Advances in molecular medicine have allowed the development of designer drugs that target specific cells, molecules, and receptors, allowing us the opportunity to limit damage to the immune system while trying to clear psoriasis. The first such drug approved for psoriasis was alefacept, a fusion protein in which the Fc portion of human IgG1 is linked to leukocyte-function-associated antigen type 3 (LFA-3), a co-stimulatory ligand that binds to CD2 on the T cell. The latter interaction is necessary for T-cell activation. While only a minority of patients achieves clearing with alefacept, the majority improves, and remissions are often prolonged. The remission may be attributed to CD45RO+ T-cell reduction caused by alefacept. Efalizumab, a second drug that interferes both with T-cell activation and with trafficking of T cells into inflamed tissues, is a humanized monoclonal antibody against the CD11a chain of LFA-1. This rapidly acting drug has recently gained approval.
Two TNF-а blocking agents previously used to treat rheumatoid arthritis and Crohn's disease are in advanced stages of testing for psoriasis and have proven highly effective. Etanercept, a fusion protein consisting of the Fc portion of human IgG1 fused with the human p75 TNF receptor, targets the trimer form of soluble TNF-а. This agent is highly effective for psoriasis and provides not only a dramatic remission of psoriatic arthritis, but prevents radiographic progression of disease. It is already approved for the treatment of psoriatic arthritis and is likely to gain approval for psoriasis in the near future. Infliximab, a chimeric monoclonal antibody to TNF-а, targets both monomer and trimer forms of soluble as well as cell-bound TNF-а. This drug has proven to be one of the most effective psoriasis therapies available.
