ÁÎËÈ Â ÏËÅ×ÅÂÎÌ ÑÓÑÒÀÂÅ
Çäðàâñòâóéòå, óâàæàåìûå ñïåöèàëèñòû! Ó ìîåé ìàìû (48 ëåò) óæå äâà ìåñÿöà ñèëüíûå áîëåâûå îùóùåíèÿ â ïëå÷åâîì ñóñòàâå, áîëü âîçíèêàåò ïðè ïîäíÿòèè ïðÿìîé ðóêè ââåðõ, íà óðîâåíü ïëå÷à (î÷åíü ñèëüíàÿ áîëü ïî âñåìó ïðåäïëå÷üþ), ïðè íàïðÿæåíèè ìûùö - ðåçêàÿ áîëü äî ëîêòÿ, êîãäà ñæèìàåò ðóêàìè ïðåäïëå÷üå òîæå áîëüíî. Òðàâì íå áûëî, ïðîñòî ïîòÿíóëàñü ðóêîé íàçàä (÷òîáû ÷òî-òî âçÿòü), îòâåëà ïðÿìóþ ðóêó çà ñïèíó è ïîñëå ýòîãî íà÷àëèñü áîëè. Ñäåëàëè ñíèìîê - çàêëþ÷åíèå ðåíòãåíîëîãà - êîñòíàÿ òêàíü áåç èçìåíåíèé. ×òî ìîæåò âûçûâàòü òàêèå áîëåâûå îùóùåíèÿ è êàêîìó âðà÷ó åé îáðàòèòüñÿ, êàêèå ïðîöåäöóðû ïîêàçàíû? Ñïàñèáî çà îòâåò.

Çäðàâñòâóéòå, óâàæàåìûå ñïåöèàëèñòû! Ñïàèáî Âàì çà âíèìàíèå! ß óæå ïèñàëà î ïðîáëåìå ìîåé ìàìû â ïåðâîì ïîñòå - áîëè â ïëå÷åâîì ñóñòàâå.
Ïîñåòèëà òðàâìàòîëîãà - äèàãíîñòèðîâàë çàñòàðåëóþ òðàâìó ìàíæåòêè ïëå÷åâîãî ñóñòàâà. Ïðîïèñàë ìàãíèòîòåðàïèþ, ïðåïàðàò Äîíà, óêîëû âàëüòàðåíà + íèêîòèíîâîé êèñëîòû (ñ äèìåäðîëîì), íàðóæíî âàëüòàðåí. Èç ðåêîìåíäàöèé âûïîëíèëà: 5 óêîëîâ äèêëîôåíàêà + íàðóæíî âàëüòàðåí, âûïèëà 1 óïàêîâêó Äîíû (íå äî êîíöà). Ñåãîäíÿ ñäåëàëà ÓÇÈ ñóñòàâîâ, ïðèâåäó ïðîòîêîë: ïëå÷åâûå ñóñòàâû: - öåëîñòíîñòü ðîòàðíîé ìàíæåòêè ñîõðàíåíà - ñòðóêòóðà ñóõîæèëèé íàäîñòíîé, ïîäîñòíîé, ïîäëîïàò÷àòîé ìûùö äèôôóçíî óïëîòíåíà, ñ âêðâàïëåíèÿìè êàëüöèÿ - ïîääåëüòîâèäíàÿ ñóìêà íå âèçóàëèçèðóåòñÿ - äåëüòîâèäíàÿ ìûùöà îäíîðîäíàÿ, õîä âîëîêîí åå îáû÷íûé - êîíòóð ïëå÷åâîé êîñòè íåîäíîðîäíûé, ñ ó÷àñòêàìè óïëîòíåíèÿ - ñóõîæèëèå äâóãëàâîé ìûùöû ïëå÷à áåç îñîáåííîñòåé - öåëîñòíîñòü êëþ÷è÷íî-àêðîìèàëüíîãî ñî÷ëåíåíèÿ íå íàðóøåíà - ñâîáîäíîé æèäêîñòè â ïîëîñòè ñóñòàâà íå âûÿâëåíî. Ñëåâà - àíàëîãè÷íûå èçìåíåíèÿ, íî â ìåíüøåé ñòåïåíè âûðàæåííîñòè. Çàêëþ÷åíèå: ÓÇ ïðèçíàêè ÷àñòè÷íîãî ïîâðåæäåíèÿ ðîòàðíîé ìàíæåòêè îáîèõ ïëå÷åâûõ ñóñòàâîâ, â áîëüøåé ñòåïåíè âûðàæåííîñòè - â ïðàâîì. Óñòíî óçèñòêà ñêàçàëà, ÷òî íà÷èíàåòñÿ îñòåîàðòðîç... Íóæíû ëè åùå êàêèå-ëèáî äèàãíîñòè÷åñêèå ïðîöåäóðû? Êàêàÿ òåðàïèÿ? Îáðàòèòü äåãåíåðàòèâíûå ïðîöåññû âîçìîæíî? Ñïàñèáî áîëüøîå. |

