Treatment of herpetic whitlow is essentially nonsu rgical. The course of the disease is self-limiting. The main goal of treatment is to prevent autoinoculation or transmission of the infection. The digit should be kept covered with a dry dressing, and all contact with the lesion should be avoided. Unroofing of the vesicles may improve patient comfort. Intravenous, oral, or topical antiviral medication may be useful in shortening the natural course of the infection. Partial nail excision may be indicated for a subungual herpetic lesion causing pressure below the nail plate. Surgical débridement of bact erial suprainfection must be done cautiously.
