Уважаемый Евгений Аркадьевич, Вы "не одобрили" - какие лоскуты? откуда лоскуты? Я имел ввиду не грыжесечение, а абдоминопластику, т.к. до того как мои сообщения перевели в "новую тему" там пациенткой говорилось не только о восстановлении диастаза мышц передней брюшной стенки, но и о возможности проведения абдоминопластики. Поэтому я упомянул о некрозе лоскутов. С уважением и признательностью к Вам.

Уважаемый доктор, право, не вижу темы для обсуждения. Однако, как я понял, Вы бы хотели услышать мои пояснения к моей оценке? Извольте.
Изначально вопрос стоял совершенно конкретно: антибиотикопрофилактика при герниопластике и/или абдоминопластике. И немедленно началось смешение в одну кучу вопросов собственно антибиотикопрофилактики в плановой хирургии, различных типов раневых осложнений, антибиотикотерапии осложнений. Говорить обо всей этой, простите, куче сразу - невозможно. В очередном Вашем сообщении: Цитата:

Например в статье доктора К.Г. Абалмасова и его соавторов "Комплексная профилактика осложнений после хирургической коррекции передней брюшной стенки". - Анналы хирургии. - 2006. - №6. - С. 44-50 (эта статья недоступна в инете в полнотексте). Вот абстракт. Приведен анализ результатов собственного опыта хирургического лечения 82 пациентов, перенесших различные варианты абдоминопластики, в основном классическую и напряженно-боковую методики. Среди пациентов преобладали женщины. Основную возрастную группу составили пациенты от 30 от 49 лет. Анализ результатов операций и полученных осложнений проводился в зависимости от антропометрических, гистоморфологических данных и вида вмешательства. Выявлено, что напряженно-боковая абдоминопластика дает наименьшее количество осложнений; оставление на апоневрозе мышц передней брюшной стенки жировой ткани не более 0,8 см способствует предупреждению развития лимфореи и образования сером; комплексная терапия в послеоперационном периоде, в том числе ношение компрессионного трикотажа, также способствует профилактике осложнений. We analyzed our own experience treatment results of 82 patients who had undergone different types of abdominoplasty. In general, we used classical and tense-lateral techniques. The predominant patients were women. Mean age was 30 - 49 years. We analyzed the operation results and acquired complications depending on anthropometrical, histomorphological data and kind of operation. It was determined that tense-lateral abdominoplasty has the less number of complications; leaving on aponeurosis of anterior abdominal wall muscles of adipose tissue no more than 0.8sm. prevents lymphorrhea development and seromal formation; complex therapy in postoperative period, including wearing compressive knitwear also contributes to prevention of complications. В послеоперационном отделении пациенты прибывают до следующего дня, здесь им выполняют инфузионную терапию, профилактику тромбоэмболических и инфекционных осложнений..........С целью профилактики инфекционных осложнений в послеоперационном периоде пациенты на протяжении пяти дней получают антибиотики широкого спектра действия. ......При гладком течении послеоперационного периода пациентов переводят в отделение. Из обсуждения:Раневая инфекция может наблюдаться при попадании инфекции после операции или активации микробов, находившихся на коже до операции, её частота составляет 7,4% случаев. Этому способствует и широкая мобилизация кожно-жирового лоскута от апоневроза мышц передней брюшной стенки, при которой повреждаются кровеносные и лимфатические сосуды. Нагноение раны является также следствием развития серомы, гематомы, некроза мягких тканей. Вот поэтому я имел ввиду, что не зная заранее исходов оперативного вмешательства, в зависимости от факторов предрасполагающих к развитию осложнений категорично утверждать о ненужности применения антибиотиков в послеоперационном периоде скорее всего не стоит. Однако ВЫ и ваши коллеги хирурги были правы, что антибиотикопрофилактика при оперативных вмешательствах с образованием чистых ран не показана. Если не возражаете занесу Вас в список друзей. Вот только не надо одевать белые колпаки и вспоминать про "помытость рук хирурга". Не уподобляйтесь сестрам нашим меньшим! Руки тут совершенно не при чем. Как и АБ-терапия при герниопластиках.

