçóåòñÿ â íåêîòîðûõ âèäàõ ïðîäóêöèè äëÿ ãèãèåíû (çóáíûõ ïàñòàõ, æåâàòåëüíûõ ðåçèíêàõ, ïîëîñêàíèÿõ), à òàêæå â ïëîìáèðîâî÷íûõ ìàòåðèàëàõ. CPP-ACP- ñîäåðæàùèå ïðîäóêòû óëó÷øàþò ðåìèíåðàëèçàöèþ òâ¸ðäûõ òêàíåé çóáîâ, è, òàêèì îáðàçîì ìîãóò èãðàòü ðåøàþùóþ ðîëü â ïðåäóïðåæäåíèè è òåðàïèè íà÷àëüíûõ ñòàäèé êàðèåñà èëè ýðîçèé ýìàëè. Ê òîìó æå, ïðè ïîâûøåííîé ÷óâñòâèòåëüíîñòè çóáîâ è äàæå â ñëó÷àÿõ ãèïîñàëèâàöèè («ñóõîé ðîò»), CPP-ACP-ñîäåðæàùèå ïðîäóêòû â öåëîì óëó÷øàþò êëèíè÷åñêîå ñîñòîÿíèå ïàöèåíòà. Íàñòîÿùàÿ ñòàòüÿ ïðåñëåäóåò èññëåäîâàíèå òð¸õ çàäà÷: ïîëó÷åíèå CPP-ACP èç êàçåèíà êîðîâüåãî ìîëîêà, îïèñàíèå âîçìîæíûõ áèîõèìè÷åñêèõ äåéñòâèé CPP-ACP â òâ¸ðäûõ òêàíÿõ çóáîâ, è êðèòè÷åñêèé îáçîð òåêóùåé ëèòåðàòóðû ïî äàííîé ïðîáëåìå.

Ìå: ß âçÿë 10 ïîñëåäíèõ ñòðàíèö íàøåãî ðàçäåëà, òî åñòü 240 ñîîáùåíèé, è îáíàðóæèë ñðåäè íèõ ïðèìåðíî 20 òåì ïîñâÿù¸ííûõ ïîñòïëîìáèðîâî÷íûì áîëÿì. Òàêàÿ âûáîðêà è ìåòîä ïîäñ÷¸òà íå ìîæåò ïðåòåíäîâàòü íà ñêîëüêî-íèáóäü îáúåêòèâíîå èññëåäîâàíèå, íî ïî ìîåìó ìíåíèþ ìîæåò ñëóæèòü ïîâîäîì ïîïðîáîâàòü óãëóáèòüñÿ â ýòîò âîïðîñ è ïîïðîáîâàòü ïîíÿòü ïðè÷èíû ýòîãî ÿâëåíèÿ. Ìåíÿ êîðîáèò òî îáñòîÿòåëüñòâî, ÷òî íàì ïðèõîäèòñÿ ïðîñèòü ïàöèåíòîâ ïîòåðïåòü èëè äàæå íàçíà÷àòü îáåçáîëèâàþùèå. Âðîäå áû â ýòîì íè÷åãî îñîáåííîãî íåò, íî áîëè íå òàê-òî óæ ðåäêè, è èíòåíñèâíîñòü èõ áûâàåò î÷åíü âûñîêîé. Ýòî âîïðîñ ïðèìåíÿåìûõ ìàòåðèàëîâ?, ýêñïîçèöèÿ?, ìåòîäèêà ïðèìåíåíèÿ ìåäèêàìåíòîâ â êàíàëå?, ïîïðîáóåì ðàçîáðàòüñÿ, âåäü ïóëüïû óæå íåò. Íåêîòîðûå èññëåäîâàíèÿ äîêàçûâàþò, ÷òî íåò ïðåèìóùåñòâ ãèäðîîêèñè êàëüöèÿ è õëîðãåêñèäèíà ïðè îáðàáîòêå êàíàëà, òàêæå íåò ïðåèìóùåñòâ ïðè ëå÷åíèè â îäíî-, äâà- , -íåñêîëüêî ïîñåùåíèé.