Êàêîâà àêêóðàòíîñòü ÓÇÈ äëÿ äèàãíîñòèêè ïîâðåæäåíèé ðîòàòîðíîé ìàíæåòû - íå çíàþ, íî ïîäîçðåâàþ, ÷òî ëó÷øå ñäåëàòü ìàãíèòíî-ðåçîíàíñíîå èññëåäîâàíèå. Åñëè ÿ íå ïðàâ - ïóñòü êîëëåãè ìåíÿ ïîïðàâÿò Îäíîçíà÷íî, "çîëîòîé ñòàíäàðò" - äèàãíîñòè÷åñêàÿ àðòðîñêîïèÿ ïëå÷åâîãî ñóñòàâà. |

Óâû, àðòðîñêîïèÿ ïëå÷à äëÿ ìåíÿ - ýòî ïîòÿíóòü çà ðóêó íà àññèñòåíöèè ñâîåìó êîíñóëüòàíòó â ÷àñòíîé êëèíèêå. Ñàì íå äåëàþ, è íå áóäó åùå äîëãî äåëàòü rotator cuff repair ïî 2 ïðè÷èíàì – 1 â ãîñ áîëüíèöå, ãäå ÿ ðàáîòàþ, íåò àðòðîñêîïà, à èäòè íà îòêðûòóþ îïåðàöèþ, íå ñäåëàâ ñêîïèþ, ñ÷èòàþ íåðàçóìíûì 2) ïðîöåíò íåóäîâëåòâîðèòåëüíûõ ðåçóëüòàòîâ äàæå â óìåëûõ ðóêàõ ñëèøêîì âûñîê, ÷òîá ÿ – îòíîñèòåëüíî ìîëîäîé èíîñòðàíåö – ðèñêîâàë ïðîô ðåïóòàöèåé
Íèêîãäà íå âèäåë ñòàòèñòèêå ïî ÿìð ïëå÷à, íî ïî ïîâîäó êîëåíà campbell ïèñàë, ÷òî òàê âûÿâëÿåòñÿ âñåãî äî 80 % ïîâðåæäåíèé. Ëè÷íûé îïûò - äóìàþ, ÷òî ïðîöåíò ñîâïàäåíèÿ àðòðîñêîïè÷åñêèõ è ìàãíèòíî-ðåçîíàíñíûõ äèàãíîçîâ - ïðîöåíòîâ 65-70, ýòî ïðè òîì, ÷òî ßÌÐ âñåãäà ïðîñìàòðèâàåì ñàìè, è íèêîãäà íå âåðèì çàêëþ÷åíèÿì ðåíòãåíîëîãîâ. Åñòü îùóùåíèå ( ïî ñêðîìíîìó ëè÷íîìó îïûòó â 10-20 àññèñòåíöèé), ÷òî ñèòóàöèÿ ñ ïëå÷îì åùå õóæå, ÷åì ñ êîëåíîì, íî ïîäòâåðäèòü ýòî êàêèìè-òî öèôðàìè íå ìîãó. Arthroscopic repair ìû íå äåëàåì – êâàëèôèêàöèÿ íå òà. Äåêîìïðåññèþ äåëàåì ïðàêòè÷åñêè âñåãäà. – àðòðîñêîïè÷åñêè, åñëè ïîëíîãî ðàçðûâà ìàæíåòû íåò, è îòêðûòî – åñëè âñå ðàâíî ïðèõîäèòñÿ îòêðûâàòüñÿ íà óøèâàíèå ðàçðûâà. |