ИМХО, проблема несколько более запутана - сушеcтвуют различные мнения (как всегда)!
Например: It is very important to underline that in incisional hernia the success of the procedure can be guaranteed only by an accurate preparation of the preperitoneal space: perfect haemostasis, temporary closure of the space inserting iodine gauzes, local antibiotic treatment, washing of the cavity and accurate drainage [3]. In general preperitoneal repair permits to have a wide vision of the inguinal, crural, and spigelian region. The dissection of this space allows to position a wide mesh that repairs the entire region with less risk of recurrence. The peritoneum also isolates the peritoneal cavity from the mesh with less risk of contamination. A systemic antibiotic therapy should be used as routine in these cases with higher risk of infection. Many studies have proven the validity of antibiotic chemotherapy in the prevention of postoperative infections after prosthetic repair of the abdominal wall [10]. It is certain that both in non-complicated inguinal hernia and in abdominal wall hernia repairs the use of antibiotics can reduce significantly the number of infections. So particularly in operations in which we think that it is possible that enteric bacteria have contaminated the operating field we should use wide spectrum antibiotics that protect against gram + and gram – bacteria. There is no convincing evidence to suggest that the new-generation Cephalosporins are more effective than first-generation [10]. Some Authors suggest in standard prosthetic repair single dose of ampicillin and sulbactam, others Authors first – second generation cephalosporine/amoxicillin and clavulanic acid and others single dose cephtriaxone. World Journal of Emergency Surgery 2008, 3:33 ...................... Hernia repair has been traditionally considered as one of the so-called clean operations along with thyroid and breast surgery. Results from recent prospective studies, however, suggest that the wound infection rate in elective hernia repair is underestimated and, when the patients are appropriately followed up, the figure approaches or even exceeds 10%.[2,7,8] This figure is certainly not acceptable for a clean operation and some authors have suggested that we may need to re-classify hernia repair as a clean-contaminated procedure. On the other hand, during the last decade, the use of tension-free mesh repair techniques has become increasingly popular worldwide and it is considered today as the method of choice for elective inguinal hernia repair. The fear of infection of an introduced foreign body such as a non-absorbable mesh, raised the question of the potentially protective role of antibiotic prophylaxis, as this has been the case with other clean operations, such as arthoplasties and vascular graft implants.[9,10] In the light of the aforementioned data, many surgeons use antibiotics in elective hernia repair with mesh, although this practice is empiric rather than evidence based.............................In conclusion, we were not able to demonstrate any significant benefit from the addition of antibiotic prophylaxis (consisting of a single dose of ampicillin and clavoulanic) in elective inguinal hernia tension-free repair using a polypropylene mesh, in patients who were not at high risk of developing septic complications. |

Results One hundred and four patients (3·3 per cent) declined to give a contact telephone number, leaving 3046 patients who agreed to take part in the study. Some 108 patients (3·4 per cent) could not be contacted at any point, giving a response rate of 93·3 per cent. Complete data were available for 2665 patients (87·5 per cent); 140 (5·3 per cent) developed SSI and 57 (2·1 per cent) thought the wound infected but this was not confirmed by the healthcare worker. Patients given a prophylactic antibiotic had a lower incidence of SSI (P = 0·002), but neither increase in the American Society of Anesthesiologists grade of fitness for operation nor prolonged duration of operation was a significant risk factor for infection. Conclusion SSI after hernia repair is common and large clinical trials are required to determine whether the use of prophylactic antibiotics reduces the incidence of infection. Думаю, что опять однозначно попадаю в меньшеньство. Ни за что бы не полез в эту драку если бы практика в моем госпитале (т.е. в том госпитале, где я работаю) не была бы на столько однозначна. Приоперационная доза антибиотика (обычно Anсef [cephazolin] 1 gm, IV) дается ВСЕМ пациентam не далее как 30 мин до начал операции. Невыполнение этого требования необходимо документировать отдельно.

Background: Antibiotic prophylaxis use in mesh inguinal hernioplasty is controversial. Available evidence is nonconclusive because of the low number of clinical trials assessing its effectiveness. Some trials have a small sample size that could overestimate or underestimate the real effectiveness of this intervention. Meta-analysis is a good method to improve these methodological flaws. Methods: Meta-analysis intended to measure the benefits of antibiotic prophylaxis on surgical site infection rate in adult patients scheduled for mesh inguinal hernioplasty. Six randomized clinical trials were found. Quality was assessed using Cochrane Collaboration criteria. Results: A total of 2507 patients were analyzed. Surgical site infection frequency was 1.38% in the antibiotic group versus 2.89% in the control group (odds ratio = 0.48; 95% confidence interval, 0.27–0.85). There was no statistical heterogeneity. Sensitivity analysis by quality did not show differences in overall results.