Abstract

ÏÐÅÄËÀÃÀÅÒÑß: Îöåíèòü ðàñïðîñòðàí¸ííîñòü è ïðîäîëæèòåëüíîñòü áîëåé ïîñëå ñòîìàòîëîãè÷åñêèõ âìåøàòåëüñòâ è èñïîëüçîâàíèå àíàëüãåòè÷åñêèõ ñðåäñòâ ó âçðîñëûõ, ñ ó÷¸òîì ãåíäåðíîãî ôàêòîðà, ìåòîäèêè ââåäåíèÿ îáåçáîëèâàþùåãî ïðåïàðàòà, ñ ó÷¸òîì êîíêðåòíîãî âìåøàòåëüñòâà, è èñòîðèè áîëåçíè îðãàíà (çóáà).

ÌÅÒÎÄÛ ÈÑÑËÅÄÎÂÀÍÈß: ×àñòîòà è ïðîäîëæèòåëüíîñòü áîëåé ïîñëå ñòîìàòîëîãè÷åñêèõ âìåøàòåëüñòâ è èñïîëüçîâàíèå àíàëüãåòè÷åñêèõ ñðåäñòâ áûëè èññëåäîâàíû ó 255 ïàöèåíòîâ, 18-42 ëåò, êîòîðûå ïîëó÷èëè îáû÷íîå äåíòàëüíîå ëå÷åíèå, âêëþ÷àÿ àìàëüãàìíûå è êîìïîçèòíûå ðåñòàâðàöèè, ëå÷åíèå êîðíåâîãî êàíàëà, ïëîìáèðîâàíèå êàíàëà, øèíèðîâàíèå, è óäàëåíèÿ çóáîâ. Àíåñòåçèÿ áûëà êàê èíôèëüòðàöèîííàÿ, òàê è ïðîâîäíèêîâàÿ. Èíôîðìàöèÿ ïðèíèìàëàñü ïî òåëåôîíó ÷åðåç 24 ÷àñà ïîñëå ëå÷åíèÿ.

ÐÅÇÓËÜÒÀÒÛ: Îáùåå ÷èñëî ñëó÷àåâ áîëè áûëî 40,4% (103 ïàöèåíòà), èç êîòîðûõ 40% èñïûòûâàëè íåçíà÷èòåëüíóþ áîëü. Îáåçáîëèâàþùèå ïðåïàðàòû ïðèìåíÿëèñü 32% ïàöèåíòàìè. Èíòåíñèâíîñòü áîëè êîððåëèðîâàëà ñ òèïîì âìåøàòåëüñòâà, íàèâûñøàÿ áîëü áûëà ñâÿçàíà ñ ïëîìáèðîâàíèåì êàíàëà, 52,8% èç íèõ îòìå÷àëè óìåðåííûå áîëè, ñîâñåì íåçíà÷èòåëüíûå ïîñëå ðåñòàâðàöèé (36,1%). Íà áîëè æàëîâàëèñü ïðåèìóùåñòâåííî æåíùèíû – 52,2%, ìóæ÷èíû – 33,7%. Ñëó÷àè áîëè íå ñâÿçàíû ñ ãëóáèíîé è îáú¸ìîì ðåñòàâðàöèé. Îäíàêî, èíòåíñèâíîñòü áîëåé, åñëè ñóäèòü ïî ÷àñòîòå ïðè¸ìà àíàëüãåòèêîâ, ñâÿçàíà ñ ãëóáèíîé ðåñòàâðàöèé (ñâûøå 3ìì).

Ãèäðîêñèä êàëüöèÿ íàø¸ë ñâî¸ ïðèìåíåíèå â ýíäîäîíòèè. Ñ äðóãîé ñòîðîíû, õëîðãåêñèäèí âñ¸ áîëüøå è áîëüøå ïðåäëàãàåòñÿ äëÿ èñïîëüçîâàíèÿ â êëèíè÷åñêîé ýíäîäîíòèè. Ñìîæåò ëè õëîðãåêñèäèí ïðåâçîéòè ãèäðîêñèä êàëüöèÿ è êàêîâû åãî ïðåèìóùåñòâà ïî ñðàâíåíèþ ñ ãèäðîêñèäîì êàëüöèÿ. Ýòî ëèòåðàòóðíîå îáîçðåíèå ñîäåðæèò äàííûå î òîì, ÷òî îáà ïðåïàðàòà ïîëåçíû è íåîáõîäèìû äàëüíåéøèå èññëåäîâàíèÿ î ïðåâîñõîäñòâå îäíîãî íàä äðóãèì.
