Уважаемые коллеги!
В Российских клиниках используется препарат Викасол - бисульфитное производное синтетического витамина К"3" /менадион/.
Как Вы считаете, рекомендации Американцев по использованию витамина К"1" - 1 мг/кг соответствуют той же дозировке для викасола- 0,1 мл/кг?
Какие дозы Вы используете?

Канадцы напр. рекомендуют следующий режим:

Vitamin K1 should be given as a single intramuscular dose of 0.5 mg (birthweight 1500 g or less) or 1.0 mg (birthweight greater than 1500 g) to all newborns within the first 6 h after birth following initial stabilization of the baby and an appropriate opportunity for maternal (family)-baby interaction.

For newborn infants whose parents refuse an intramuscular injection, the physician should recommend an oral dose of 2.0 mg vitamin K1 at the time of the first feeding. Use of the parenteral form of vitamin K for oral administration is all that is currently available. This should be repeated at two to four weeks and six to eight weeks of age.

так как ...menadione, even in low doses, shows significant toxicity in neonates...

И ситуация с К3 такова: Menadione (2-Me-1,4-naphthoquinone) is currently used in animal feeds, but not in foods for human consumption, because it causes hemolytic anemia, hyperbilirubinemia, and kernicterus in infants.

Все по той же причине, так как в нашей стране не зарегистрированы необходимые в неонатологии фармакологические препараты.
Почему мы не любим недоношенных детей с ОАП - у нас нет внутривенного индометацина?
У нас в стране много чего не зарегистрировано.
У нас нет внутривенного фенобарбитала - препарата №1 в купировании судорог у новорожденных, и так далее...

С фенобарбиталом (как и с викасолом) похоже все понятно: государственная монополия - и другого (других) нам не надо.
Насчет индометацина: похоже, что существует иньекционный польский метиндол, может правда его перестали ввозить или не годится для в/в введения - этих тонкостей не знаю.
Но интересно следующее: более 10 работ опубликовано, показывающих, что в/в ибупрофен столь же эффективен, но менее токсичен:

Конечно, и ибупрофена парентерального-то нет, но тайцы показали, что и пероральный не так уж и плох:
