Сразу хочется принести свои извинения за досадную опечатку в моих предыдущих высказываниях: фраза из книги Whintrobe's Clinical Hematology, в разделе кровь и гипотиреоз конечно же выглядит таким образом: "Микроцитарная микрохромная анемия выявляющаяся при микседеме имеет железодефицитный генез из-за обилизации месячных кровопотерь у женщин, снижении кислотности желуд. сока, а также отсутствия стимулирующего влияния Т4 на всасывание железа. Ее коррекция Т4 зачастую неэффективна без назначения пр-тов железа, а коррекция железного статуса приводит к ликвидации АНЕМИИ."
о целесообразности определения ТТГ при обильных месячных: мне нисколько не нужно было дискредитировать или ставить под сомнение правильность этой рекомендации В ГЛАЗАХ ПАЦИЕНТКИ, а вообще о диагностической ценности этого теста в данной клинической ситуации (если Вы помните, то я обратился лично к Вам в совершенно отдельном от своих рекомендаций пациентке топике). На начало разговора подтолкнули отрывочные данные, прочитанные по теме и благодаря Вашим ответам, теперь картина более или менее мне прояснилась.

Sideropenia affects ca. 20% of the world population, and iron dependent anemia is the most frequent type of anemia worldwide. The aim of the study was to investigate the incidence of sideropenia and dependent anemia in patients with subtle changes of the thyroid function, such as subclinical hypothyroidism (SH). 57 women with SH and 61 euthyroid controls (CG) were studied. Serum concentrations of T4, T3, TSH, anti-TPO, anti-Tg, ferrum (Fe), ferritin (Frt) total iron binding capacity (TIBC) and blood count were determined. In SH 17 patients (29.8%) presented low Fe levels (<50 microg/dl). 9 (15.7%) also had decreased Frt, confirming iron deficiency, whereas 8 patients presented additionally diminished hematocrit and hemoglobin levels, suggesting manifested sideropenic anemia. In CG, 10 persons (16%) had sideropenia, 6 (9.8%) had low Fe and Frt and only 3 (4.9%) had blood count alterations suggesting manifested sideropenic anemia. In SH, anti-TPO were positive in 39 patients (68%), whereas, in CG only 2 (3.2%) were positive. 8 patients with SH and manifested sideropenic anemia were treated with ironproteinsuccinylate (I-PSL), (80 mg Fe /day, for three months), a new iron compound. The repletion treatment safely led to the clinical and laboratory correction of sideropenia and showed a good tolerability. Furthermore, iron treatment provoked a minor increase of T4 and a mild decline of TSH, but the levels were not significant. These results suggest that sideropenia is a common finding in patients with slightly decreased thyroid activity, and that determination of Frt should be routinely advised. Finally, in the assessment of sideropenia and dependent anemia, evaluation of the thyroid function must be taken into account.

A high incidence of occult hypothyroidism in menorrhagic women has been reported and emphasized in recent studies. In order to verify this statement, we have evaluated the functional status of the thyroid gland in intrauterine device (IUD) wearers suffering from increased menstrual bleeding. The study group consisted of 40 IUD-wearing women, aged 26-46 years, suffering from metrorrhagia. The control group consisted of 38 IUD-fitted women, 22-44 years old, in whom menstrual bleeding was not significantly increased. Menorrhagia was defined as excessive vaginal bleeding lasting for 6 days or more, and/or containing a significant amount of blood clots. Free thyroxine (FT) and thyroid-stimulating hormone (TSH) levels were assessed. FT levels were identical in both groups and within the normal range (1.31 +/- 0.28 ng/dl). TSH levels were significantly higher in the study group than in the controls, although they remained within the normal range (2.75 +/- 2.06 vs. 1.45 +/- 0.45 microU/ml, p < 0.01). A thyrotropin-releasing hormone (TRH) test was performed in 10 women having the highest TSH levels. All the results of TRH tests were consistent with occult hypothyroidism. These women were treated with L-thyroxine and all had a significant improvement in their bleeding within 3 months of treatment. We concluded that any IUD-wearing woman suffering from menorrhagia may have occult hypothyroidism. Should FT and TSH be within normal limits, a TRH test should be performed as the definitive diagnostic test. A frequent side-effect of intrauterine contraception is menometrorrhagia. The intrauterine device (IUD) has to be removed in 5-15% of cases to prevent iron deficiency anemia.
можно заключить, что вряд ли кто сможет сказать какое состояние у женщин было первичным ЖД или лаб. признаки гипотиреоза, изменения в гормонах ЩЖ при ЖД/ЖДА настолько минимальны, что хоть и усиливаются меноррагии, но ТТГ/Т4 находятся в пределах лаб. диапазонов норм, поэтому для возможных доказательств этиопричины ЩЖ потребуются или более чувствительные исследования или сравнение с первоначальными данными конкретного пациента (теперь понятно почему гинекологические-эндокринологические изыскания Ваших коллег так и не достигли своей цели), коррекция ЖД ведет к улучшению ЩЖ-гормонального статуса, указывая на важность ионов Fе в выработке этих гормонов, хотя опять эти параметры могут колебат'ся в пределах лаб. норм.
Кстати, а не существует ли в тирологии коэффициент соотношения Т4 к ТТГ или наоборот, судя он мог бы быть более чувствительным маркером для диагностики субклинических (лабораторных) гипотиреозов, нежели ТТГ или Т4 в отдельностях (провожу аналогию с Fе/ОЖСС - показатель насыщения трансферрина железом -более чувствительный маркер ЖД)?
И не стоит забывать о таком феномене как простое сосуществование различных клин. патологий, особенно в сфере повышенной частоты встречаемости как ЖД так и гипофункции среди женского населения, тогда же можно рекомендовать все возможные анализы, но во всем мире их все же назначают, исходя из высокой клинической вероятности, те опираясь на клиническую картину.