The diagnosis of full thickness rotator cuff tear was highly specific on both imaging techniques (100% for US and 97.1% for MR) but was not as sensitive on US (57.7%) as on MR (80.8%). The diagnosis of partial tear was acceptably specific (67.9% for US and 75.5% for MR) but was not sensitive (overall, 12.5% for US vs 50% for MR). The specificity of a US diagnosis of a tear allows us to recommend surgery, thus making it a good imaging technique for the initial study of painful shoulders. The low sensitivity makes it necessary to undertake an additional MR study. If the MR also discovers no pathology, the sensitivity based on the US and MR images (92.7%) can be sufficient to avoid a false-negative diagnosis. Äèïðîñïàí íèêóäà "òûêàòü" íå íàäî, äî òåõ ïîð ïîêà íå ÿñåí äèàãíîç. ß áû ïîêà è ïðî àðòðîñêîïèþ ïëå÷åâîãî ñóñòàâà íå äóìàë. Åù¸ ÷åòêî íå ëîêàëèçîâàí î÷àã ïàòîëîãèè (ïî êðàéíåé ìåðå äëÿ ìåíÿ). À åñëè ýòî, íàïðèìåð, ñêàëåíèóñ-ñèíäðîì? Îòêóäà ýòè áîëè â ïðåäïëå÷üå (åñëè, êîíå÷íî, ïàöèåíòêà ïðàâèëüíî îðèåíòèðóåòñÿ, ÷òî èìåííî ÿâëÿåòñÿ ïðåäïëå÷üåì)? Íèêòî íå âèäåë ðåíòãåíîãðàìì. Ìíå áû õîòåëîñü èìåòü áîëüøå èíôîðìàöèè, ÷òîáû ñëîæèëîñü êàêîå-ëèáî ìíåíèå î âîçìîæíîé ïàòîëîãèè. - õàðàêòåð áîëåé (æãó÷èå, íîþùèå, îñòðûå) - áîëè òîëüêî ïðè äâèæåíèè, èëè â ïîêîå åñòü áîëè? Êàê ñïèòå íî÷üþ? - åñëè ÿ ïðàâèëüíî ïîíÿë, áîëè îò ïëå÷åâîãî ñóñòàâà ðàñïðîñòðàíÿþòñÿ íà ïðåäïëå÷üå (ýòî îáëàñòü íèæå ëîêòåâîãî ñóñòàâà, èíîãäà ïðîñòî çà ïðåäïëå÷üå ïðèíèìàþ âñ¸, ÷òî íèæå ïëå÷åâîãî ñóñòàâà). Îòäàþò ëè áîëè â ïàëüöû? Ïî êàêîé ïîâåðõíîñòè ïðåäïëå÷üÿ èäóò ýòè áîëè (êîòîðàÿ ñî ñòîðîíû 1 ïàëüöà èëè ñî ñòîðîíû 5 ïàëüöà)? Îòäàþò ëè áîëè â øåþ? - áûâàåò ëè ÷óâñòâî "ïîëçàíèÿ ìóðàøåê" â ðóêå? - îòåêàåò ëè ðóêà? - èìååòñÿ ëè ñâÿçü áîëåé â ðóêå ñ äâèæåíèÿìè â øåå (íàêëîíèòå ãëîâó â áîëüíóþ ñòîðîíó, çàòåì â çäðîâóþ) - âûëîæèòå ðåíòãåíîãðàììû |
