Лечение предгипертонии (ГБ I стадии)
Stevo Julius. Feasibility of Treating Prehypertension with an Angiotensin-Receptor Blocker. Trial of Preventing Hypertension (TROPHY) Study. N Engl J Med 2006;354. [Ссылки доступны только зарегистрированным пользователям ]
"Conclusions: Over a period of four years, stage 1 hypertension developed in nearly two thirds of patients with untreated prehypertension (the placebo group). Treatment of prehypertension with candesartan appeared to be well tolerated and reduced the risk of incident hypertension during the study period. Thus, treatment of prehypertension appears to be feasible." Известно, что пациенты с эпизодическими подъёмами АД имеют больший шанс развития АГ. В представленной работе лечение АРА в течение 3 лет достоверно предотвращало развитие стойкой АГ. Неизвестно, влияет ли на жесткие конечные точки лечение таких пациентов. 1. Эффективность немедикаментозного лечения транзиторной АГ невысокая. Не будет ли проще для пациента/доктора сразу вручить безопасный/эффективный препарат для предотвращения АГ? Сколько пациентов мы будем "перелечивать"? 2. Сколько времени следует оценивать влияние немидикаментозного лечения? Как часто в Вашей практике модификация образа жизни приводила к снижению АД? |

Верошпирон в целях снижения АД - крайне редко (спасибо за статью, буду знать). Собственно, с верошпироном могу вспомнить только одну даму, по всей видимости, с с-мом Конна. |

А насчет снижения калорийности питания и прочего БЕЗ изменения мотивации, приводящей к обжорству-ожирению, я в другой теме отвечала. Это симптоматическая терапия. И эффективность соответствующая. У нас есть чудесная модель. "Три ночных сеанса психотерапии". Эффект потрясающий, но длится максимум года 1,5 - 2, а потом надо снова проходить "обработку", иначе вес ползет обратно.

"The GDG stated that beta blockers were usually less effective in the head-to-head trials in reducing cardiovascular events, particularly stroke. Further, because most of these trials used atenolol and because of the limited data with another beta blocker, it is unclear whether this is a drug class effect or a specific dug effect. However, they note that if the data with atenolol studies are excluded, the “total evidence on the use of beta blockers for the treatment of hypertension is much less than for the other main drug classes.”Thus, beta blockers should not be used as initial therapy, unless there is a compelling indication. The GDG also stated that good studies with beta blockers other than atenolol are needed. Further, concerns about the increased risk of new-onset diabetes related to beta blockers also factored into their excluding the use of beta blockers, except for compelling indications." |

К вопросу о комплаенсе
Пациента с хроническим заболеванием (например, АГ) не только трудно мотивировать на изменение образа жизни, но и очень, очень тяжело убедить постоянно принимать лекарства. Интересная статья о "трудном" пациенте, без сомнения заслуживающая внимания.

В моей практике резистентная АГ встречается очень часто (полиморбидные пожилые больные с СД 2 типа). Даже при условии аккуратного регулярного приема препаратов (с модификацией образа жизни завсегда проблемы у таких бабушек, но пытаемся).
Комбинация зависит от букета сопутствующих патологий, но обычно включает ингибитор+гипотиазид, амлодипин, бета-блокер. Верошпирон - тоже редко.

Резистентную АГ вижу при отсутствии комплаенса, а такое наблюдается, увы, нередко.
В дело идут ИАПФ (чаще что то из эналаприла, но может и рами-, и лизино-), диуретик (чаще индапамид), пролонгированный дигидропиридиновый антагонист Са и бета-блокер - почти обязательно, поскольку большинство гипертоников с ИБСом. Вообще, АГ, если она не симптоматическая, для стацлечения - болячка очень благодарная. Вот удержать полученный эффект на амбулаторном этапе - задачка посложнее. |
